<strong>Reclaiming</strong> <strong>Identity</strong> <strong>Kit</strong> – Birth certificate<br />
To obtain a new birth certificate:<br />

You must complete the application for birth certificate (copy attached). If you are also<br />
applying for a new birth certificate for family members please complete a separate form for<br />
each person<br />
You must provide a letter signed by either Centrelink, the Country Fire Authority, the State<br />
Emergency Service or Victoria Police verifying that your birth certificate has been lost in the<br />
bushfires. Attached is a sample letter you could give to the authority to use.<br />
You are not required to provide any other form of identification to have your birth certificate<br />
replaced. You should ignore the identification requirements on the application form.<br />
The $25.80 application fee will be waived. You should ignore the section requesting payment<br />
details on the application form.<br />
If you can attend the Victorian Registry of Births, Deaths and Marriages in person and<br />
take the application form/s and letter with you and they will process your application and issue<br />
you with a new birth certificate/s on the spot.<br />

If you cannot attend the Victorian Registry of Births, Deaths and Marriages in person,<br />
you should send the application form/s and letter to Ms Kathy McLaren at the following<br />

You will need to provide a return address for your birth certificate/s to be sent to. You can ask<br />
for your birth certificate/s to be sent to any address that you choose, for example to a friend or<br />
family member. It is not recommended that you have the certificate/s sent to a local post<br />
office close to the bushfire areas as the post offices are dealing with a backlog of mail.<br />
