TANIA MYSAK, BSP, PHARMD, KENT TOOMBS, BSCPHM, ACPR, MOIRA WILSON, BSCPHM As outlined in CSHP 2015, our profession aims, by 2015, to have all new pharmacists entering hospital or related practice complete a residency accredited by the Canadian Hospital Pharmacy Residency Board. The purpose of this session is to debate the merits, or lack thereof, of requiring every hospital pharmacist to have a residency at entry to practice in a hospital or related healthcare settings. There has been a demonstrable shift in the pharmacist’s scope of practice from drug preparation and distribution toward direct patient care activities. With this continuing trend toward increased clinical responsibilities, professional organizations, academia, health care systems and other stakeholders have increased their expectations with respect to the amount of training required to produce an entry level pharmacist. Through structured rotations in pharmacy practice, education, research and administration, residency programs aim to prepare pharmacists for challenging and innovating pharmacy practice. Graduate residents are an important source of practitioners and many employers list residency training as a preferred qualification for clinical practice. Controversy does exist as to whether residency training should be a mandatory prerequisite for practice. One of the central questions in this controversy is whether there is sufficient value obtained from completing a residency. There are many perspectives on the perceived value from patients, providers, employers, payers, society, the profession as a whole and from those individuals who have completed residency training. Goals and Objectives 1. To provide different perspectives to either refute or support the position that all hospital pharmacists should have a residency. 2. To provide pharmacists with sufficient information to assist them to develop an opinion on this issue. Self-Assessment Questions 1. What are the merits of all pharmacists having a residency at entry to practice? What are the challenges with achieving this goal? 2. What is the best way to ensure pharmacists have the required competencies to deliver direct patient care? The Future Beckons: New Antiviral Agents and the Cure for Hepatitis C? KATHRYN SLAYTER, BSCPHM, PHARMD, FCSHP, CAPITAL HEALTH, DALHOUSIE UNIVERSITY, HALIFAX, NS In Canada, approximately 250,000 individuals are currently infected with Hepatitis C virus (HCV). Every year, approximately 5000 Canadians, mostly youth, become infected with HCV – often during the first year following initiation of injection drug use. Hepatitis C has rapidly surpassed HIV as a cause of death in the US. Roughly three quarters of these deaths occurred in people between the ages of 45 and 64. The relatively young age of people dying from HCV portends a large and ever-increasing health care burden. Before 1990, HCV was an incurable, chronic infection and had only a 10% cure with early interferon monotherapy. Subsequently, therapy for HCV evolved to using pegylated interferon and ribavirin. The combination of pegylated interferon and ribavirin produces an overall SVR of between 50% and 90% depending on the genotype. In 2011, the first HCV specific protease inhibitors, boceprevir and telaprevir were licensed, after clinical trials showed that these drugs combined with pegylated interferon and ribavirin, could achieve close to 70% SVR for patients with genotype 1 infections. Treatment of chronic HCV remains complex, however with newer therapies we are now able to decrease treatment durations and increase cure rates. Currently more than 50 clinical trials are evaluating new direct-acting antivirals to treat HCV infection. The future holds great promise for newer drugs to improve the sustained virologic response, shorten the duration of treatment and improve tolerability. Goals and Objectives 1. To evaluate the impact of results of clinical trials of approved treatment for chronic hepatitis C on current and future patient management strategies. 2. To provide pharmacists the evidence on best practice to maximize response and minimize treatment failure in patients receiving hepatitis C treatment. Self-Assessment Questions 1. How can pharmacists be better able to provide accurate and appropriate counseling as part of the hepatitis C treatment team?
