Message from the Chair and Executive Director<br />
Maya Angelou, an American Poet, said “I've learned that people will forget<br />
what you said, people will forget what you did, but people will never<br />
forget how you made them feel (1928).” We asked past and present members<br />
of <strong>Langs</strong> to “tell us your story” and you told us how <strong>Langs</strong> made you feel in a<br />
resounding way. Our story “Growing Up <strong>Langs</strong>,” captures our 32 year history<br />
from infancy to young adulthood. Thanks to our authors Geoff Nelson and<br />
Rachel Fayter of Wilfrid Laurier University and our Book Action Team<br />
Volunteers Mary Jo Lyons, Graham John, Muriel Bechtel, Ben Benninger,<br />
Ann Hawkins, Jeanne Dumont, Kerry-Lynn Wilkie and Bill Davidson. Special<br />
thanks to Greg Pautler, Graphic Designer and Preston Historian for his talents.<br />
We turned several pages this year in the area of partnership development. We<br />
welcomed St Mary’s Counselling Service as a 4th co-located partner onsite and<br />
have seen the benefits of this collaboration in a short period of time. The<br />
Waterloo-Wellington <strong>Community</strong> Care Access Centre has also joined us as an<br />
onsite partner and several other agencies are offering programs at <strong>Langs</strong> or<br />
co-facilitating groups with our staff. We were also pleased to be selected to take<br />
on the role of leading one of fourteen Regional Diabetes Co-ordination Centres<br />
for the Waterloo-Wellington area. This new role is obviously reflective of the<br />
calibre of work of our diabetes education team and it is their work that was the<br />
impetus that resulted in the organization being recognized with a partnership<br />
award by the Canadian Diabetes Association.<br />
The roots of our work have been deeply embedded with the faith community<br />
for the past thirty years. Preston Mennonite Church has been a founder,<br />
mentor, community builder and a friend throughout our history. We are grateful<br />
for the significant in kind and financial contribution the members of the<br />
congregation have made to <strong>Langs</strong>. They are an important part of our history<br />
along with several other churches in our community. We were pleased to<br />
recognize five churches this year at our annual church appreciation event<br />
held in partnership with the Preston Heights <strong>Community</strong> Group.<br />
In their first full year of operation, the North Dumfries <strong>Community</strong> Health<br />
Centre Satellite team has established an impressive range of programs and<br />
services and has received overwhelming support in the community. Our<br />
participants and patients have told us that the work of both the North<br />
Dumfries and <strong>Langs</strong>’ teams has resulted in a high rate of satisfaction in the 313<br />
surveys that were conducted and the 5 focus groups we held. We are sure you<br />
will agree we have come along way from operating our preschool programs in<br />
the panelled basement of our townhouse when you see some of the results of<br />
our survey on page 17 of this report.<br />
Unfortunately we must conclude one of our chapters by saying goodbye to<br />
three outstanding board members who have completed their terms, Donna<br />
Mae Fleet, Ann Hawkins and Brian Arn. Their enormous commitment to our<br />
work and their dedication to move the organization forward will truly be<br />
missed. We are also pleased to have the interest of many in serving on the<br />
<strong>Langs</strong>’ Board over the past three years. We are fortunate to welcome the skills<br />
and talents of Cathy Schafe, Dale Hart and Lisa DiNunzio to our board this<br />
year. We are grateful to the substantial contribution that all of our 249<br />
volunteers make to the organization.<br />
We are pleased to report that we have made substantial progress on the<br />
development of our new building. We have submitted an application to the<br />
FEDDEV Program of the federal government and are anxiously waiting to hear<br />
the outcome. A project team was established in partnership with the City of<br />
Cambridge and Roberston Simmons Architects have been hired to complete<br />
drawings. They have developed an exciting an innovative plan that combines<br />
our space with the Allan Reuter Seniors Centre to become an important focal<br />
point in the community.<br />
A new building will truly be the beginning of a new era in the history of <strong>Langs</strong>.<br />
It will look different and feel different; however the one thing that will<br />
continue to remain constant is our commitment to our values and principles<br />
that guide our work. This new reality resonates well with our book and one of<br />
our favourite television shows that epitomizes the experience of growing up,<br />
the Wonder Years and is fittingly described by Fred Savage’s character when he<br />
says, “Growing up is never easy. You hold on to things that were. You<br />
wonder what's to come. But that night, I think we knew it was time to let<br />
go of what had been, and look ahead to what would be. Other days. New<br />
days. Days to come (The Wonder Years).<br />
While we gear up for our 4th accreditation process, we hope you will take a<br />
moment to review the many other highlights in our 28th Annual Report. Please<br />
also order a copy and enjoy reading “Growing Up <strong>Langs</strong>.” We expect it will be<br />
a community story telling classic.<br />
Our satisfaction survey highlights that important work of our staff. Their<br />
commitment both past and present featured predominately in our story<br />
demonstrates that its relationships that matter. We continue to be proud of<br />
the dedication and talent of our hard working team at <strong>Langs</strong>. Relationships also<br />
matter with our partners and our accomplishments would not be possible<br />
without the support of the Waterloo Wellington LHIN, the City of Cambridge,<br />
the United Way of Cambridge and North Dumfries, the Lyle S. Hallman<br />
Foundation, the Cambridge and North Dumfries <strong>Community</strong> Foundation,<br />
RBC Foundation, the Ministry of Children and Youth and numerous others to<br />

