The Fun Club is a before and after school childcare program<br />

operated by <strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District Office of<br />

Child Development.<br />

The Fun Clubs are funded solely by parent tuition fees.<br />

Each Fun Club must maintain sufficient attendance to<br />

financially support the full cost of operations. When a<br />

Fun Club fails to fully fund its operation, the District may<br />

close part or the entire program.<br />

The Fun Clubs are staffed with caring adults who meet the<br />

No Child Left Behind requirements and hold a minimum of<br />

six Early Childhood Education college-level units. Our Fun<br />

Clubs provide a safe and enriching environment where<br />

children can complete their homework, explore their own<br />

unique interests through enrichment activities, and receive a<br />

nutritious snack after school.<br />

The Office of Child Development offers assistance to lowincome<br />

families who meet eligibility guidelines at select Fun<br />

Club locations.<br />

Eligibility is based on income and need regulations<br />

established by the California Department of Education. If<br />

you believe you qualify for assistance, please contact the<br />

Office of Child Development for enrollment information.<br />

ENROLLMENT REQUIREMENTS<br />

The Fun Club program is a large, center-based childcare enrichment<br />

program, which serves grades K-6. It is important for you, the parent to<br />

partner with us before enrollment to ensure our program is right for you<br />

and your child.<br />

The enrollment contract will be in effect for the <strong>2013</strong>-<strong>2014</strong> <strong>School</strong><br />

Calendar - contracts must be renewed yearly.<br />

Children will be enrolled in the program on a first-come, first-serve basis<br />

depending upon space availability.<br />

• Child must reside in and/or be registered to attend school in the<br />

<strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District.<br />

• Child must be enrolled in Kindergarten through Sixth grade.<br />

Students from Other C.V.U.S.D. <strong>School</strong>s<br />

Students who attend school where our program is not on-site are eligible<br />

for childcare at the Fun Club of their choice under the following<br />

conditions:<br />

• <strong>Parent</strong>s must provide their own transportation.<br />

• An authorized adult must sign the child in and out.<br />

Each of our programs will follow the bell schedule of the elementary<br />

school where situated. When the child’s school of attendance has a<br />

different minimum day schedule, available childcare services may be<br />

limited.<br />

Enrollment Packets<br />

Enrollment packets are available at the Office of Child Development,<br />

during regular business hours or on our website at<br />

http://childdevelopment.chino.groupfusion.net<br />

ALL enrollment packets must be submitted in original form with original<br />

signatures and in person by the parent or legal guardian to the Office of<br />

Child Development. <strong>Parent</strong>s are required to complete the enrollment<br />

requirements listed below:<br />

1) Program Enrollment Contract<br />

2) Financial Obligation<br />

3) Child Safety Information (1 per child)<br />

4) Field Trip/Excursion Waiver & Medical Authorization– Minor<br />

5) Grounds for Suspension and Expulsion (K-Adult)<br />

6) Student Waiver of Liability, Assumption of Risk, and Indemnity<br />

Agreement.<br />

7) Homework Contract (1 per child, 1 st -6 th grade only)<br />

8) Current year photo (1 per child)<br />

9) <strong>Parent</strong> and Physician Request for Administration of Medication, if<br />

applicable<br />

10) Current court-validated child custody order, if applicable<br />

11) Payment of applicable enrollment fees - $50 per child or $75 per<br />

family, plus the first week’s tuition.<br />

12) Present current, valid picture identification.<br />

Enrollment fee and first week of tuition is NON-REFUNDABLE<br />

Childcare services will begin once all of the enrollment requirements are<br />

completed and approved by the Office of Child Development.<br />

A minimum of 72 hours BEFORE the child’s first day of attendance is<br />

necessary to process the enrollment packet.<br />

We reserve the right to deny enrollment when any part of the above<br />

enrollment requirements are not fully met.<br />

Sibling Discount Rate<br />

The Sibling Discount only applies to parents or guardians with legal<br />

custody of a child.<br />

We reserve the right to request documents establishing legal custody of<br />

the child(ren). 2nd child tuition (sibling discount rate) = lowest weekly fee.<br />

Enrollment Renewal<br />

Currently enrolled families are given priority enrollment for the<br />

succeeding contract year.<br />

Enrollment of any kind, including priority enrollment will not be extended<br />

to any family who does not present current, valid picture identification or<br />

who has a current balance due on the account and/or prior history of<br />

delinquent, returned or late payments.<br />

Children with Special Needs<br />

Our program welcomes the enrollment of children with disabilities as<br />

defined by the Americans with Disabilities Act [ADA] and the Individuals<br />

with Disabilities Act [IDEA]. Prior to an offer of enrollment into the<br />

program, the parent is required to:<br />

a) Visit the on-site program facility and meet with the program<br />

administrator and the site teacher to review and develop a plan for<br />

care.<br />

b) Periodic update of the plan will be conducted as needed.<br />

Please note: Full disclosure of a child’s special medication, mental<br />

health, behavioral, developmental and/or learning needs is essential to<br />

provide safe provisions for the child while in our care. Failure on the part<br />

