RE: 2013 <strong>Benefit</strong> <strong>Open</strong> <strong>Enrollment</strong><br />

As we finish the fall semester, it is time for us to make benefit elections for the upcoming benefit<br />

plan year beginning January 1, 2013. <strong>Benefit</strong> guidelines are strict, allowing for benefit elections<br />

during the open enrollment period, with only a very limited opportunity to make changes at other<br />

times. This year the open enrollment period will run from November 12, 2012 through<br />

November 30, 2012. The deadline for all completed enrollment forms to be received in the<br />

Human Resources Office is 4:30 PM, November 30, 2012.<br />

There will be no changes to our benefit plans for 2013. Again this year the <strong>College</strong> will <strong>of</strong>fer its<br />

medical and dental insurances, as well as, its Health Reimbursement Account (HRA) and Health<br />

Savings Account (HSA) and Wellness Incentive Program through CIGNA. The Flexible<br />

Spending Account (FSA) and Dependent Care Spending Account (DCA) will remain with Group<br />

Dynamics, Inc. Basic life, long term disability and supplemental life insurance programs will<br />

remain with Lincoln.<br />

Please refer to http://www.sjcme.edu/hr/<strong>Benefit</strong>s2013, our open enrollment website under<br />

Human Resources. I recommend you use this website as a key resource when considering your<br />

benefit options. You will find definitions for understanding plan descriptions, frequently asked<br />

questions, helpful links to benefit carriers and prescription drug options, plans summaries, forms<br />

and rates. If you are not electing a benefit, please select ‘waive’.<br />

This year’s enrollment process will require you to complete the <strong>Saint</strong> <strong>Joseph's</strong> <strong>College</strong> <strong>Benefit</strong><br />

Election Form. For every benefit that you elect, refer to the information below explaining<br />

additional carrier enrollment forms that may need to be completed. Forms can be found on the<br />

<strong>Open</strong> <strong>Enrollment</strong> website.<br />

Medical: If your election represents a change in coverage, please complete the SJC<br />

CIGNA enrollment form located on the <strong>Open</strong> <strong>Enrollment</strong> Website and return it with your<br />

enrollment information.<br />

Dental: If your election represents a change in coverage, please complete the SJC<br />

CIGNA enrollment form located on the <strong>Open</strong> <strong>Enrollment</strong> Website and return it with your<br />

enrollment information.


Supplemental Life Insurance: You may choose to add or increase your supplemental<br />

life insurance in increments <strong>of</strong> $10,000 up to 7 times your salary (cap $500,000). This<br />

increase is subject to a successful review <strong>of</strong> your evidence <strong>of</strong> insurability. If you choose<br />

to make this kind <strong>of</strong> change, please visit the <strong>Open</strong> <strong>Enrollment</strong> Website, download an<br />

Evidence <strong>of</strong> Insurability form, and return it with your enrollment information.<br />

Supplemental Life Insurance: If you choose to drop or decrease your supplemental<br />

life insurance please visit the <strong>Open</strong> <strong>Enrollment</strong> Website, download a Group Insurance<br />

Change Form, and return it with your enrollment information.<br />

Medical Care and Dependent Care Reimbursement Accounts (FSA): These are<br />

calendar year plans that allow you to take money out pre-tax and deposit it into one <strong>of</strong><br />

these accounts for use to cover eligible medical expenses or eligible dependent care<br />

expenses. To establish a new account for the calendar year 2013, please complete the<br />

Salary Reduction Agreement included in this packet.<br />

Health Savings Account (HSA): This benefit is paired with the QHDP (qualified high<br />

deductible plan) and allows you to take money out pre-tax and deposit it into this account<br />

for use to cover eligible medical expenses. If you are electing this coverage new for 2013,<br />

visit the <strong>Open</strong> <strong>Enrollment</strong> website and follow the link provided to complete the online<br />

enrollment form where you will be able to create a new CIGNA HSA account. For those<br />

returning and new for 2013, please note your optional annual election on the space<br />

provided on the <strong>Benefit</strong> Election Form.<br />

SJC Wellness Incentive Program: This program provides both general and customized<br />

programs to support your wellness activities. Each program is paired with incentive<br />

points for completion: 1 point = $1 dollar with a maximum <strong>of</strong> 300 points that may be<br />

earned in a calendar year. All wellness rewards will be deposited into your HRA, HSA<br />

or FSA account upon completion <strong>of</strong> each <strong>of</strong> the programs. In order to be eligible for the<br />

SJC Wellness Incentive Program you (and your spouse if covered by SJC), must<br />

complete the online Health Risk Assessment once per year on www.mycigna.com<br />

. You will also need to complete by January 31, 2013 the<br />

Culture Audit. The link to the audit is available on our <strong>Open</strong> <strong>Enrollment</strong> website.<br />

Our health, dental and reimbursement account benefits are part <strong>of</strong> our Cafeteria <strong>Benefit</strong>s<br />

Plan, which allows for pretax premium deductions if you meet certain requirements. I would<br />

like to stress that in order to ensure that you are able to participate in the pretax deduction<br />

program for health and dental premiums and to elect changes in other benefits, your benefit<br />

enrollment material must be completed, dated and returned to the Human Resource <strong>of</strong>fice by<br />

4:30 PM Friday, November 30, 2012 with no exceptions. If you have any questions don’t<br />