• 21 students from 13 different post secondary educational institutions completed placements at <strong>Langs</strong><br />

• 28 Human Resources Polices were revised and approved by the Board of Directors<br />
which included new policies on ethics, workplace violence, accommodation,<br />
compensatory time and exit interviews<br />
• A new procurement policy was developed and approved by the Board of Directors<br />
• The board reviewed and updated the organization’s bylaw<br />
• The organization Occupational Health and Safety, Volunteer and Privacy Policies<br />

Manuals were reviewed and updated and new IT policies were developed<br />
• <strong>Langs</strong> has developed and implemented new electronic forms including an<br />
electronic timesheet<br />
• <strong>Langs</strong> applied to the federal government for <strong>Community</strong> Adjustment Fund and FEDDEV<br />

• Over 20 letters of support from local agencies and businesses were received to support<br />

• A review of the Waterloo Region Diabetes Education Program was undertaken to<br />
prepare for the expansion of 1.5 Registered Dietitian and Registered Nurses funded by<br />
the Ministry of Health and Long Term Care<br />
• The Youth and Teen Centre has been upgraded with new signs, painting, carpeting<br />
and a new kitchen with the help of Waterloo Region Housing<br />
• Renovations occurred at our main site to build a new kitchenette and storage space<br />
• Flat Screen TVs were installed with in the Resource Centre and Waiting Room to<br />
promote LFVA programs and services and to provide directions to participants to<br />
programs and meeting rooms<br />
• 14 grants were received including funding from New Horizons for the Seniors<br />
Outreaching and Supporting Seniors Program and Capital Assistance, a grant from the<br />
Trillium Foundation for a Resource Development Officer, funding for the PLAY<br />
Program from the Ministry of <strong>Community</strong> Safety and Correctional Services, RBC<br />
Foundation and the Cambridge and North Dumfries <strong>Community</strong> Foundation<br />
• We recruited two new physicians and three new nurse practitioners (including a<br />
Grow Your Own NP) at <strong>Langs</strong> and North Dumfries and have obtained our full<br />
compliment of clinical staff<br />
• <strong>Langs</strong> staff attended professional development training offered by the Alzheimers<br />
Society, <strong>Community</strong> Care Access Centre, St Mary’s Counselling Centre and diversity<br />
training facilitated by CAMH<br />
• 75 children and youth were recipients of the Payless Shoes 4 Kids grant; 13 children<br />
accompanied LFVA staff to the local Payless Shoes to pick shoes to wear at the Gym<br />
Generation program<br />
• 63 families received support at Christmas with assistance from existing partnerships<br />
and new partnerships such as: Toronto Dominion Bank, Babcock and Wilcox, the<br />
Cambridge Fire Fighters, Cambridge Champion Sales Club, and the Zonta Club<br />
of Cambridge.<br />
• <strong>Langs</strong> welcomed two new corporate partners; Starbucks Coffee Company,<br />

• 77 children have been screened at Early Years Screening Clinics.<br />
• A total of 250 participants accessed clothing at our quarterly clothing giveaways<br />
• 29 programs were offered to participants with a focus on skills and strength building<br />
• 22 attended the new GIRLZ and BOYZ program for preteens in August 2009<br />
• 5 youth were sponsored to attend summer camps and 4 youth attended the Kids and Cops<br />
fishing trip in August 2009<br />
• A total of 365 seniors participated in the Seniors Outreaching & Supporting Other Seniors program<br />
funding by the New Horizons Program<br />
• The After 3 Club introduced a new environmental program, Preserving Our Water, sponsored by RBC.<br />
• There were 119 new volunteers at <strong>Langs</strong>, including student placements and 20 William G. Davis<br />

