This form is to be completed by the faculty member assigned as supervisor to the <strong>GTA</strong>. It should<br />

be completed within the first five weeks <strong>of</strong> the semester <strong>and</strong> copies given to the <strong>GTA</strong> <strong>and</strong><br />

Director <strong>of</strong> the <strong>GTA</strong>’s Graduate Program. Copies <strong>of</strong> First Semester evaluations are due in the<br />

Associate Dean’s Office by the eighth week <strong>of</strong> classes. It is suggested that all <strong>GTA</strong>s be evaluated<br />

by class/lab visits at least once per year.<br />

Please rate the <strong>GTA</strong> on each <strong>of</strong> the teaching skills listed below by placing an “X” in the appropriate<br />

box (EE = Exceeds Expectations; ME = Meets Expectations; NI = Needs Improvement). Please<br />

include comments, especially for students in the categories <strong>of</strong> EE (should use these in nomination <strong>of</strong><br />

student for Outst<strong>and</strong>ing award) or Needs Improvement (give specific examples <strong>and</strong> suggestions).<br />

Students receiving a NI rating should be given a follow-up evaluation during the last five weeks <strong>of</strong><br />

the semester, with copies given to the <strong>GTA</strong>, GPD <strong>and</strong> Associate Dean (by last day <strong>of</strong> final exams).<br />
