Personally appeared before me, Dan Centofanti, P.G., who after being duly sworn deposes
and says on oath as follows:
1. My name is Dan Centofanti, P.G.. I am over the age of 18 years of age and under no legal
disability.
2. I am a geologist licensed to practice in the state of Georgia and make this affidavit of my
own personal knowledge. I am employed by Mill Creek Environmental Services, Inc..
3. Attached hereto is a true and correct copy of Exhibit “A” (letter dated October 20, 2012,
Subjec: File Review and Property Inspection Ivyshaw Subdivision Gainesville, Forsyth County,
Georgia).
4. Also attached is a true and correct copy of my invoice for the above work, Exhibit “B”.