• Over 600 individuals have received diabetes education through individual appointments and<br />
education workshops in various locations.<br />
• 913 shots were given which included 325 seasonal flu shots and 588 H1N1 shots<br />
• 35 patients attended a new Hypertension Clinic and Education Session<br />
• 7 patients accessed the Memory Clinic in partnership with the Centre for Family Medicine and Trellis<br />
• 42 volunteers of all ages participated in training opportunities including: youth behaviour<br />
management; conflict resolution and problem solving; infection control; diversity and inclusion; and<br />
increasing accessibility<br />
• 18 Leaders In Training contributed 852 hours volunteering with youth summer programs<br />
• 6 new volunteer street representatives were recruited and trained to conduct outreach and promote<br />
LFVA programs in the community<br />

• The North Dumfries Satellite CHC is fully established and has recruited its full compliment<br />
of staff<br />
• North Dumfries rostered 276 new patients through group intake sessions and continues to<br />
welcome 30 new patients per month<br />
• 420 shots were given which included 158 seasonal flu shots and 302 H1N1 shots<br />
• Weekly venipuncture clinics were implemented which eliminates the need for patients to<br />
travel outside of their home community to have blood tests completed<br />
• In 2009, 4 summer students were hired for the first time from the township with<br />
funding from the Canada Summer Jobs Program<br />
• 379 young people participated in summer programs; registration for the programs filled in 30<br />
minutes demonstrating the need for youth programs in the rural community<br />
• A full range of successful youth programs were offered including physical fitness programs,<br />
cooking programs and leadership programs<br />
• 40 participants attended March Break Programs at the Countryside Camp and<br />
Conference Centre<br />
• 13 participants attended the Stitch It, Intergeneration Program where youth were paired with<br />
seniors to learn how to knit<br />
• Grants were received from the Social Planning Council of Cambridge and the Ayr 150th<br />
Anniversary Grant Committee to offer youth programs<br />
• 11 North Dumfries Advisory Committee members meet monthly and contributed over 250<br />
volunteer hours to this important leadership role in their community<br />
• An older adults group in Drumbo and Ayr ran offering seniors the opportunity to establish<br />
social networks and learn about available community resources<br />
• The Social Worker saw 31 new participants and had 181 follow up visits<br />
• The Registered Dietitian saw 26 new patients with Type 2 diabetes and had 59 follow up visits<br />
• 44 new general nutrition appointments were held and 78 follow ups visits occurred with the<br />
Registered Dietitian<br />
• A “Hearing Clinic” ran in North Dumfries and provided hearing tests and information about<br />
assistive devices. 11 participants accessed this resource made available through the Ontario<br />
Hearing Society<br />
• A free Income Tax service was made available to qualifying residents.<br />
• The Connection Newsletter, produced in partnership with the Social Planning Council, Family<br />
Counselling Centre and the Centre for the <strong>Community</strong> Group is sent three times per year to<br />
all homes in North Dumfries<br />
• A presentation was conducted to North Dumfries Town Council about the CHC Satellite<br />
and how it is meeting the priorities of the Waterloo-Wellington LHIN<br />
• The North Dumfries CHC will be part of the new multi-use facility that received federal<br />
infrastructure funding that was announced in June 2009. This new facility will consist of<br />
an ice surface, walking track, seniors and youth lounge and the satellite CHC<br />

• St Mary’s Counselling Service co-located with <strong>Langs</strong> as one of 4 co-located partners on site<br />
• 15 partners providing 18 services on site including a new partnership with the <strong>Community</strong> Care<br />
Access Centre<br />
• The North Dumfries CHC Satellite welcomed the Allan Reuter Centre Adult Day Program to their site<br />
• 14 partners attended a <strong>Community</strong> Partnership Forum at <strong>Langs</strong> and 15 partners attended a<br />
North Dumfries partnership session to celebrate successes, network and plan for future initiatives<br />
• 7 children received bicycles through the Blended Bicycles program offered in partnership with<br />
St. Benedicts Catholic Secondary School<br />
• A new partnership was established with King Street Baptist Church to operate a weekly Parent<br />
Caregiver and Tots program onsite in downtown Preston<br />
• 18 <strong>Langs</strong> and 31 Region of Waterloo’s Growing Healthy Together program participants attended an<br />
apple picking trip together<br />
• A new <strong>Community</strong> Partner’s Handbook was created in consultation with community partners<br />
• 4 new partnership programs were offered onsite including: the Motivational Learning Group<br />
Program co-facilitated by <strong>Langs</strong> <strong>Farm</strong> and Preston Heights <strong>Community</strong> Group; Fathering Now by<br />
Mosaic Counselling; Bridges to Health by the House of Friendship; and a new Girl Power program in<br />
partnership with Mosaic Counselling and Family and Children’s Services Parenting for Parenting of Teens<br />
• Youth peer workers were hired with the donation received from the Waterloo Regional Police Caught in<br />
the Act II Play at <strong>Langs</strong> and North Dumfries<br />
• An LFVA staff joined School Council of Preston High School, increasing the community links and<br />
partnership opportunities between <strong>Langs</strong> and PHS<br />
• A new partnership was established with the “Social Work Bridge Training Program”. A student is placed<br />
at 7th “Inning. The Program focuses on new Canadians, is collaboration between The Working Centre,<br />
WLU Social Work, Extend a Family Association, Waterloo Region Immigrant Employment Network and<br />
Centre for <strong>Community</strong> Based Research.<br />
• The Preston Chapter IODE (Imperial Order of Daughters of the Empire) donated money to be used for<br />
the Books for Birthdays program, Christmas Family Sponsorship and New Family Support.<br />
• Planning has begun with Waterloo Region <strong>Community</strong> Legal Services to offer a Tenant Information<br />
session on-site in early 2010.<br />
• Partnership agreements were reviewed and revised with the Arthritis<br />
Society and Fairview Mennonite Home and Family and Children's Services.<br />
• Waterloo Region Public Health will be offering dental screening<br />
services onsite in late 2010<br />

