PART 2: PERFORMANCE EVALUATION OF GTAS Department: Student’s Name: _______________________ Teaching Assignment: __________________ First Semester Evaluation _____ Semester & Year: ______________ Regular Semester Evaluation _____ This form is to be completed by the faculty member assigned as supervisor to the GTA. It should be completed within the first five weeks of the semester and copies given to the GTA and Director of the GTA’s Graduate Program. Copies of First Semester evaluations are due in the Associate Dean’s Office by the eighth week of classes. It is suggested that all GTAs be evaluated by class/lab visits at least once per year. Please rate the GTA on each of the teaching skills listed below by placing an “X” in the appropriate box (EE = Exceeds Expectations; ME = Meets Expectations; NI = Needs Improvement). Please include comments, especially for students in the categories of EE (should use these in nomination of student for Outstanding award) or Needs Improvement (give specific examples and suggestions). Students receiving a NI rating should be given a follow-up evaluation during the last five weeks of the semester, with copies given to the GTA, GPD and Associate Dean (by last day of final exams). Section I. PERFORMANCE CATEGORIES RATINGS 1. Performance of assigned duties as set out in Part 1 EE ME NI* • Completes assignments and meets commitments and deadlines (e.g., keeps office hours, returns student work in a timely manner, etc) • Attends required meetings and/or training sessions • Interacts effectively and maintains a professional demeanor with a wide diversity of individuals and work styles and is receptive to feedback 2.
