All completed and duly signed application/nomination form should be submitted on/before 16 May <strong>2013</strong> by fax (852 25374426)<br />
or email to efma<strong>2013</strong>@hkifm.org.hk. 所 有 填 妥 之 申 請 / 提 名 表 格 必 須 於 <strong>2013</strong> 年 5 月 16 日 或 之 前 傳 真 至 852 2537 4426 或 電 郵 至<br />
efma<strong>2013</strong>@hkifm.org.hk。<br />
All nomination documents/materials and a non-refundable administration fee of HK$2,500 for each nomination/application<br />
should be sent to “The HKIFM – Suite 2102, Connaught Commercial Building, 185 Wanchai Road, Hong Kong” no later than 5:00<br />
p.m. 17 June <strong>2013</strong> (Monday). 所 有 提 交 文 件 及 不 獲 退 還 之 行 政 費 用 (HK$2,500) 須 於 <strong>2013</strong> 年 6 月 17 日 ( 星 期 一 ) 下 午 五 點 前 遞 交 香 港 設 施 管<br />
理 學 會 ( 地 址 : 香 港 灣 仔 道 185 號 康 樂 商 業 大 廈 2102 室 ) 。<br />
Name of Facility and Applicant should be the full title as would appear on any award trophy/certificate. Any change is not<br />
allowed after submission of documents/materials. 設 施 名 稱 及 參 賽 者 的 名 稱 將 印 於 獎 項 上 , 已 遞 交 之 文 件 不 能 轉 換 。<br />
Only one category is allowed for one facility. 每 個 設 施 項 目 只 可 申 請 參 與 一 個 獎 項 類 別 。<br />
Separate forms for individual entry is required. 每 張 申 請 表 只 供 一 個 獎 項 類 別 之 申 請 。<br />
Relevant information such as charts, photos or statistics, etc. can be attached as Appendix with this application/nomination. 其 他<br />
有 關 文 件 如 圖 表 、 相 片 、 數 據 等 資 料 文 件 可 作 為 提 交 文 件 之 附 件 。<br />
The HKIFM may consider to disqualify the application if the required materials under the <strong>Application</strong>/Nomination Guidelines are<br />
not submitted. 如 申 請 者 未 能 提 交 所 需 之 文 件 , 香 港 設 施 管 理 學 會 有 權 取 消 其 參 賽 資 格 。<br />
The Grand Award Winners shall attend an experience sharing session in 2014. 卓 越 大 獎 得 獎 者 必 須 出 席 2014 年 舉 辦 之 分 享 會 。<br />
