Important Insurance Notice<br />
If you have a child age 19 or older that is currently ineligible for coverage<br />
under the <strong>District</strong>’s Health or Dental Plans, you may be able to add them to<br />
coverage under <strong>Public</strong> Act 95‐0958. The Act provides extensions for health<br />
and dental coverage to adult dependent children who previously may not<br />
have qualified for coverage under our eligibility requirements. This provision<br />
will be effective on September 1, 2009. An initial 90‐day enrollment period<br />
will begin on September 1, 2009. During the initial 90‐day enrollment period,<br />
requirements for creditable coverage, continuous coverage, or breaks in<br />
coverage will not be applied.<br />
Adult dependent children ages 19 to 26 that are not eligible as a dependent<br />
child (see attached) or handicapped child by definition in the <strong>District</strong>’s Plans,<br />
qualify if they are unmarried, at least 50% financially dependent on their<br />
parents, and have the same legal address. For adult children who as veterans<br />
served in active or reserve components of the U.S. Armed Forces, coverage<br />
will continue until age 30. In addition to meeting the same requirements as<br />
above, Veterans must have received an honorable discharge. A copy of the<br />
DD2‐14, Certificate of Release from Active Duty is needed for verification.<br />
The monthly premium for this new coverage is $461.53 for Health/Vision<br />
coverage. Dental coverage premiums would be $27.24 for those covered<br />
under the AFT/Food Service/ Transportation Plan and $35.19 for those<br />
covered under the Administrators/Secretary/Tech. Plan. Premiums will be<br />
made via payroll deduction only, and will be calculated to reflect the biweekly<br />
pay schedule of the parent, on either 19, 21, 23, or 26 pay periods.<br />

2. your unmarried, dependent children:<br />
• to the age of 23; or,<br />
• who is unable to support himself or herself because of a physical or mental handicap and<br />
has been continuously incapacitated since attainment of age 23. Proof of incapacity must<br />
be submitted to the Human Resources Office upon enrollment or, if later, within 60 days<br />
after the date on which the dependent no longer will be eligible because of age, and at<br />
reasonable intervals thereafter. You will have a period of 60 days to provide the<br />
information requested by our claims administrator to support your dependent’s eligibility<br />
under this provision. If you fail to provide the requested information within the 60-day<br />
period, coverage for your dependent will terminate. In addition, the dependent child must<br />
meet all of the eligibility requirements other than age to continue to be eligible.<br />
A “Child” is the Participant’s natural born child or legally adopted child, or step child or foster<br />
child who resides in the Participant’s household in a regular parent-child relationship and is<br />
principally dependent(named as an exemption on the Parent’s most current Federal Income Tax<br />
Return) on the Participant for support and maintenance. An eligible “child” also includes:<br />
• A child of a Covered Employee or Covered Dependent spouse required to be covered in<br />
accordance with applicable requirements of any Qualified Medical Child Support Order<br />
as required by ERISA section 609;<br />
• An adopted child placed with the employee for adoption, whether or not the adoption<br />
has become final, who has not attained age 18 as of the date of such adoption or<br />
placement for adoption. The term “placed” means the assumption and retention by such<br />
person of a legal obligation for total or partial support of such child in anticipation of<br />
adoption of such child;<br />
• A child for whom you have been appointed legal guardian or have legal custody and<br />
who resides in the Participant’s household in a regular parent-child relationship;<br />
• A child (who does not reside in the Participant’s household) of a Covered Employee or<br />
Covered Dependent spouse required to be covered in accordance with a divorce decree.<br />