of the parent to fully disclose the nature and extent of the child’s special<br />

needs may render your contract with the Fun Club void.<br />

Child Custody<br />

Program staff must enforce court-validated Child Custody and/or<br />

Restraining Orders.<br />

A current court-validated copy of the Child Custody and/or Restraining<br />

Order is needed to ensure the proper enforcement of said order[s] by<br />

program staff. Child Development and/or program staff cannot enforce<br />

orders that have expired nor can they modify an order based on a verbal<br />

request of a parent<br />

Bus Riders<br />

In cases where a child arrives by bus to Fun Club, the parent is solely<br />

responsible to arrange for the transportation and delivery of the child into<br />

the Fun Club facility.<br />

Fun Club staffs are not permitted to make special arrangements to pick<br />

up a child from a bus drop-off location. It will be the parent’s sole<br />

obligation to provide a person to escort the child into the facility, and sign<br />

in on the child’s Attendance Sheet in accordance with our sign-in policy.<br />

Dress Code<br />

Fun Club will enforce District and school site dress code rules.<br />

NO OPEN-TOED SHOES ARE PERMITTED AT ANY TIME.<br />

When a child does not comply with dress code, we reserve the right to<br />

prohibit your child’s participation in center activities. In some cases of<br />

inappropriate dress, it may be necessary to contact the parent to pickup<br />

the child and/or bring an appropriate change of clothing.<br />

Homework Assistance<br />

We provide a one-hour homework assistance period for children in<br />

grades 1-6, utilizing teachers and instructional aides. Fun Club IS NOT<br />

a homework completion program. This means that child and parent<br />

maintain full responsibility for accurate homework completion.<br />

When a parent prefers their child not participate in homework, written<br />

parental permission is required. When the child is excused from<br />

participating, the child will work on an enrichment packet provided by<br />

Fun Club teacher.<br />

Fun Club homework assistance policies are as follows:<br />

• Homework is only spot-checked for accuracy and neatness by a<br />

teacher or instructional aide; the parent is responsible to thoroughly<br />

check their child’s homework for accurate completion each evening.<br />

• One hour is provided for homework after which a child may voluntarily<br />

choose to continue working; however, assistance of a teacher or aide<br />

is very limited due to the schedule of activities.<br />

• Fun Club teacher may consult with the child’s school teacher to clarify<br />

questions or assignments; Fun Club maintains full support of teachers<br />

and the school day curriculum.<br />

• Children are not permitted to return to their classroom for any reason.<br />

• To assist all children equally with homework, Fun Club staffs do not<br />

provide one-on-one homework assistance.<br />

Kindergarten Homework Policy<br />

Fun Club does not provide a homework period for kindergarteners due to<br />

the high level of parental involvement needed for school success in the<br />

child’s first year.<br />

TUITION PAYMENTS<br />

Tuition must be paid in advance.<br />

All tuition payments are due on the Thursday<br />

before the upcoming week of childcare, whether<br />

the child is present or not.<br />

Tuition fees will not be waived for any<br />

unexpected absences regardless of circumstances.<br />

The Fun Club and the Child Development staff are not responsible for<br />

reminding parents of their obligation to keep their account current and to<br />

make their timely tuition payments. <strong>Parent</strong>s are responsible to ensure<br />

their account remains current at all times. Allowing your account to<br />

ecome one week past due will be grounds for immediate termination<br />

from the program.<br />

The Fun Club and the Office of Child Development DO NOT<br />

honor postdated checks. All checks are deposited on the day<br />

of receipt.<br />

• Make checks or money orders payable to CVUSD or <strong>Chino</strong> <strong>Valley</strong><br />

