Changes in MHSA Care Coordination<br />

(Reference Communication Bulletin FY 1314 AA 01 <strong>2013</strong>0819)<br />

We are excited to announce some recent changes at<br />

<strong>Cardinal</strong> <strong>Innovations</strong> in MHSA Care Coordination. As<br />

part of our MHSA Redesign plan, <strong>Cardinal</strong> <strong>Innovations</strong><br />

MHSA Care Coordination has implemented<br />

improvements to the processes utilized in our clinical<br />

support of children and adolescents that meet<br />

medical necessity and are placed in child residential<br />

Level III and PRTF levels of care. In both our Northern<br />

and Southern regions, we have identified two care<br />

coordinators that will serve as Child Residential<br />

Specialists and be responsible for the monitoring of<br />

participants throughout the duration of their stay in<br />

these specified levels of care.<br />

As part of this redesign, our child residential<br />

specialists will make a minimum of monthly contact<br />

with the residential provider, as well as the<br />

participant and their legally responsible person, to<br />

provide ongoing support during the treatment<br />

process. If a participant is receiving services from a<br />

Level III provider, the child residential specialist will<br />

also participate in monthly Child and Family Team<br />

meetings in support of our System of Care principles<br />

and assist in development of appropriate transition<br />

planning. If a participant is receiving services from a<br />

PRTF provider, our child residential specialists will<br />

facilitate monthly Child and Family Team meetings<br />

and work collaboratively with the team to develop<br />

clinically appropriate transitions.<br />

NC <strong>Innovations</strong> Respite: Service Location<br />

(Reference Communication Bulletin FY 1314 MP 01 <strong>2013</strong>08<strong>23</strong>)<br />

Respite services provide periodic support and relief to<br />

the primary caregiver(s) from the responsibility and<br />

stress of caring for the participant. The primary<br />

caregiver is the person principally responsible for the<br />

care and supervision of the participant, and must<br />

maintain his/her primary residence at the same address<br />

as the participant. Respite is only delivered at the<br />

participant's primary residence. Respite is only received<br />

in other community locations if the participant is<br />

accompanied by the respite worker while going on an<br />

outing (eating out, going to the park, etc.) that<br />

originates from the participant’s primary residence.<br />

Respite is not provided for support and relief to family<br />

members that do not maintain their primary residence<br />

at the same address as the participant’s primary<br />

residence.<br />

Provider agencies are responsible for the delivery of NC<br />

<strong>Innovations</strong> services at locations that are specified in the<br />

waiver. <strong>Cardinal</strong> <strong>Innovations</strong> cannot approve the<br />

delivery of services at locations outside of those<br />

specified in the service definition found in the NC<br />

<strong>Innovations</strong> Technical Guide or NC <strong>Innovations</strong> Waiver.<br />

NC <strong>Innovations</strong> Back-up Staffing<br />

(Reference Communication Bulletin FY 1314 MP 02 <strong>2013</strong>08<strong>23</strong>)<br />

Individual and Family Directed Supports:<br />

Employer of Record Model<br />

For an individual who participates in Individual and<br />

Family Directed Supports the Individual Support Plan<br />

(ISP) must include a back-up staffing plan that is used<br />

when<br />

There is a need to change or fire employees.<br />

The employee quits unexpectedly.<br />

The employee is sick or on vacation.<br />

Back-up staffing can be paid or unpaid supports. Ways<br />

that an Employer of Record can identify back-up staffing<br />

include<br />

Asking regular employees if they know of individuals<br />

who could substitute for them when they are sick or<br />

taking vacation.<br />

Hiring more than one employee to provide services<br />

for the individual. If one employee has an<br />

emergency or is sick, the other may be able to fill in<br />

for the absent employee.<br />

Keeping an updated list of applicants that were<br />

interviewed and check with them from time to time<br />

to see if they are available to work as back-up staff.<br />

If the Employer of Record plans to use an<br />

applicant or other individual as a back-up<br />

employee, all new hire paperwork must be<br />

completed prior to scheduling the back-up<br />

employee for work.<br />

Asking family, friends, or neighbors if they can fill in<br />

when there is an unexpected staffing need, and<br />

determining what community resources are<br />

available. If unpaid individuals are used, services are<br />

not billed, and the Individual and Family Directed<br />

Budget (auto-calculator) is updated, and the<br />

Employer of Record makes a note of how care was<br />

provided to the individual in the absence of<br />

scheduled services. The Community Guide trains and<br />

assists the Employer of Record in updating the autocalculator<br />

and understanding documentation<br />

requirements.<br />

Note: Routine back-up staffing does not include the use<br />

of a provider agency.<br />

For individuals who choose the Employer of Record<br />

Model, <strong>Cardinal</strong> <strong>Innovations</strong> has a contract with a backup<br />

staffing agency that can help with staffing needs,<br />

beyond the planned back-up that each Individual<br />

Support Plan includes. There is a single back-up staffing<br />

agency, and this is the only agency that is authorized as a<br />

back-up staffing agency for <strong>Cardinal</strong> <strong>Innovations</strong><br />

Employers of Record. Contact information for the Back-<br />

Up Staffing Agency is provided to Employers of Record by<br />

the Community Guide.<br />

The use of the back-up Staffing Agency is also included in<br />

the Individual Support Plan for individuals participating in<br />

the Employer of Record Model of Individual and Family<br />

Directed Supports for the purpose of unplanned,<br />

emergency staffing needs when the routine back-up<br />

staffing plan is not operational. The auto-calculator also<br />

includes use of the back-up staffing agency when the<br />

Employer of Record expects to use the back-up staffing<br />

agency. The auto-calculator may need to be adjusted if<br />

the use of the back-up staffing agency exceeds the<br />

original projected use of the back-up staffing agency.<br />

Employers must remember to adjust the Individual and<br />

Family Directed Service Budget (auto-calculation) if<br />

overtime pay is authorized to meet unexpected back-up<br />

staffing needs. Using overtime pay for employees on a<br />

regular basis will likely result in over spending of the<br />

Individual and Family Directed Services Budget. This<br />

could result in termination from Individual and Family<br />

Directed Supports.<br />

Community Guides should review this information with<br />

the Employers of Record that they are supporting. If the<br />

back-up staffing plan needs to be revised, the<br />

Community Guide should contact the participant’s Care<br />

Coordinator.<br />

The Hispanic and Latino LGBTTQ Community<br />

and its Behavioral Health Service Needs<br />

Addiction Technology Transfer Center Network will hold<br />

a free webinar on Wednesday, <strong>August</strong> 28 from 1:00 to<br />

2:00 pm. This webinar will cover the relationship between<br />

Hispanic and Latino LGBTTQ individuals and substance use<br />

and mental health disorders.<br />

Behavioral Health Services Forum<br />

Set for Military Members/Families<br />

On Tuesday, September 10, from 10:00 am-12:00 pm,<br />

<strong>Cardinal</strong> <strong>Innovations</strong> will hold an information-sharing<br />

forum on behavioral health services for service members,<br />

veterans and their families.<br />

The forum will be held at the Warren County Public<br />
