ENTRY BLANK – RELEASE
BE SURE AND READ THE ENCLOSED INFORMATION CAREFULLY!
The undersigned, for and in consideration of the right to drive and participate in the NORTHWEST
MONTANA FAIR DEMOLITION DERBY at the Flathead County Fairgrounds, Kalispell, Montana, hereby
releases the Flathead County Fair Board, its officers and/or agents, Flathead County, and KOFI/Monster
from any and all liability and responsibility for any damages to the owner’s car, bodily injury , or death
which may result from preparations for or participation in the DEMOLITION DERBY at the Flathead
County Fairgrounds, or any activities incidental thereto.
It is further understood and agreed that the undersigned is at least eighteen (18) years of age or older.
The further consideration for this release and waiver is the right to participate in the prize money
offered to the winners.

For value received and privilege of participation in competition in the Flathead County Fairground’s
DEMOLITION DERBY, and other valuable considerations, hereby acknowledged, I hereby waive any and
all claims I might have or assert against the Flathead County Fair Board, Flathead County, or any other
persons connected therewith for loss and damage.
It is understood and agreed that I am entering these contests at my own risk and responsibility, both for
my person and property and I agree to read, abide by, and be bound by the rules and regulations
governing the NORTHWEST MONTANA FAIR DEMOLITION DERBY.
