Congratulations! You have been selected to participate in the 2011 OMEA <strong>All</strong>-<strong>State</strong> <strong>Band</strong>. This is an<br />

honor and we are proud of your achievement. Make sure the dates of January 20-22 are scheduled for this<br />

event. It is very important that you read the following guidelines for the event. Please note that four<br />

signatures are required on this contract: (1) your signature as the participating member (2) the signature of<br />

your parent or guardian (3) the signature of your music director (4) the signature of your school principal.<br />

Rules of Conduct:<br />

(1) Students must remain reasonably quiet at all times in the hotel rooms, both day and night. TV sets, radios, and<br />

any other electronic devices must be played at low levels at all times.<br />

(2) Students must wear their Festival Badges at all times when outside the hotel rooms. The badges must be clearly<br />

visible at all times.<br />

(3) Hotel doors should be locked at all times.<br />

(4) Boys are not allowed in girls’ rooms and girls are not allowed in boys’ rooms. If it is necessary to make an<br />

exception, a chaperone must be present.<br />

(5) Curfew for students will be 11:00 o’clock on Thursday evening and 12:00 o’clock (Midnight) on Friday evening<br />

of the festival. The curfew means that students must be in their rooms and not leave them after the hour of curfew.<br />

(6) Possession or use of illegal materials at any time including but not limited to fireworks, weapons, printed<br />

materials and videos, is prohibited.<br />

(7) Possession or use of alcoholic beverages or narcotics at any time is prohibited.<br />

(8) There shall be no practicing or playing of instruments in the hotel room after 9:00 p.m. on any night.<br />

(9) Proper rehearsal etiquette will be observed at all times.<br />

(10) Tardiness will not be permitted. Students arriving late to rehearsals will be dismissed from the festival.<br />

<strong>All</strong> regulations must be adhered to rigidly. Any infringement or violation of rules will result in automatic<br />

dismissal from all activities of the festival and a request that the student be immediately sent home.<br />

I accept membership in the OMEA <strong>All</strong>-<strong>State</strong> <strong>Band</strong>. I understand that I must attend all rehearsals and the Saturday concert<br />

and that I cannot be excused for illness or death in the family. I understand that to be a member of this <strong>All</strong>-<strong>State</strong> Organization,<br />

I must make room reservations and be present for at least two nights, January 20 & 21, 2011 at the Crowne Plaza or Marriott<br />

Courtyard in Tulsa. I understand that all auditions and rehearsals will be held in Tulsa at either the Crowne Plaza Hotel or the<br />

Performing Arts Center. The Festival Concert will be held at the PAC at 2:30 p.m. on January 22 (Ticket price is $10).<br />

I understand that the deadline date for this contract to be returned is Friday, December 17, 2010 (post-marked). I understand<br />

that a personal check, money order, certified funds or school purchase order (not a school requisition) must accompany this form<br />

in the amount of $69. This fee will cover membership, t-shirt, lunch on Thursday, dinner on Thursday evening and lunch on<br />

Friday. I agree to accept the ‘Rules of Conduct’ as stated in this contract and I also agree that I must attend all rehearsals and the<br />

festival concert to complete my obligation as a member of the 2011 OMEA <strong>All</strong>-<strong>State</strong> <strong>Band</strong>. Note: In case of emergency,<br />

rehearsals and concert times are subject to change by the executive board.<br />

PARENT PERMISSION / PARENT MEDICAL PERMISSION<br />

I hereby give my permission for my child to participate in the OMEA <strong>All</strong>-<strong>State</strong> <strong>Music</strong> Festival on January 20, 21 and 22 in<br />

Tulsa. I understand that if my child cannot perform the music satisfactorily upon arrival at the OMEA <strong>Music</strong> Festival, he/she<br />

will be sent home. I accept the responsibility of arranging for necessary housing as it relates to my child. I understand that<br />

my child must stay at least two room nights, January 20 & 21 at the Crowne Plaza or Downtown Marriott Courtyard in Tulsa<br />

to participate in this OMEA <strong>All</strong>-<strong>State</strong> <strong>Music</strong> Organization. I understand that the registration fee of $69. includes membership,<br />

t-shirt, lunch on Thursday, dinner on Thursday evening and lunch on Friday. I understand that my child will be responsible for<br />

the other meals during the festival. I agree with the ‘OMEA Rules of Conduct’ as listed in this contract. I understand that in case<br />

of emergency, every effort will be made to contact me. However, I give my permission for proper medical attention to be given<br />

to my child if the OMEA <strong>All</strong>-<strong>State</strong> Committee members cannot reach me and medical treatment needs to be pursued. I am aware<br />

that in the event of necessary medical treatment, my medical coverage will be employed. It is important that you attach in<br />

writing if your child has a medical condition of which we should be aware.<br />