<strong>Unified</strong> <strong>School</strong> District.<br />

• Only preprinted checks will be accepted.<br />

Cash payments are ONLY accepted at the Office of Child<br />

Development. The Fun Club does not accept cash.<br />

For the weeks of Thanksgiving, Christmas Break, and Spring Break,<br />

the tuition must be paid by the Thursday before the break as<br />

payment for the week Fun Club resumes.<br />

Tuition will be considered late if paid on the day Fun Club resumes<br />

operation.<br />

Tuition fees will not be waived for days a child does not attend Fun<br />

Club for any unexpected absences regardless of circumstances.<br />

All weekly tuition fees must be paid in full. When the weekly tuition<br />

fee is not received in full by Thursday, the account will be<br />

automatically charged a $25 late payment fee.<br />

Tuition may be paid by check or money order at the Fun Club or<br />

Office of Child Development.<br />

WE STRONGLY DISCOURAGE SENDING YOUR TUITION PAYMENT<br />

WITH YOUR CHILD. Your child’s failure to turn in your tuition payment<br />

will not prevent a late payment fee from being assessed to your account.<br />

Automatic Payments<br />

At this time we do not accept credit card payments.<br />

We do accept bank issued checks through your banks bill pay.<br />

Include your child’s name in the comment section and allow 3-4 business<br />

days for processing.<br />

Late Payment<br />

Tuition payments received after Thursday will be considered late<br />

regardless of the reason.<br />

A late payment fee of $25 will be automatically assessed to your<br />

account.<br />

<br />

Missing one week’s tuition payment may warrant immediate<br />

termination from the program. Only Money Order or Cashier Check<br />

will be accepted as payment for accounts that are one week behind<br />

payment.<br />

The accumulation of five (5) late payment fees within a contract year is<br />

deemed excessive and will be grounds for termination.<br />

Account in Arrears: If your account becomes one week behind, YOUR<br />

CHILD MAY BE TERMINATED FROM THE FUN CLUB.<br />

NSF (Non-Sufficient Funds)<br />

A $20 processing fee will automatically be charged to your account for all<br />

returned checks. Only cash, Cashiers Check or Money Order will be<br />

accepted as payment for any returned checks.<br />

A pending balance caused by a returned check warrants immediate<br />

termination from the program.<br />

Two returned checks within a contract year warrants immediate<br />

termination from the program.<br />

Additional Fees<br />

Additional charges may be assessed to your account based on your<br />

child’s attendance sheets. (i.e. your child was signed up to attend 4-<br />

days after school only, but the sign in sheet reflects attendance of 4-days<br />

before and after school, charges on your account will be adjusted<br />

accordingly). You will be financially responsible for any and all additional<br />

fees that are applied to your account.<br />

Dual Custody<br />

<strong>Parent</strong>s with dual custody who split the tuition payments must keep both<br />

accounts current in order to avoid a loss in child care services. It is the<br />

obligation of each party to work together to ensure their account remains<br />

current. It is not the responsibility of the Office of Child Development to<br />

settle payment disputes between parents.<br />

We strongly encourage weekly tuition fees be paid by one financially<br />

responsible party. In rare cases when each parent pays a portion of the<br />

designated tuition fees, each parent must sign the Financial Obligation<br />

form. Failure of one parent to make payment or to keep their portion of<br />

the account current may result in termination of the childcare contract.<br />

If the weekly tuition fee is not paid in full by the payment deadline, a Late<br />

Payment Fee of $25 will be charged to the account. Both parties are<br />

financially responsible for any additional fees assessed on the account.<br />

Collections/Past Due Accounts<br />

Any party that signs the Financial Obligation Form is financially<br />

responsible for any balances on the account.<br />

Failure to pay your account balance within 90 days of the termination<br />

date may result in referral to our Collection Agency, The Outsource<br />

Group.<br />

When a contract is terminated for current or prior history of delinquent,<br />

late, or returned payments any future enrollment will be at the sole<br />

discretion of the Office of Child Development, and will be contingent<br />

upon paying $100 cash deposit per family. This deposit will be returned<br />

when the child exits the program and once the account is paid in full.<br />

Payments by check will NOT be allowed for future tuition payments.<br />

It is not the responsibility of Fun Club or Child Development office<br />

staff to inform or remind parents of their obligation to keep the<br />

account current.<br />

Customer Statements<br />

You can access your Fun Club account information anytime, online by<br />

visiting https://parents.kinderstreet.com. When logging on for the first<br />

time, please contact the Office of Child Development to activate the<br />

account.<br />

State/County Agency Tuition Reimbursement<br />

<strong>Parent</strong>s who qualify for assistance from the state/county agencies are<br />

eligible for enrollment in the Fun Clubs under the following conditions:<br />

• Proof of assistance is provided at the time of registration.<br />

• A security deposit of $100 (this deposit will be reimbursed at the time<br />

of exit from the program, provided the state/county pays for these<br />

fees).<br />

• <strong>Parent</strong>s are responsible for all payment of fees that the state/county<br />

agency does not pay.<br />

• <strong>Parent</strong>s are responsible for calling the case worker for the adjustment<br />

of unpaid fees after adjustments were submitted and denied.<br />

<strong>Parent</strong>s enrolled in a county program on a variable work schedule do not<br />

qualify for enrollment in the Fun Club.<br />

Changes that would affect the third-party reimbursements to the <strong>Chino</strong><br />

<strong>Valley</strong> USD Office of Child Development require a Change of Contract to<br />

be submitted by the parent to the school site by the deadline.<br />

Continuing enrollment in the Fun Club is contingent on the parent<br />

completing their attendance sheets DAILY as required by the county<br />

agencies.<br />

Copayment is due prior to and no later than the last business day of<br />

each month at the Fun Club.<br />

<strong>Parent</strong>s are responsible to communicate and ensure that the Fun<br />

