Management
• Complex Chronic Condition and Disease Management is a
program to help our members better manage Chronic
Conditions. A registered nurse case manager will work with
members to help them learn skills to enable them to
effectively manage their condition.
• Case Management is a program that gives access to a
dedicated nurse professional that will work with members,
families and treating physicians when members are faced with
complex health issues.
