This <strong>for</strong>m should be used when the standards committee is recommending that the Executive Board of <strong>Delta</strong> <strong>Delta</strong><br />
<strong>Delta</strong> consider a member <strong>for</strong> <strong>Fraternity</strong> Probation or <strong>Fraternity</strong> Termination. This <strong>for</strong>m should accompany the<br />
Individual Meeting Minutes from the standards committee meeting in which this recommendation was discussed<br />
with the member.<br />