Club staff forwards the completed attendance sheet and the copayment<br />

(as stated in the parent’s county’s Certificate).<br />

SIGN-IN/SIGN-OUT PROCEDURES<br />

The Terms and Conditions of your enrollment contract require that an<br />

authorized person must enter the Fun Club to sign the child in and out<br />

each day. <strong>Parent</strong>s are required to provide current, valid picture<br />

identification whenever requested by Fun Club staff.<br />

PERSONS ALLOWED TO PICK UP A CHILD<br />

We will only release children to adults, 18 years or older, who are listed<br />

on the child’s Emergency Information. Verbal requests to release a child<br />

to a person not listed on the current Emergency Information will not be<br />

honored.<br />

Persons authorized by the parent to pickup or drop-off the child must<br />

record the arrival/departure time, and must sign with a full signature on<br />

the student attendance sheet.<br />

Failure to comply with the Sign-In / Sign-Out procedure may warrant<br />

termination.<br />

After-<strong>School</strong> Activities<br />

Once your child is signed into Fun Club, your child’s safety and welfare<br />

become our responsibility. We reserve the right to prohibit your child<br />

from exiting the Fun Club to participate in an after-school activity. This<br />

includes helping the teacher in the classroom after school. Each center<br />

shall have the right to establish their own site policy regarding<br />

participation in after school-activities.<br />

<strong>Parent</strong>s are urged to check with their Fun Club site staff regarding the<br />

site policy before making after school plans for their child.<br />

Missing-Child Procedure<br />

FOR YOUR INFORMATION ONLY: The Fun Club staff uses the<br />

following procedures to locate a missing student who is scheduled to<br />

attend Fun Club. These procedures are provided for parent information<br />

only.<br />

Within the first 30 minutes the Fun Club staff will:<br />

1. Check the sign-in/sign-out sheet for staff notes or for contract<br />

changes.<br />

2. Check with Fun Club staff and/or message pad for changes or calls.<br />

3. Ask other students if they have seen the child. If yes, then: Call the<br />

child’s teacher AND the school office. Did the child attend school that<br />

day? Did the parent sign student out early? If child attended school<br />

and was NOT signed out early, go to Step 4.<br />

4. Go to the school office to request an All-Call on campus.<br />

5. Check in the child’s classroom/look around on campus. Inform<br />

school principal that the child is missing.<br />

6. Call BOTH parents at work or at home. If unable to make direct<br />

person-to-person contact, leave a message. THEN:<br />

7. If parent/s confirm that they DO NOT KNOW where the child is, OR if<br />

there is no response within 30 minutes of starting Steps 1-6, call<br />

District Security.<br />

8. Be prepared with a copy of the child’s emergency card including the<br />

picture.<br />

9. Contact the Child Development Coordinator or Director.<br />

Late Pick-Up<br />

The Fun Clubs close promptly at the posted time. Failure to arrive<br />

BEFORE the closing time will result in a Late Pick-Up violation and fee.<br />

LATE PICKUP IS DEFINED AS ANY TIME AFTER CLOSING.<br />

THE CENTER CLOCK WILL BE THE ESTABLISHED METHOD OF<br />

TIME KEEPING.<br />

If you know you will arrive late, please call to speak with a Fun Club<br />

program staff member immediately to alert them. While this will not deter<br />

the ensuing consequences for the late pick-up nor eliminate the<br />

applicable fee, it will alleviate any distress your child may experience<br />

from waiting for you.<br />

It is strongly recommended parents identify a local, reliable backup<br />

person to pickup for the RARE situations such as mechanical<br />

breakdown, heavier-than-usual traffic, unusual weather, etc. This<br />

individual needs to be on your Emergency Information form as an<br />

authorized pickup person.<br />

A written violation of late pick-up will be issued to the parent at the time<br />

of pick-up. The late fee will automatically be assessed to your account<br />

and is due and payable immediately.<br />

Failure to pay the late fee within 5 days will result in termination from the<br />

