Inaugural meeting Moray Long Term Conditions Stakeholders
I am pleased to invite you to attend the inaugural meeting of the Moray Long Term Conditions
Stakeholders on Thursday the 21 st of January 2010 at the new Dental Centre, Spynie Hospital
Duffus Road, Elgin, IV30 5PW at 11.30am. To be chaired initially by Tracey Gervaise (Public
Health Lead MCH&SCP). A light lunch will be provided.
As you will be aware managing long term conditions is seen as the biggest challenge facing
health and social care services worldwide with 60% of deaths attributable to them.
Prevalence increases with age and the elderly population in Moray is projected to increase
from 18% of the population to 39% of the population by 2031
Long Term Conditions has been identified as an NHS Grampian, MCHSCP and Moray Single
Outcome Agreement priority.
The interdependencies of this programme activity and other projects currently underway in
Moray cannot be understated .e.g. Older Peoples Strategy, shifting the balance of care and
Rehabilitation Framework.
A multi-agency Moray Long Term Conditions Steering Group has been established and high
impact changes agreed at a Grampian level. Each HIC has a bundle of improvement actions.
The overall emphasis is on providing the majority of care in the community as locally as
possible, with a focus on improving health and reducing health inequalities; providing a more
integrated and targeted approach in local settings; reducing hospital admissions; providing

systematic support for people with long term conditions; and allowing patients and carers to
have more say in what services they receive.
The local steering group and the above stakeholders group will be responsible for the
development of a Moray Long Term Conditions Work Plan 2009-2010 and clear and robust
communication mechanisms and performance management. The plan in its initial stages will
include the pre-identified and agreed rapid service improvement programmed required locally.
There will be some presentations and examples of current initiatives in Moray (to be conformed
at a later date.
The thoughts, views and opinions of you and the organisation you represent are important.
The success of the Long Term Conditions programme which is expected to contribute to both
HEAT and Community Care Outcome measures; if you are unable to attend please nominate a
deputy to attend.
