• Develop & distribute Individual Care Plans<br />

• Maintain a Health Alert List of student conditions<br />

• Conduct annual vision and hearing screenings<br />

• Participate in 504/ IEP meetings<br />

• Provide annual HIV/AIDS teacher training<br />

• Instruct Growth & Development lessons for grades 4 & 5<br />

• Hold annual parent previews for materials<br />

• Provide training for Nutrition Services & Transportation<br />

• Collaborate with Benton-Franklin County Health Department for<br />

school immunization clinics for 5 th graders<br />

• Assist in on-site dental services<br />

• Provide referrals for “Free Dental Day”

• Saturday, February 7th<br />

• 4 clinics participated<br />

• 1 provided appointments for<br />

PSD students only<br />

• 1 provided appointments for all<br />

participating districts<br />

• 2 were “walk in” with first-come,<br />

first-serve approach<br />

• No medical coupons needed