program.<br />

The following consequences will result for late pick-up violations:<br />

FIRST VIOLATION: Verbal and written late pick-up violation issued to<br />

parent.<br />

SECOND VIOLATION: Written late pick-up violation and $20 late pick up<br />

fee to be paid within 5 days of the late pick-up.<br />

THIRD VIOLATION: Written late pick-up violation, a $35 late pick up fee<br />

to be paid within 5 days of the late pick-up; and child may not return<br />

until the parent meets with the program coordinator and/or the director.<br />

FOURTH VIOLATION: Immediate termination from the program.<br />

In the event the child is not picked up by 7:00 p.m., it may be<br />

necessary for program staff to contact the local police department.<br />

ABSENCE NOTICE<br />

When your child will be absent from the program for a full week, you may<br />

receive tuition credit by using the Absence Notice form.<br />

“Full week” as used in this section = Monday through Friday<br />

DEADLINE: Monday BEFORE the upcoming week of absence.<br />

Absence Notices received after the deadline will not be honored.<br />

Tuition fees will not be waived for any unexpected<br />

absences, regardless of circumstances.<br />

The Absence Notice form is available at the Fun Club site, the Office of<br />

Child Development, or on the Child Development webpage. When the<br />

Absence Notice is received by the deadline, your account may be eligible<br />

for tuition credit on the following conditions:<br />

There is a limit of two (2) weeks per contract year.<br />

Absence Notice received IN WRITING on or before the Monday<br />

deadline, and is completed in full and signed by parent. Verbal<br />

notices will NOT be accepted.<br />

Absence Notice is for a full week (Monday-Friday).<br />

Absence Notice does not exceed the two maximum allowed per<br />

child, per contract year.<br />

An Absence Notice will not be honored in lieu of or in combination<br />

with the Contract Termination in an effort to avoid the two full weeks’<br />

advance notice required to terminate your contract.<br />

CHANGE OF CONTRACT<br />

To alter an existing Enrollment Contract, a written Change of Contract is<br />

required. Verbal notices will NOT be accepted. A fee of $25 will be<br />

required for each Change of Contract. A $20 late fee will apply to all<br />

Change of Contracts received after the deadline.<br />

DEADLINE: Monday BEFORE the upcoming week of requested change.<br />

A Change of Contract is needed to:<br />

• To Add/Cancel Day(s): A Change of Contract is required to add or<br />

delete days from your child’s program (i.e. your child attends 3 days<br />

after school and you now need 5 days after school).<br />

• To Add Unplanned/Extra Minimum Days: A Change of Contract is<br />

required to temporarily change your child’s program due to<br />

unplanned/extra minimum days, including, but not limited to, <strong>Parent</strong><br />

Conference Week.<br />

• To Change Days of Attendance: A Change of Contract is required<br />

to change your child’s contracted days of attendance (i.e. your child<br />

attends Monday, Tuesday and Thursday and you now need them to<br />

attend Wednesday, Thursday and Friday).<br />

• To Transfer Programs: A Change of Contract is required to transfer<br />

to another program location. If there is no space at the requested<br />

program location, the parent will be placed on the Wait List effective<br />

the day of the transfer request. Currently enrolled families do NOT<br />

have priority for enrollment at other Fun Clubs.<br />

• To Add a Sibling: A Change of Contract is required to add a sibling<br />

to your contract, in addition to a Child Safety Information and<br />

Homework Contract. Applicable enrollment fees may apply.<br />

All Change of Contracts must be approved by the Office of Child<br />

Development BEFORE the requested change takes effect. <strong>Parent</strong>s will<br />

be notified in writing if and when a Change of Contract is not approved.<br />

The Change of Contract form is available at the Fun Clubs, the Office of<br />

Child Development, or the Child Development webpage.<br />

EMERGENCY DAY<br />

An Emergency Day is any non-contracted day or time of attendance. The<br />

Emergency Day fee is $35 per child, per Emergency Day.<br />

Example 1:<br />

If your child’s contracted days of attendance are Monday<br />

and Tuesday, but they come on Wednesday also;<br />

Wednesday would be deemed an Emergency Day.<br />

If your child is contracted for 5-days after school only,<br />

but they come to Fun Club before school on Monday;<br />

Monday before school would be deemed an Emergency<br />

Day.<br />

<strong>Parent</strong>s MUST contact the Fun Club PRIOR to using an Emergency Day.<br />

There is no limit on how many Emergency Days you can use during the<br />

contract year.<br />

<br />

A child who shows up at the Fun Club without prior notification will<br />

be escorted to the school’s front office.<br />

TERMINATION OF ENROLLMENT CONTRACT<br />

To terminate your child’s enrollment at the Fun Club, a Termination of<br />

Enrollment Contract must be completed.<br />

DEADLINE: Monday before the upcoming week of termination, two full<br />

weeks in advance of final attendance date.<br />

Notice of termination must be submitted in writing. Verbal notices will<br />

NOT be accepted.<br />

A Termination of Enrollment Contract is available at the Fun Club, the<br />

Office of Child Development, or the Child Development webpage.<br />

The final two (2) weeks of attendance must be paid at your contracted<br />

tuition rate.<br />

An Absence Notice will not be honored in lieu of or in combination<br />

with the Contract Termination in an effort to avoid the two full weeks’<br />

advance notice required to terminate your contract.<br />

Should you leave the program without giving the required full two weeks’<br />

written notice, your account will automatically be charged for the full two<br />

