<strong>The</strong> <strong>attached</strong> <strong>forms</strong> <strong>can</strong> <strong>be</strong> used to request permission to remove your child(ren) from the State of<br />

Michigan or to move greater than 100 miles from your current legal residence.<br />

<strong>The</strong> motion requesting change of domicile must <strong>be</strong> filed with the Circuit Court Records Office.<br />

<strong>The</strong>re is a filing fee of $20.00. Circuit Court Records will record your payment and send your<br />

form to the Circuit Court Trial Division Assignment Clerk who will complete the “NOTICE <strong>OF</strong><br />

HEARING” section. <strong>The</strong> Assignment Clerk will add the date, time and place to appear for the<br />

hearing on the notice and mail it to you and the other party. Your hearing will <strong>be</strong> scheduled<br />

within 4-6 weeks as the Court’s calendar allows<br />

You must turn in the original copies of all <strong>forms</strong>.<br />

After the hearing you will <strong>be</strong> required to prepare an order allowing or denying the change of<br />

domicile. This order must then <strong>be</strong> given to the Judge for his signature. <strong>The</strong> original order then<br />

would <strong>be</strong> filed with the Circuit Court Records office. You then should provide a copy to the<br />

other party and keep a copy for yourself.<br />
