4. Promotion commences at 9:00 am (AEST) on 1st April 2013 and entries must be received by 5:00 pm (AEST) on 31st July 2013<br />

6. To enter and claim the cash back offer: Purchase a Participating Product during the Purchase Period from any national retailer in<br />
New Zealand and send by mail (1) completed entry <strong>form</strong> (attached to these terms & conditions or visit www.oral-b.co.nz, and (2) a<br />
photocopy of the purchase receipt, to:<br />

7. If claimant has successfully completed and submitted the Entry Form, including the cash back requirements, in accordance with these<br />
Terms and Conditions and the Promotion Terms and Conditions, cash back will be provided via cheque within 60 working days of<br />
receiving claim. Cheque will be addressed to name of claimant indicated on this <strong>form</strong>.<br />
8. Claimant must retain a copy of the receipts for all claims as proof of purchase. Failure to produce the proof of purchase<br />
for all claims when requested may, in the absolute discretion of the Promoter, result in invalidation of ALL of claimant’s claims and<br />
forfeiture of any right to a refund. Purchase receipt(s) must clearly specify the store of purchase and that the purchase was made<br />
during the Promotional Period but prior to claim.<br />
9. Multiple claims are permitted, subject to the following: (a) each claim corresponds to a Participating Product (e.g. three (3) claims must<br />
correspond to three (3) Participating Products (whether the Participating Products are identical or different); (b) the number of claims<br />
per <strong>form</strong> must be clearly identified above and can be verified by a proof of purchase and in accordance with claim requirements; and<br />
(c); maximum of five (5) claims in total per household is permitted.<br />
10. The Promoter, its contractors and related companies in <strong>Oral</strong>-B and Procter & Gamble groups, to the extent permitted by applicable<br />
law, disclaim all liability whether in contract, tort (including negligence), under any statute or otherwise, in relation to this cash back<br />
promotion. Cash back cheques will be sent by post to claimants at the addresses indicated in the Entry Form. The Promoter, its<br />
contractors and related companies in <strong>Oral</strong>-B and Procter & Gamble groups, shall not be responsible for any postal delivery failure<br />
beyond its control and shall be under no obligation to re-issue cash back cheques to replace lost cheques.<br />
11. Promoter reserves the right to request proof of purchase for each claim made to verify all claims before issuing the cash back.<br />
12. Incomplete, incorrect or indecipherable claims will be deemed invalid.<br />
13. All claims become the property of the Promoter. By participating in this promotion, unless otherwise advised by the claimant, each<br />
claimant consents to the Promoter and its contractors collecting and holding the personal in<strong>form</strong>ation provided under Condition 6 “Provided<br />
In<strong>form</strong>ation”. The Promoter or its contractors may use the Provided In<strong>form</strong>ation for the conduct of this promotion and, unless otherwise<br />
requested by the claimant, in any media or <strong>form</strong> of communications to the claimant for future promotional, marketing and publicity<br />
purposes of the Promoter. The Promoter and its contractors may disclose any or all of the Provided In<strong>form</strong>ation, including personal<br />
in<strong>form</strong>ation, to its other contractors and related companies in the <strong>Oral</strong>-B and Procter & Gamble groups (some of which may be outside New<br />
Zealand) and may authorise those contractors and related companies to use it for similar uses. The Promoter and its contractors may use<br />
the Provided In<strong>form</strong>ation or authorise its use without any further reference, payment or other compensation to the claimant. Claimants have<br />
the right to access and correct any personal in<strong>form</strong>ation being held by the Promoter by addressing that request to the office of the Promoter.<br />

administration, security, fairness, integrity or proper conduct of this promotion, then the Promoter reserves the right in its sole<br />
discretion and subject to any written directions given by any relevant authority, to cancel, terminate, modify or suspend the promotion,<br />
or invalidate any affected claims.<br />
