PLEASE READ THE FOLLOWING INSTRUCTIONS PRIOR TO COMPLETING THE
APPLICATION. MISSING INFORMATION MAY CAUSE THE APPLICATION TO BE RETURNED
TO YOU FOR CORRECTION.
1. The applicant must NOT have been previously issued a hunting license by the Department, another
state, agency of a Canadian province or an agency of any other foreign country, including another
apprentice license.
2. "Resident" means any person who is a citizen of, or is lawfully entitled to remain in, the United States;
and during the 6 months next preceding his application was physically present in Nevada, except for
temporary trips outside of the state; and did not purchase or apply for any resident license, tag or
permit to hunt, fish or trap in another state, country or province.
3. Each application must be accompanied by the proper fee; submit a cashier's check or money order.
OUT-OF-STATE personal or company checks are not accepted. A $25 return check fee and non
refundable fees will be due for checks returned by the bank.
4. Applicant must attach proof of residency (may include Nevada driver's license or other proof,
containing name and address and dated 6 months prior.)
5. Parent or legal guardian signature required for youth 17 years of age and younger purchasing a hunting
or combined hunting and fishing license as per NRS 41.472, as provided below.
*NRS 41.472
1. If a parent, guardian or other person legally responsible for a minor under the age of 18 years:
(a) Knows that the minor has previously been adjudicated delinquent or has
been convicted of a criminal offense;
(b) Knows that the minor has a propensity to commit violent acts; or
(c) Knows or has reason to know that the minor intends to use the firearm for
unlawful purposes,
and permits the minor to use or possess a firearm, any negligence or willful misconduct of the
minor in connection with such use or possession is imputed to the person who permits such use
or possession for all purposes of civil damages, and, notwithstanding the provisions of
subsection 2 of NRS 41.470**, that person is jointly and severally liable with the minor for any
and all damages caused by such negligence or willful misconduct.
**NRS 41.470 states the joint and several liability of one or both parents or guardian having custody or
control of a minor under this section shall not exceed $10,000 for any such act of willful misconduct of
the minor.
6. Prior to entering the field your Apprentice license must be signed and the affidavits initialed by the
applicant and the parent or legal guardian must sign if required per NRS 41.472.

Applicant Affidavit acknowledging the following apprentice license requirements: Initial in boxes after reading.
I have not previously been issued a hunting license or apprentice license in this state, another
state or a Canadian province or an agency of any other foreign country including another
apprentice license.
I will at all times while in the field be accompanied and directly supervised by at least one adult
licensed mentor hunter who is 18 years of age or older. ("Accompany and directly supervise"
occurs when a person over 18 maintains close visual and verbal contact with, provides adequate
direction to, and can immediately assume control of the firearm from the apprentice hunter.)
I will follow all applicable laws and regulations regarding hunting and the use of firearms.
