Lee Memorial Health System Services
Keep You Out of the Hospital
Many people end up in the hospital because of a lack of communication and coordination of care. Patients often do not fully
understand what they have to do to maintain their health. Teamwork and communication can keep patients safe and healthy
at home—right where they want to be. Lee Memorial Health System has created a number of programs and services to help
patients avoid hospital stays, including:
House Calls
For chronically ill, medically fragile, elderly or homebound
patients unable to get to the physician’s office, the House Calls
program provides the care they need for recovery. Patients are
visited in their homes and medical needs are assessed. Referrals
for other services—like home health or physical or occupational
therapy—are made, if needed.
Lifeline
Lifeline Medical Alert Service provides independence and peace
of mind for older adults. Help is available with just the press of a
button, 24 hours a day. Simply press your personal help button
and you are connected to one of our caring and highly trained
staff via the two-way voice communicator. Depending on the
help you need, we can call a friend or a neighbor or dispatch
local emergency services.
Telehealth
Telehealth provides daily monitoring of patients. Information
from a scale, a blood pressure cuff and a pulse oximeter is
transmitted daily to a central monitoring station where it is
reviewed by registered nurses to determine if additional care
is required. The Telehealth program combines technology and
high-quality medical attention to provide patients the care they
require right in their own home.
Geriatric Care Management CHOICES
Geriatric Care Management CHOICES is the Lee Memorial Health
System program that enables seniors to remain independent
and in their own home while providing peace of mind to
adult children who live throughout the country. Geriatric Care
Management CHOICES licensed professionals will:
• Accompany client to physician appointments and report to
families
• Develop a care plan with input from family members and
physicians, which includes a complete medical, social, safety and
nutritional evaluation
• Coordinate and oversee the care plan to be followed by all those
in connection with the care of the patient
• Arrange for in-home services
• Coordinate the delivery of medications, meals, medical
equipment, etc.
These programs and services help keep patients on track, but
patients also have to take an active role and be accountable for
managing their own health. Studies show that nearly a quarter of
patients discharged from the hospital will return within a 30-day
period. A majority of these readmissions are avoidable. Teamwork
and communication are key to enhancing prevention efforts and
increasing patient safety and wellness.
services.