(2) weeks’ tuition.<br />

Failure to pay these fees may result in your account being sent to our<br />

collection agency, The Outsource Group, and this could negatively affect<br />

your credit rating. Additionally, future enrollment in any Child<br />

Development program operated by <strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District<br />

is subject to denial.<br />

Re-enrollment<br />

A family who drops from the program and wishes to re-enroll during the<br />

same contract year will be required to complete a re-enrollment form and<br />

pay a non-refundable $25 re-enrollment fee provided space is available<br />

to accommodate the child.<br />

Re-enrollment will not be honored to a family who has a current balance<br />

due on their account and or prior history of delinquent, returned or late<br />

payments.<br />

INFORMATION POLICY<br />

Account information will only be released to the parties listed as the<br />

primary and secondary sponsors on the enrollment contract.<br />

A valid picture ID will be required prior to the release of any records.<br />

A minimum of $10 fee will be charged for copies of student’s attendance<br />

sheets, receipts, and/or statements.<br />

Personal Information<br />

In the interest of child safety and welfare, it is critical that Fun Club staffs<br />

are able to make immediate contact with the authorized adult in the<br />

event of injury or illness to the child. It is the obligation of the parent to<br />

update the personal information contained on the child’s safety<br />

information whenever there is a change.<br />

ILLNESS OR INJURY<br />

The <strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District does not provide medical<br />

insurance coverage for accidents that may occur during child care.<br />

<strong>Parent</strong>s and/or legal guardians are responsible for the medical bill(s)<br />

incurred by an injured child. Student accident/health insurance is<br />

available for a fee. You may contact the <strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong><br />

District Office of Risk Management for a brochure.<br />

To ensure that all children and staff are protected from the spread of<br />

infectious disease and/or contagious illness, the Fun Clubs will follow<br />

standardized health procedures established by CVUSD Health Services.<br />

We reserve the right to seek consultation from a school nurse and/or<br />

District Health Services to ensure proper prevention procedures are<br />

followed.<br />

• Fun Club staff has the authority to refuse a child with obvious signs of<br />

illness.<br />

• If symptoms of illness become apparent during the day, prompt<br />

contact will be made with parent or guardian for immediate pickup of<br />

the child.<br />

• In cases where contact with the child’s parent or guardian is<br />

unsuccessful, Fun Club staff will contact the authorized adults on the<br />

child’s emergency card.<br />

• Once contact is made, the child must be picked up WITHIN ONE<br />

HOUR. Failure to do so may result in suspension from Fun Club.<br />

When Fun Club staff is unable to make contact with an authorized adult<br />

listed on the Child Safety Information form due to inadequate or incorrect<br />

information supplied by parent, we will exclude the child from childcare<br />

until the Child Safety Information form is updated with current, correct<br />

contact names and phone numbers.<br />

YOUR CHILD WILL NOT BE ALLOWED TO ATTEND FUN CLUB IF:<br />

• There are signs of communicable illness.<br />

• Your child has a high fever, vomiting, diarrhea or a bad cough.<br />

• Your child has had a high fever within the last 24-hour period.<br />

• Your child is too sick or uncomfortable to participate in typical Fun<br />

Club activities.<br />

• Your child either did not attend school due to illness, or your child was<br />

sent home from school due to illness.<br />

• Your child becomes ill or injured while at Fun Club.<br />

In the event a child requires immediate medical attention, Fun Club staff<br />

will call 911.<br />

Medication<br />

Fun Club staff MAY NOT dispense any medication without a doctorcertified<br />

<strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District “<strong>Parent</strong> and Physician<br />

Request for Administration of Medication” form.<br />

This includes any product with active ingredients such as, cough drops,<br />

inhalers, vitamins, Tylenol, aspirin, Binaca, Listerine mouth spray or<br />

wash.<br />

All medications must be in the original pharmacy prescription<br />

container. A child may not carry medication on their person at the<br />

Fun Club without valid written permission by the child’s doctor.<br />

In the event you do not have the CVUSD medication form with you at the<br />

time of a doctor visit, we will accept the doctor’s written orders on the<br />

doctor’s prescription pad.<br />

<strong>Parent</strong> will be required to sign the “<strong>Parent</strong> and Physician Request for<br />

