INSTRUCTIONS TO FILL THE ADMISSION FORM nEach prospectus contains two forms; a student can apply in any two colleges/two courses. nFill a separate form for each college and each course. nWrite in blue ink. nFill up all the columns carefully. nWrite neatly. Forms with overwriting shall not be accepted. nGive the full form of the Board/University from which you have passed the last examination, eg Punjab School Education Board. nAttach the character certificate from the institution (recognized and affiliated) last attended. Private candidates should attach a character certificate issued by a gazetted officer. nAttach attested copies of the following certificates: nAffix one copy of your passport size photograph on the specified column / place in the Admission Form. nApplication for admission must be submitted within the specified period as given by the colleges in their respective sections. nApplications incomplete in any respect or received in the office after the expiry of due date shall not be entertained. nPlease collect the Acknowledgment Slip from the college in which you are applying for admission. Important 1. Bring all original certificates and documents at the time of interview. 2. Parents/Guardians should sign the declaration and undertaking. i) Matriculation/Sr. Secondary certificate showing date of birth ii) Board/University certificate showing the previous examination passed iii) Certificates of merit/prizes won, if any iv) Proof of your residence i.e. Ration Card / Driving License / Voter's Identity Card (any one) nTick mark Reservation, if any. If Reserved Category, attach the required documents. nIf you have been declared pass in +2 but have failed in one or more subjects, mention this fact clearly in the column of the previous class passed. Note Old students can purchase the Admission Form for Rs.5/-each. i) U.T. Pool Candidates Those who have passed their lower qualifying examination as regular candidates from schools and colleges in U.T., Chandigarh. ii) General Pool Candidates Those who have passed their lower qualifying examination from schools and colleges outside U.T.,Chandigarh. nFill the details of the classes passed.
