authorized and certified within the first three (3) years).<br />
Contractor shall indemnify and hold harmless the City & County of San Francisco, its officers and employees and<br />
furnish certificates of insurance directly protecting himself, any subcontractors and the City & County of San<br />
Francisco. The City & County of San Francisco, its officers, agents and employees shall be named as additional<br />
insureds and insurance shall be for:<br />
Commercial General Liability<br />
(bodily injury and property damage)<br />
Business Automobile Liability<br />
(bodily injury and property damage)<br />
Workers’ Compensation; Employers’ Liability<br />
Professional Liability<br />
$1,000,000 Single Limit<br />

The Accounting Division Manager, Department of <strong>Public</strong> <strong>Works</strong>, shall issue CONTRACT PURCHASE<br />
ORDERS to the Contractor on an as-needed basis upon receipt of complete Contract Service Orders (CSO’s)<br />
