NURSING PROGRAM<br />

STEP ONE: Admission to <strong>Terra</strong> <strong>State</strong> <strong>Community</strong> <strong>College</strong><br />

Individuals who wish to apply for admission to the Associate Degree in <strong>Nursing</strong> Program<br />

must first apply for admission to the <strong>College</strong>. Admission to the <strong>College</strong> does not guarantee<br />

admission into the <strong>Nursing</strong> Program. For additional information regarding this process,<br />

contact the Office of Admissions and Enrollment Services at 419-559-2349 or by email at<br />

admissions@terra.edu.<br />

1. Graduate from high school or complete the GED. You must submit an official<br />

high school transcript or official GED certificate (including scores) to the Office<br />

of Admissions and Enrollment Services.<br />

2. Submit an official transcript from all previously attended colleges or universities<br />

to the Office of Records & Registration.<br />

3. Complete the placement assessment and meet with an admissions advisor to<br />

discuss the nursing program.<br />

4. NO STUDENTS WILL BE ACCEPTED INTO THE NURSING PROGRAM IF THEY<br />

HAVE FAILED OUT OF ANOTHER NURSING PROGRAM.<br />

STEP TWO: Pre Admission to the <strong>Nursing</strong> Program<br />

1. Achieve scores on ACT or COMPASS for placement into college-level reading<br />

and writing (ENG 1020 or ENG 1050) and college-level math (MTH 1310). If a<br />

student places into a remedial course(s) and successfully completes the<br />

course(s), the student will also have satisfied this requirement.<br />

2. The student must have placed high enough on the chemistry placement exam<br />

to begin BIO 1230 or complete a college chemistry course with a laboratory within<br />

the past 5 years of enrolling in BIO 1230 (Anatomy and Physiology I). The<br />

chemistry course must be equivalent to CHM 1010 and CHM 1015. Anatomy and<br />

Physiology and Microbiology courses completed more than five years prior to<br />

enrollment in the first nursing courses, must be retaken.<br />

3. Complete BIO 1230 and 1235, Anatomy & Physiology I with a “C” or better.<br />

4. In addition to completing BIO1230 and 1235, students must pass the TEAS V<br />

test with at least a proficient level before beginning the application process for the<br />

nursing program. This test may be taken only twice, and each time the test is taken a<br />

fee must be paid. The TEAS V (Test of Essential Academic Skills) can be accessed at<br />

www.atitesting.com. Please follow the directions to create an account and pay for<br />

the test.<br />

5. After completing BIO 1230 and scoring at the Proficient level on the TEAS V<br />

test, a student may apply to the <strong>Nursing</strong> Program. The student will be placed on<br />

the candidate list. However, students must have a Cumulative GPA of 2.8 on a<br />

4.0 scale in courses that apply to the nursing program. Additionally, the<br />

student must receive a grade of “C” or better in all courses that apply to the<br />

nursing program.<br />

6. Before the student is enrolled in any nursing (NUR) courses, Anatomy &<br />

Physiology II must be completed.<br />

STEP THREE: Admission for the <strong>Nursing</strong> Program Process<br />

This Admission criterion is effective with the incoming Fall 2012 <strong>Nursing</strong> Class.<br />

1. Once the student reaches the top of the candidate list, the student’s record is<br />

reevaluated to ensure the cumulative GPA is still 2.8 or higher on a 4.0 scale.<br />

2. Students will be ranked on a point system based on their TEAS V test score,<br />

GPA and science scores. Students can achieve a maximum of 12 points and must<br />

earn a minimum of 5 points in addition to scoring on at least the proficient level on<br />

the TEAS V to be considered for the program.<br />

TEAS levels for TEAS V – This can be taken twice and must be passed with a proficient<br />

level or higher before beginning the nursing application process. The TEAS test must be<br />

completed by March 1 for fall semesters and October 1 for spring semesters. The levels are<br />

as follows:<br />

Basic level – 0 points<br />

Proficient level – 1 point<br />

Advanced level – 2 points<br />

Exemplary level – 3 points<br />

GPA criteria:<br />

GPA of 3.5 – 4.0 – 3 points<br />

GPA of 3.0 – 3.499 – 2 points<br />

GPA of 2.8-2.99 – 1 point<br />

The GPA Calculation is based on the following courses:<br />

BIO 1230 - A & P I, BIO 1240 - A & P II, BIO 1710 – Microbiology, ENG 1050 – <strong>College</strong><br />

