Executive Summary<br />

• Incident occurred on NH6 during intervention activities on Lambert 6<br />

on 10 th October 2010<br />

• Part of the production hydraulic riser spider (500Kg) dropped to the<br />

deck 300mm while being moved<br />

• No one injured – Tag lines in use<br />

• Incident categorised as HPH (Cat C)<br />

• <strong>Eye</strong> bolt pulled out of thread due to mismatch of thread diameters<br />

(14mm eyebolt in 16mm tapped hole)<br />

• Equipt was certified. <strong>Eye</strong> bolt had previously been visually<br />

inspected in situ but not removed from the equipment for more<br />

detailed “complete visual inspection”<br />
