Disclosures<br />
In accordance with FDA <strong>and</strong> ACCME guidelines, which are concerned with the balance, independence, objectivity, <strong>and</strong> scientific<br />
rigor of all accredited programs, <strong>ISCD</strong> requires all faculty to make full disclosure. Full disclosure entails indicating whether the<br />
faculty member <strong>and</strong>/or his/her immediate family has any relationships with pharmaceutical companies, biomedical device<br />
manufacturers, <strong>and</strong>/or other corporations whose products or services are related to pertinent therapeutic areas. All faculty<br />
participating in <strong>ISCD</strong> sponsored programs are expected to disclose to the audience any relationship(s) with companies who<br />
manufacture products having a direct bearing on the subject matter of the CME activity <strong>and</strong>/or relationship(s) between the speaker<br />
<strong>and</strong> commercial contributor of the activity. Additionally, faculty members are required to inform the audience when they are<br />
discussing off-label, unapproved uses of devices <strong>and</strong> drugs. Devices or drugs that are still undergoing clinical trials should be<br />
identified as such <strong>and</strong> should not be portrayed as st<strong>and</strong>ard, accepted therapy. In weighing the benefits of treatment against the risks,<br />
clinicians should be guided by clinical judgment. Any procedures, medications or treatments discussed in this program should not<br />
be used by clinicians without evaluation of their patient’s condition or possible contraindication or dangers in use, review of<br />
applicable product information <strong>and</strong> comparison with recommendations of other authorities.<br />