• 462 participants attended the annual <strong>Community</strong> Picnic at William G. Davis School sponsored<br />

• The Director of Education, the Superintendent of the Preston family of schools and Principals<br />
of local public schools attended an information session and tour at <strong>Langs</strong><br />
• Over 140 participants attended two special North Dumfries summer events: a Family Fun Fair<br />

• 10 participants from 6 different educational institutions attended a Post Secondary<br />

• An orientation session for 3 Renison University College Social Development Studies Students<br />

In 1992, <strong>Langs</strong> relocated the Youth and Teen Centre<br />
to 581-E <strong>Langs</strong> Drive. In 1998, teens designed and painted a<br />
mural for Youth and Teen Centre with local artist Dave Sopha.<br />
In 2006, <strong>Langs</strong> expanded with the addition of a<br />
portable at the Youth and Teen Centre.<br />

is Proud to Acknowledge Paula Carrere’s<br />
Leadership Role as the President of the<br />
Nurse Practitioner Association of Ontario<br />
Paula Carere took on the role of President of Nurse Practitioner<br />
Association of Ontario having previously served as Treasurer and<br />
President Elect. After a diverse nursing career in critical care and<br />
health administration, Paula entered the NP education program to<br />
support her belief that health care needs of Ontarians will best be<br />
served by a system shift from an illness approach to a focus on<br />
supporting and maintaining healthy behaviours. Paula hapracticed<br />
as a Primary Health Care Nurse Practitioner at <strong>Langs</strong> <strong>Farm</strong> CHC in<br />
Cambridge for close to twelve years and served as Clinic Co-ordinator<br />
for five years. In 2008, Paula received an RNAO clinical<br />
practice fellowship focusing on care of the elderly in primary care<br />
settings. Paula has a leadership role supporting the interprofessional<br />
and inter-organizational team to develop an integrated<br />
community based program for seniors. She hasa strong<br />
commitment to education and integration of the role as an<br />
instructor at Laurentian University, preceptor in the COUPN<br />
program, member of theAssociation of Ontario Health Centres NP<br />
Leadership Group and member of the HHR Council of the<br />
Waterloo Wellington LHIN. Paula models healthy lifestyle by riding<br />
her bike to work whenever she can.<br />

Organizational Satisfaction Survey<br />
• 313 survey responses were collected and 6 focus groups were held.<br />
Data was also collected through our organizational satisfaction tool,<br />
the Ticket to Comment cards and quarterly program evaluations.<br />
• 82% of those who responded indicated that they are very satisfied<br />
with what they got out of <strong>Langs</strong> <strong>Farm</strong> programs and services.<br />
• 83% to 88% of those who responded indicated that they are very<br />
satisfied with the front desk when speaking to them either on the<br />
phone or in person.<br />
• Focus groups reveal that the preventative nature of <strong>Langs</strong> programs<br />
and services is valued. Working with more than one provider is<br />
associated with satisfaction.<br />
• 84% of those responding indicated that they are very satisfied with<br />
their overall experience.<br />
• “Been here three years, and I like it, I come in because I got nobody<br />
back home, I live by myself, so I come here then I come in here and<br />
I have the people.”<br />
• “Knowing that you’re able to come here and you know you’ve got<br />
friends here, that’s important to me.”<br />
• “When you come here, you’re an equal human being, on equal<br />
footing with everyone else, and it makes you feel better about<br />
yourself and makes you want to come back.”<br />