Administration of Medication” form and doctor’s written orders will be<br />

attached.<br />

NOTE: Medication forms expire annually on June 30—new authorization<br />

is required on July 1.<br />

Child Safety Information<br />

Our Fun Clubs are large, center-based childcare and enrichment<br />

programs, serving grades K through 6. Portions of our program may not<br />

be suitable for every child. It is important for you, the parent, to partner<br />

with our Office before enrollment to ensure that our program is right for<br />

your child. To help ensure your child’s success and happiness, please<br />

be aware of the following:<br />

1. Our average adult-to-child ratio is 1 adult to 15 children; most of our<br />

programs average 50 – 70 children.<br />

2. There is a one-hour structured homework period after school.<br />

Students work independently on homework or an approved quiet<br />

activity.<br />

3. Child-selected or teacher-directed activities comprise the remainder<br />

of the daily schedule.<br />

4. To be successful, a child should be able to self-select an activity of<br />

interest, happily complete the activity with some independence,<br />

follow simple directions from adults, independently engage in free<br />

play, and participate in teacher-directed/group activities as<br />

scheduled.<br />

5. Our program could be unsuitable when a child requires a highly<br />

structured setting.<br />

6. Our program does not have adequate staff to permit pickup/drop-off<br />

of the child to a bus or classroom outside of the immediate perimeter<br />

of the Fun Club building.<br />

7. ALL children must adhere to the Program’s Discipline Policy and<br />

standards for appropriate behavior.<br />

PERSONAL ARTICLES<br />

Children are prohibited from bringing large sums of money or valuable<br />

items to the Fun Club. This includes, but is not limited to-Toys, Games,<br />

Audio Devices, Ds, DVDs, or other listening/viewing materials. The Fun<br />

Club staff and CVUSD are not responsible for any items that may be lost,<br />

stolen, or broken.<br />

Telephone Usage<br />

Students are not allowed to use the site telephone for outgoing calls.<br />

<strong>Parent</strong>s are strongly discouraged from regularly calling the Fun Club to<br />

speak to their child. If a parent has an extreme need to speak to their<br />

child directly, Fun Club staff will restrict the duration of the call to less<br />

than two minutes.<br />

Telephone calls to students at the Fun Club are restricted to<br />

emergencies only.<br />

Cell Phone Use<br />

In accordance with CVUSD Board Policy, students are permitted to carry<br />

cell phones on school campuses.<br />

• Students may only use cell phones BEFORE SCHOOL, AFTER<br />

SCHOOL, or DURING LUNCH WHEN OUTSIDE, and cell phones<br />

shall be turned off during class time and any other time they are<br />

directed to do so by a District employee.<br />

• Cell phones shall not be used during class as a calculator and/or for<br />

text messaging.<br />

• Cell phones shall not disrupt the educational program or school<br />

activity.<br />

If a disruption occurs, the employee shall direct the student to turn off the<br />

device and/or confiscate it. If a school employee finds it necessary to<br />

confiscate a device, he/she may either return it at the end of the day or<br />

keep it until the teacher has consulted the student’s parent/guardian.<br />

CVUSD Board Policy 5131(c), Student Conduct.<br />

SNACK PROGRAM<br />

A nutritious daily snack is offered before and after school. Please inform<br />

your Fun Club teacher of any food allergies your child may have<br />

BEFORE your child’s first day of enrollment.<br />

• Children will not be forced to eat by Fun Club staff.<br />

• A SNACK IS NOT A MEAL. Pre-packaged, healthy snacks may be<br />

brought from home; however, we reserve the right to determine<br />

nutritional value.<br />

• Our Fun Club program follows the nutrition guidelines set forth by the<br />

<strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District Board Policy.<br />

Food that has been prepared or stored at a private home will not be<br />

served at our facility. Monthly snack menus are posted at each Fun Club.<br />

Children are responsible for how much and whether they eat.<br />

PETS AT THE PROGRAM<br />

Board Policy 1330(a) and Administrative Regulation 1330(a)<br />

Except for police dogs, seeing-eye dogs, and dog obedience classes<br />

sponsored by County or City organizations, the <strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong><br />

<strong>School</strong> District strictly prohibits animals on District property. Violation of<br />

