<strong>Lee</strong> Center for Rehabilitation & Wellness offers you membership<br />
for $25 per month. This membership offer provides access to all<br />
exercise equipment and classes from noon-close on Tuesdays<br />
and Thursdays, as well as all day Saturdays and Sundays.<br />
The Wellness Center of Cape Coral is offering <strong>SHARE</strong> <strong>Club</strong><br />
members special pricing for membership. Now, you can join the<br />
Wellness Center for just $40 per month, and additional family<br />
members are $35 per person.<br />

Keep You Out of the Hospital<br />
Many people end up in the hospital because of a lack of communication and coordination of care. Patients often do not fully<br />
understand what they have to do to maintain their health. Teamwork and communication can keep patients safe and healthy<br />
at home—right where they want to be. <strong>Lee</strong> <strong>Memorial</strong> <strong>Health</strong> <strong>System</strong> has created a number of programs and services to help<br />
patients avoid hospital stays, including:<br />
House Calls<br />
For chronically ill, medically fragile, elderly or homebound<br />
patients unable to get to the physician’s office, the House Calls<br />
program provides the care they need for recovery. Patients are<br />
visited in their homes and medical needs are assessed. Referrals<br />
for other services—like home health or physical or occupational<br />
therapy—are made, if needed.<br />
Lifeline<br />
Lifeline Medical Alert Service provides independence and peace<br />
of mind for older adults. Help is available with just the press of a<br />
button, 24 hours a day. Simply press your personal help button<br />
and you are connected to one of our caring and highly trained<br />
staff via the two-way voice communicator. Depending on the<br />
help you need, we can call a friend or a neighbor or dispatch<br />
local emergency services.<br />
Telehealth<br />
Telehealth provides daily monitoring of patients. Information<br />
from a scale, a blood pressure cuff and a pulse oximeter is<br />
transmitted daily to a central monitoring station where it is<br />
reviewed by registered nurses to determine if additional care<br />
is required. The Telehealth program combines technology and<br />
high-quality medical attention to provide patients the care they<br />
require right in their own home.<br />
Geriatric Care Management CHOICES<br />
Geriatric Care Management CHOICES is the <strong>Lee</strong> <strong>Memorial</strong> <strong>Health</strong><br />
<strong>System</strong> program that enables seniors to remain independent<br />
and in their own home while providing peace of mind to<br />
adult children who live throughout the country. Geriatric Care<br />
Management CHOICES licensed professionals will:<br />
• Accompany client to physician appointments and report to<br />
families<br />
• Develop a care plan with input from family members and<br />
physicians, which includes a complete medical, social, safety and<br />
nutritional evaluation<br />
• Coordinate and oversee the care plan to be followed by all those<br />
in connection with the care of the patient<br />
• Arrange for in-home services<br />
• Coordinate the delivery of medications, meals, medical<br />
equipment, etc.<br />
These programs and services help keep patients on track, but<br />
patients also have to take an active role and be accountable for<br />
managing their own health. Studies show that nearly a quarter of<br />
patients discharged from the hospital will return within a 30-day<br />
period. A majority of these readmissions are avoidable. Teamwork<br />
and communication are key to enhancing prevention efforts and<br />
increasing patient safety and wellness.<br />
