Call for CSHP National Awards
like to invite you to submit applications and nominations for the
2012-2013 CSHP National Awards Program. The goal of the CSHP awards
CSHPwould
program is to improve patient outcomes by promoting excellence in
hospital pharmacy practice. Awards are presented to pharmacists worthy of recognition for
professional dedication and commitment to the profession and the patient.
The program includes both individual awards as well as general awards.
GENERAL AWARDS
• CSHP 2015 Hospital Pharmacy Residency
Award (NEW)
• Management and Leadership Best Practice
Award
• Patient Care Enhancement Award
• Pharmacotherapy Best Practices Award
• Safe Medication Practices Award
• Teaching, Learning, and Education Award
INDIVIDUAL AWARDS
• Distinguished Service Award
• Isabel E. Stauffer Meritorious Service Award
• New Hospital Pharmacy Practitioner Award
• CSHP/CAPSI Hospital Pharmacy Student
Award
NOTES: For the GENERAL AWARDS program, members can
apply for two awards per project using the general
application form. For the INDIVIDUAL AWARDS, members
are required to complete a separate application or
nomination form.
The criteria, application and nomination forms
for these awards can be found on the CSHP
website.

The deadline for all award applications and
nominations is Monday, September 10,
2012 with the exception of the CSHP/CAPSI
Student Award which is Monday, October 1,
2012.
