September 2010, QH provided <strong>the</strong> <strong>Office</strong> with fur<strong>the</strong>r in<strong>for</strong>mation <strong>and</strong> submissions<br />
relevant to <strong>the</strong> review.<br />
15. In emails dated 21 September 2010, QH provided <strong>the</strong> <strong>Office</strong> with submissions relevant<br />
to its section 40(c) claim <strong>and</strong> provided copies <strong>of</strong> fur<strong>the</strong>r documents it had located.<br />
16. In an email dated 14 October 2010, QH clarified what actions it had taken to improve<br />
its records management systems.<br />
17. On 30 November 2010 a staff member <strong>of</strong> <strong>the</strong> <strong>Office</strong> attempted to consult with <strong>the</strong> QH<br />
employee named on pages 56 <strong>and</strong> 60 <strong>of</strong> Part Two but was unable to speak with that<br />
individual.
