RECOMMENDATION FOR FRATERNITY DISCIPLINE
This form should be used when the standards committee is recommending that the Executive Board of Delta Delta
Delta consider a member for Fraternity Probation or Fraternity Termination. This form should accompany the
Individual Meeting Minutes from the standards committee meeting in which this recommendation was discussed
with the member.
On behalf of the Chapter of Delta Delta Delta, please accept this as recommendation for Sister

The standards committee must fill out this recommendation in its entirety and must include the required
information as well as any supporting documentation that may assist the executive board in determining the action
that is best for the specific member situation. At the discretion of the executive board, a standards committee may
be asked to submit additional information before making a decision regarding a member's discipline.
