Instructions<br />

The attaches T-<strong>Series</strong> <strong>Special</strong> <strong>Warranty</strong> <strong>Claim</strong> <strong>Form</strong> is to be completed and submitted for all warranty<br />

repairs involving the replacement of part number 303238002 Cord Rewind Assembly.<br />

Please make additional copies of the attached form to support your repair needs. Additional forms<br />

may also be found on the <strong>Hoover</strong> Dealer Portal or by contacting Joe Caracci at 1-440-996-2554.<br />

All information must be completed to receive credit. Complete forms as follows:<br />

1. <strong>Claim</strong> Number – consists of the last four digits of the customer’s phone number and the two<br />

month and day that the customer’s machine was brought in for service. (example: Customer<br />

John Doe brought in his T- <strong>Series</strong> on March 3, 2010 and his phone number is 123-456-7890, the<br />

claim number would be 7890 0303)<br />

2. Dealer information – Please complete with all pertinent information including contact name and<br />

email.<br />

3. Product information – Please list Model Number, Mfg. Code (date code), and customers date of<br />

purchase (actual or approximate)<br />

4. Labor – Labor will be paid at a Standard rate of $20.00 if repair is completed within 72 hours<br />

of customer drop off or at an Expedited rate of $25.00 if the repair is completed within the<br />

same day of customer drop off and is verified by the customer at the bottom of the form.<br />

5. Parts – All parts will be replaced to maintain a consistent level of inventory in your shop. All<br />

parts are at no cost to you so no credits will be issued.<br />

6. Customer information – It is very important that we capture all customer information.<br />

Please make certain that the customer complete all information on the warranty claim form.<br />

7. Customer acknowledgement – If the customers repair is completed within the same day of drop<br />

off, and you are requesting Expedited <strong>Warranty</strong> Labor, the customer must acknowledge the<br />

service. (random audits may be completed with customers to verify acknowledgement)<br />

8. Signatures – All warranty claims must contain the customer and Tech signatures to be valid.<br />

9. Submit <strong>Special</strong> warranty claim form, all information complete, within 24 hours of repair.<br />
