Changes in MHSA Care Coordination
(Reference Communication Bulletin FY 1314 AA 01 20130819)
We are excited to announce some recent changes at
Cardinal Innovations in MHSA Care Coordination. As
part of our MHSA Redesign plan, Cardinal Innovations
MHSA Care Coordination has implemented
improvements to the processes utilized in our clinical
support of children and adolescents that meet
medical necessity and are placed in child residential
Level III and PRTF levels of care. In both our Northern
and Southern regions, we have identified two care
coordinators that will serve as Child Residential
Specialists and be responsible for the monitoring of
participants throughout the duration of their stay in
these specified levels of care.
As part of this redesign, our child residential
specialists will make a minimum of monthly contact
with the residential provider, as well as the
participant and their legally responsible person, to
provide ongoing support during the treatment
process. If a participant is receiving services from a
Level III provider, the child residential specialist will
also participate in monthly Child and Family Team
meetings in support of our System of Care principles
and assist in development of appropriate transition
planning. If a participant is receiving services from a
PRTF provider, our child residential specialists will
facilitate monthly Child and Family Team meetings
and work collaboratively with the team to develop
clinically appropriate transitions.
NC Innovations Respite: Service Location
(Reference Communication Bulletin FY 1314 MP 01 20130823)
Respite services provide periodic support and relief to
the primary caregiver(s) from the responsibility and
stress of caring for the participant. The primary
caregiver is the person principally responsible for the
care and supervision of the participant, and must
maintain his/her primary residence at the same address
as the participant. Respite is only delivered at the
participant's primary residence. Respite is only received
in other community locations if the participant is
accompanied by the respite worker while going on an
outing (eating out, going to the park, etc.) that
originates from the participant’s primary residence.
Respite is not provided for support and relief to family
members that do not maintain their primary residence
at the same address as the participant’s primary
residence.
Provider agencies are responsible for the delivery of NC
Innovations services at locations that are specified in the
waiver. Cardinal Innovations cannot approve the
delivery of services at locations outside of those
specified in the service definition found in the NC
Innovations Technical Guide or NC Innovations Waiver.
NC Innovations Back-up Staffing
(Reference Communication Bulletin FY 1314 MP 02 20130823)
Individual and Family Directed Supports:
Employer of Record Model
For an individual who participates in Individual and
Family Directed Supports the Individual Support Plan
(ISP) must include a back-up staffing plan that is used
when

There is a need to change or fire employees.
The employee quits unexpectedly.
The employee is sick or on vacation.
Back-up staffing can be paid or unpaid supports. Ways
that an Employer of Record can identify back-up staffing
include

Asking regular employees if they know of individuals
who could substitute for them when they are sick or
taking vacation.
Hiring more than one employee to provide services
for the individual. If one employee has an
emergency or is sick, the other may be able to fill in
for the absent employee.
Keeping an updated list of applicants that were
interviewed and check with them from time to time
to see if they are available to work as back-up staff.
If the Employer of Record plans to use an
applicant or other individual as a back-up
employee, all new hire paperwork must be
completed prior to scheduling the back-up
employee for work.
Asking family, friends, or neighbors if they can fill in
when there is an unexpected staffing need, and
determining what community resources are
available. If unpaid individuals are used, services are
not billed, and the Individual and Family Directed
Budget (auto-calculator) is updated, and the
Employer of Record makes a note of how care was
provided to the individual in the absence of
scheduled services. The Community Guide trains and
assists the Employer of Record in updating the autocalculator
and understanding documentation
requirements.
Note: Routine back-up staffing does not include the use
of a provider agency.
For individuals who choose the Employer of Record
Model, Cardinal Innovations has a contract with a backup
staffing agency that can help with staffing needs,
beyond the planned back-up that each Individual
Support Plan includes. There is a single back-up staffing
agency, and this is the only agency that is authorized as a
back-up staffing agency for Cardinal Innovations
Employers of Record. Contact information for the Back-
Up Staffing Agency is provided to Employers of Record by
the Community Guide.
The use of the back-up Staffing Agency is also included in
the Individual Support Plan for individuals participating in
the Employer of Record Model of Individual and Family
Directed Supports for the purpose of unplanned,
emergency staffing needs when the routine back-up
staffing plan is not operational. The auto-calculator also
includes use of the back-up staffing agency when the
Employer of Record expects to use the back-up staffing
agency. The auto-calculator may need to be adjusted if
the use of the back-up staffing agency exceeds the
original projected use of the back-up staffing agency.
Employers must remember to adjust the Individual and
Family Directed Service Budget (auto-calculation) if
overtime pay is authorized to meet unexpected back-up
staffing needs. Using overtime pay for employees on a
regular basis will likely result in over spending of the
Individual and Family Directed Services Budget. This
could result in termination from Individual and Family
Directed Supports.
Community Guides should review this information with
the Employers of Record that they are supporting. If the
back-up staffing plan needs to be revised, the
Community Guide should contact the participant’s Care
Coordinator.
Reminder
If you haven’t completed and returned
your Provider Data Quality Update forms,
please do so as soon as possible.