this policy is punishable under California Penal Code VC 21113.<br />

EMERGENCY DISASTER PLAN<br />

In case of an emergency, disaster, or crisis, all children will be kept at<br />

their individual school sites until an adult is able to pick them up. As<br />

telephone contact may not be possible, parents are urged to come to<br />

school immediately and leave phone lines available for emergency<br />

personnel. Emergency supplies are available at the schools. The school<br />

Emergency Plan is located at your Fun Club site. Please contact your<br />

Fun Club teacher if you wish to review the plan.<br />

ADULT BEHAVIOR CODE<br />

All authorized adults are expected to model appropriate behavior in their<br />

day-to-day relationships with Fun Club staff. It is understandable that<br />

some situations may arise and a parent may get upset or concerned.<br />

This does not excuse maltreatment or inappropriate behavior toward a<br />

Fun Club staff member. A display of inappropriate behavior in front of the<br />

children and staff is unacceptable.<br />

UNDER NO CIRCUMSTANCES WILL OFFENSIVE, RUDE, OR<br />

MALICIOUS ACTIONS, PROFANITY OR YELLING BE TOLERATED<br />

TOWARD ANY FUN CLUB STAFF. CHILD CARE MAY BE<br />

IMMEDIATELY TERMINATED WHEN AN ADULT COMMITS THESE<br />

ACTS TOWARD FUN CLUB STAFF.<br />

When adult behavior is deemed inappropriate, threatening, or dangerous<br />

by Fun Club staff or administration, the child will be immediately<br />

suspended from Fun Club. Return to child care will be considered once<br />

a meeting is held with the parent and administration.<br />

When an adult abuses the Adult Behavior Code, we reserve the right to<br />

ban that individual from Fun Club property and/or terminate child care for<br />

failure to follow the Adult Behavior Code.<br />

Board Policy 1250, Education Code Section 32210-33211, and Penal<br />

Code Section 627.7 prohibit the disruption and/or interference of<br />

educational programs by any individual. These policies and legal codes<br />

will be enforced.<br />

CHILD DISCIPLINE POLICY<br />

Every child has the right to a safe, healthy and stress-free learning<br />

environment while attending Fun Club. To ensure this right for every<br />

child, standards for student behavior are implemented at each Fun Club.<br />

Every child must follow the Fun Club standards for acceptable behavior.<br />

The <strong>Chino</strong> <strong>Valley</strong> <strong>Unified</strong> <strong>School</strong> District Behavior Code and school rules<br />

for acceptable student conduct are also enforced.<br />

When child behavior is deemed inappropriate by a staff member,<br />

depending on the severity of the behavior, the teacher will report the<br />

incident to the parent. Minor incidents may be reported verbally or may<br />

be handled without the need to report to the parent. Major incidents will<br />

be reported in written form on an Incident Report form. The Incident<br />

Report must be signed by parent to acknowledge they received and<br />

understand the facts surrounding the incident.<br />

• If a parent refuses or fails to sign an Incident Report, it does not affect<br />

the facts or documentation surrounding the incident.<br />

• Some inappropriate behavior may warrant suspension or termination.<br />

• Serious cases may warrant immediate suspension or termination.<br />

• When a decision to suspend or terminate is necessary, the decision<br />

may not be rendered immediately; it is our duty to thoroughly<br />

investigate prior to rendering a decision of this magnitude.<br />

• When a decision is made to terminate the child, the parent will receive<br />

a ten-day notice to give ample time to make other child care<br />

arrangements.<br />

Tuition credit IS NOT issued for the day of suspension—this is part<br />

of the logical consequences for the child’s inappropriate behavior.<br />

When a child continues in a pattern of inappropriate behavior and/or<br />

when a child causes intentional injury to another person, termination<br />

from child care may be necessary.<br />

If a child is suspended from the school day, the child is ineligible for child<br />

care on the day of suspension.<br />

WAIT LIST<br />

When childcare space is not available at a program, families will be<br />

placed on a wait list in the order in which they contact the Office of Child<br />

Development. Please note: families who require full-time childcare will<br />

receive priority over those who require part-time childcare. When space<br />

becomes available, families will be contacted by telephone and/or mail<br />

and will have five (5) days to contact the Office of Child Development.<br />

Failure to contact the Office of Child Development within the five-day<br />

period will result in the family’s removal from the wait list.<br />

Open enroll begins March 1 st . Space is offered on first come, first serve<br />

basis when a program becomes full a wait list will be started.<br />

<strong>Parent</strong>al request for transfer to another program will be based on space<br />

availability. If no space is available at the requested program, the parent<br />

will be placed on the wait list effective the day of the transfer request.<br />

Currently enrolled families DO NOT have priority for enrollment at other<br />

programs. In addition, priority to re-enroll is NOT given to families once<br />

they terminate their contract. <strong>Parent</strong>s wishing to re-enroll in a program<br />

with an existing wait list will be placed on the wait list in the order of their<br />

notifying the Office of Child Development IF the office is taking names for<br />

the wait list at the time of the parent’s call. The parent will be contacted<br />

when or if space becomes available.<br />

Special Needs Child: A child, who has been determined eligible for early<br />

intervention services pursuant to the California Early Intervention Services Act<br />

(Title 14 commencing with Section 95000 of the Government code) and its<br />

implementing regulations, and children 3 years of age and older who have been<br />

determined to be eligible for special education and related services by an<br />

individualized education program team according to the special education<br />

requirements contained in Part 30 (commencing with Section 56000), and<br />

meeting eligibility criteria described in Section 56026 and Sections 56333 to<br />

56338, inclusive, and Sections 3030 and 3031 of Title 5 of the California Code of<br />

Regulations. These children shall have an active IEP or IFSP, and be receiving<br />

early intervention services or appropriate special education services. These<br />

children, ages birth to 21 years, inclusive, may be autistic, developmentally<br />

disabled, hearing impaired, speech impaired, visually impaired, seriously<br />

emotionally disturbed, orthopedically impaired, otherwise health impaired, multihandicapped,<br />

or children with specific learning disabilities, who require the<br />

special attention of adults in a child care setting.<br />
