<strong>Medical</strong> Ethics, Human Rights and Socio-medical affairs<br />

Ethical Trade Group (MFETG), which is<br />

an independent group with membership<br />

from fair trade groups, industry associations<br />

and government bodies. The MFET Group<br />

is coordinated by the BMA International<br />

Department and the promotion of fair trade<br />

in healthcare goods is a key objective for the<br />

BMA’s work on improving global health.<br />

The Group aims to promote fair and ethical<br />

trade in the manufacture and supply of<br />

medical commodities. Recent successes include<br />

a code of conduct for all major suppliers<br />

of healthcare goods to the UK, and the<br />

development of guidance by the NHS Purchasing<br />

and Supply Agency - the group responsible<br />

for national purchasing decisions.<br />

These tools enable the UK health system to<br />

question suppliers about whether they have<br />

adhered to basic labour rights in the production<br />

and supply of their goods. The British<br />

<strong>Medical</strong> <strong>Association</strong> will be engaging with<br />

its members and other health professionals<br />

to help them learn to ask the right questions<br />

and bring change to these industries. Another<br />

exciting effort is supporting the development<br />

of product lines that may represent<br />

especially good practice in fair and ethical<br />

trade, including fair trade and environmentally<br />

friendly rubber for surgical gloves, and<br />

fair trade cotton for healthcare textiles.<br />

In Sweden, the three largest authorities for<br />

health care have jointly produced a Code of<br />

Conduct for fundamental social responsibility<br />

in procurement contracts with suppliers,<br />

and there are plans to make the Code<br />

apply nationally in the near future. Recently<br />

signed contracts will be monitored annually,<br />

and suppliers asked how they comply with<br />

the contract. If problems are discovered,<br />

further steps include requesting the supplier<br />

to contact the producing factory or to undertake<br />

independent audits to assess labour<br />

conditions.<br />

Both the guidance in England and the<br />

Swedish Code of Conduct contain the following<br />

points as key principles of developing<br />

fair and ethical trade:<br />

• Suppliers shall respect principles of fundamental<br />

social responsibility in business<br />

deals.<br />

• All products shall be produced according<br />

to ILO and UN fundamental labour<br />

conventions and shall be related to health<br />

and safety legislation in the manufacturing<br />

country.<br />

• Discrimination, forced labour or child labour<br />

must not occur.<br />

• Wages shall be paid as agreed directly to<br />

workers.<br />

• Remuneration must not be below national<br />

statutory minimum wage.<br />

• Weekly working hours must not exceed<br />

legal limit and overtime is to be paid for.<br />

• Working environment shall correspond<br />

to international guidelines.<br />

When combined, the policies in the UK<br />

and Sweden have the potential to affect billions<br />

of dollars of health service contracts<br />

per year. However, this is still only a small<br />

percentage of healthcare expenditure: the<br />

UK and Sweden together account for only<br />

7% of hundreds of billions of dollars global<br />

purchasing of medical devices. If guidance<br />

and policies were developed in other countries<br />

throughout the world, the impact could<br />

be much more significant. But this will only<br />

happen if key partners work together, and<br />

with suppliers, to identify problems and fix<br />

them.<br />

We are now working with other medical<br />

associations in Europe and recommend the<br />

following actions for all medical associations:<br />

• Contact the British <strong>Medical</strong> <strong>Association</strong><br />

for advice (email fairtrade@bma.org.uk).<br />

• Contact your members about this issue to<br />

get their support.<br />

• Contact your national health service purchasing<br />

agency or regional body to ask<br />

them to engage on this issue.<br />

• Contact your local industry association<br />

and ask them about their supply chains.<br />

We need you to join us to mobilise health<br />

professionals throughout the world to insist<br />

that their health system treats the world<br />

fairly.<br />
