Inaugural <strong>meeting</strong> <strong>Moray</strong> Long Term Conditions Stakeholders<br />

I am pleased to invite you to attend the <strong>inaugural</strong> <strong>meeting</strong> of the <strong>Moray</strong> Long Term Conditions<br />

Stakeholders on Thursday the 21 st of January 2010 at the new Dental Cent<strong>re</strong>, Spynie Hospital<br />

Duffus Road, Elgin, IV30 5PW at 11.30am. To be chai<strong>re</strong>d initially by Tracey Gervaise (Public<br />

Health Lead MCH&SCP). A light lunch will be provided.<br />

As you will be awa<strong>re</strong> managing long term conditions is seen as the biggest challenge facing<br />

health and social ca<strong>re</strong> services worldwide with 60% of deaths attributable to them.<br />

P<strong>re</strong>valence inc<strong>re</strong>ases with age and the elderly population in <strong>Moray</strong> is projected to inc<strong>re</strong>ase<br />

from 18% of the population to 39% of the population by 2031<br />

Long Term Conditions has been identified as an NHS Grampian, MCHSCP and <strong>Moray</strong> Single<br />

Outcome Ag<strong>re</strong>ement priority.<br />

The interdependencies of this programme activity and other projects cur<strong>re</strong>ntly underway in<br />

<strong>Moray</strong> cannot be understated .e.g. Older Peoples Strategy, shifting the balance of ca<strong>re</strong> and<br />

Rehabilitation Framework.<br />

A multi-agency <strong>Moray</strong> Long Term Conditions Steering Group has been established and high<br />

impact changes ag<strong>re</strong>ed at a Grampian level. Each HIC has a bundle of improvement actions.<br />

The overall emphasis is on providing the majority of ca<strong>re</strong> in the community as locally as<br />

possible, with a focus on improving health and <strong>re</strong>ducing health inequalities; providing a mo<strong>re</strong><br />

integrated and targeted approach in local settings; <strong>re</strong>ducing hospital admissions; providing<br />

systematic support for people with long term conditions; and allowing patients and ca<strong>re</strong>rs to<br />

have mo<strong>re</strong> say in what services they <strong>re</strong>ceive.<br />

The local steering group and the above stakeholders group will be <strong>re</strong>sponsible for the<br />

development of a <strong>Moray</strong> Long Term Conditions Work Plan 2009-2010 and clear and robust<br />

communication mechanisms and performance management. The plan in its initial stages will<br />

include the p<strong>re</strong>-identified and ag<strong>re</strong>ed rapid service improvement programmed <strong>re</strong>qui<strong>re</strong>d locally.<br />

The<strong>re</strong> will be some p<strong>re</strong>sentations and examples of cur<strong>re</strong>nt initiatives in <strong>Moray</strong> (to be conformed<br />

at a later date.<br />

The thoughts, views and opinions of you and the organisation you <strong>re</strong>p<strong>re</strong>sent a<strong>re</strong> important.<br />

The success of the Long Term Conditions programme which is expected to contribute to both<br />

HEAT and Community Ca<strong>re</strong> Outcome measu<strong>re</strong>s; if you a<strong>re</strong> unable to attend please nominate a<br />

deputy to attend.<br />
