• Claims with incomplete information will not be honored. Product
registration card, proof of purchase, and original reward form are
required. Handwritten receipts, sales history receipts, or credit
card statements will not be accepted as proof of purchase. Claims
that do not comply with the terms of this offer will be rejected.
• The serial number of your new heat pump must be recorded on
this form. You can find the serial number located on the left side of
the unit. Offers received without the serial number cannot be
processed. (See examples of serial numbers in chart below.)
• Claims must be mailed within 30 days of purchase to receive
reward. Limit one offer per household.
• Offer is valid only for a qualifying purchase of an UltraTemp heat
pump purchased and installed within North America or the
Caribbean by end-users. This offer is not good on products
purchased for resale. Units purchased directly from an authorized
wholesale distributor, Internet auction site, or from a dealer
outside North America or the Caribbean are not eligible.
• Reward card is issued by MetaBank , Member FDIC, pursuant to a
license from Visa U.S.A. Inc. The reward card is given to you as a
reward, refund, rebate or gift and no consideration, value, or
money has been paid by you in exchange for the reward card. Card
is not transferable, issued in the name submitted on reward form
and cannot be issued to minors. Card does not have cash access
and can be used at any merchants that accept Visa debit cards.
Card valid for up to 6 months, unused funds forfeit at midnight
EST the last day of the month of the valid thru date, subject to
