Instructions to correctly fill up the Application form for an Indian Visa
1 a- Name as it appears in the passport.
1 b-FATHER´s given name has to be written here ( even if he has expired )
1 c- Husband´s name , if the applicant is married.
2 This is only filled if there is a minor included in the passport, and is of 15 years of age and
below. If he/she needs a visa you must fill in this and include 3 photographs for the visa and no
fees will be applied. If the child holds his/her own passport, he/she must fill out an application
form and pay the corresponding fees.

b- present , if it is the same, please specify it is as above/the same
6.Present Nationality
7. Previous Nationality with details - if you do not hold any other nationality please put NONE .
8.You must indicate if you have ever changed your name. YES / NO. If yes, you must include
copy of a legal document that certifies that.
9. Passport Information: number, place and dates of emission of the passport. All this
information is found on the main page of your passport.
a) passport number
b) Place of issue - if you know where the passport was issued please write the city, if
not the issue place is specified on the passports as a DGP followed by a number (in Spain)
c) Date of Issue of passport
d) Date of Expiry of passport.
10. Please specify your current profession/occupation - If your profession is Journalist you
MUST send an email to Ms.Paulomi Tripathi at culture@embassyindia.es with a fotocopy of
your press card, brief description of object of journey, dates of entry/exit to india (if possible
attach itenery). Arke BLS Center will not process visas of Journalists without prior confirmation
from the Ms. Paulomi Thripathi ( Press and Information wing in the Embassy). ALL
JOURNALISTS/PROFESSIONAL PHOTOGRAPHERS SHOULD APLY FOR ONLY
JOURNALIST VISA EVEN THEY ARE VISITING INDIA FOR OTHER PURPOSES.
11. Specify YES or NO , if you, parents or grandparents ever held Pakistan nationality and bring
any documents that certify that. I
12. Please specify if you have visited India previously. In the case of YES please specify
approximate dates (for example, month and year)