Composition I, ENG 1060 – <strong>College</strong> Composition II, PSY 1210 – General Psychology, PSY<br />

1360 – Life Span Development, SPE 2200 – Interpersonal Communication and HUM<br />

Elective (PHL 1010 is recommended).<br />

Science grade(s):<br />

A & P I – A – 3 points A & P II – A – 3 points<br />

A & P I – B – 2 points A & P II – B – 2 points<br />

A & P I – C - 1 point A & P II – C – 1 point<br />

The points for A & P I and A & P II will be awarded for the first time taken and students<br />

must achieve a C or better in the course. Revised 12/2011<br />

3. Students who meet the admission criteria will be scheduled for a meeting with the<br />

nursing faculty. The student will complete the Life Plan document, obtain the<br />

Background Check, and obtain three letters of recommendation (one of which must<br />

be from a science faculty member). These documents must be received by the Dean<br />

of Allied Health, <strong>Nursing</strong>, and Science prior to the meeting.<br />

4. During the meeting, the student will complete the written essay “Why I want to be<br />

a Nurse.<br />

5. The Admissions Points Rating criteria will be completed and discussed with the<br />

student during the meeting.<br />

6. The student will then either be sent an official letter of acceptance with the date of<br />

the mandatory orientation, or be placed on the waiting list. Not all students will<br />

advance to the nursing program following this meeting.<br />

7. Those accepted will receive an acceptance letter with the date of the mandatory<br />

orientation and documents to be completed for orientation.<br />

STEP FOUR: Orientation to the <strong>Nursing</strong> Program<br />

1. The student will complete the health requirements, obtain a Health Provider<br />

CPR card, obtain health insurance and complete any required drug tests.<br />

2. The student will attend the mandatory orientation meeting.<br />

3. All documentation will be completed and brought to the orientation.<br />

4. Acknowledgement forms will be sent to the Academic Administrative Assistant<br />

of the Allied Health, <strong>Nursing</strong>, and Science division prior to the first day of class.<br />

STEP FIVE: Enrollment into the <strong>Nursing</strong> Courses<br />

1. Upon successful completion of all requirements, the student will be enrolled in<br />

the nursing courses and given their schedule prior to class.<br />

LPN to RN Program (Advanced Placement):<br />

Licensed Practical Nurses, who have an active license and are in good professional<br />

standing, may apply for the nursing program in the same manner. They shall be placed on<br />

a separate waiting list.<br />

For LPN to RN students, one year of experience as an LPN in a home care, hospital,<br />

physician’s office or long term care setting will be required before beginning the<br />

application process for the program as well as fulfilling the above mentioned<br />

admission criteria.<br />

LPNs are required to complete the program perquisite courses and general education<br />

course. Upon completion of the program prerequisite courses, the LPN may apply for<br />

admission to the nursing program. BIO 1230, BIO 1240 must be completed prior to<br />

enrolling in NUR 1130 and NUR 1135.<br />

If the student earns a grade of “C” or better in NUR 1130 (<strong>Nursing</strong> Transition Lecture), NUR<br />

1133 (<strong>Nursing</strong> Transition Clinical), and NUR 1134 (<strong>Nursing</strong> Transition Skills Laboratory),<br />

the student will receive credit for NUR 1050, NUR 1111, NUR 1112, NUR 113, NUR 1115,<br />

NUR 1120, NUR 1140, NUR 1143, and NUR 1145.<br />
