4. 혈전용해 요법(thrombolysis) • 혈역학적으로 불안정한 환자 혹은 대량의 장대퇴골 혈전증이 있는 환자, 즉 acute massive PE 환자에서 first line therapy로 사용된다. • 효과는 폐동맥을 막고 있는 혈전을 상당부분 용해시 키고, 혈전에서 나오는 세로토닌과 같은 폐 고혈압 유발물질의 유리를 방지하며, 골반이나 하지 정맥의 혈전까지 용해하는 효과가 있다.
• 성공적인 혈전용해 요법은 우심실 부전을 조속하 게 회복시키고 사망률과 APE의 재발을 감소시킨다. • 선호되는 방법은 2시간에 걸쳐 r-TPA 100mg을 정맥 주사는 것이며 APE 발생 후 14일까지는 혈전용해 치 료에 대해 반응한다. • 그러나 실제 임상에서는 출혈 합병증이나 금기등으 로 치료의 제한이 상당히 많다. 최근 한 임상연구에 서는 massive PE 환자에서 thrombolysis가 의미 있는 사망률 감소효과가 없다는 보고한 바도 있다.
