<strong>Research</strong> <strong>Project</strong>: Onl<strong>in</strong>e learn<strong>in</strong>g <strong>in</strong> a mentored, Face-to-Face educational environment.<br />

Introduction: Th<strong>an</strong>k you <strong>for</strong> agree<strong>in</strong>g to help with this research. The purpose of this study is to<br />
evaluate the effectiveness of on-site face-to-face peer mentor<strong>in</strong>g comb<strong>in</strong>ed with <strong>in</strong>dependent on-l<strong>in</strong>e<br />
learn<strong>in</strong>g as a method to <strong>in</strong>crease teacher underst<strong>an</strong>d<strong>in</strong>g of <strong>an</strong>d implementation of technology to<br />
assist <strong>in</strong> the delivery of curriculum.<br />
Benefits: The most subst<strong>an</strong>tial benefits of the research are those ga<strong>in</strong>s made by the particip<strong>an</strong>ts <strong>in</strong><br />
better underst<strong>an</strong>d<strong>in</strong>g how to use new hardware <strong>an</strong>d software technology tools <strong>an</strong>d <strong>in</strong> the use of<br />
technology with<strong>in</strong> their curriculum.<br />
Risks, Inconveniences, Discom<strong>for</strong>ts: There are no risks associated with this study. Your responses<br />
are completely <strong>an</strong>onymous, <strong>an</strong>d the questions themselves are not personally prob<strong>in</strong>g ones.<br />
Cost of <strong>Participation</strong>: The only cost to you is the time you take to participate. I am ask<strong>in</strong>g you to<br />
take 60-90 m<strong>in</strong>utes out of your lengthy <strong>in</strong>volvement <strong>in</strong> the technology cadre, so there is a m<strong>in</strong>or<br />
<strong>in</strong>convenience <strong>in</strong>volved.<br />
Confidentiality of Records: I will treat your identity with professional st<strong>an</strong>dards of confidentiality.<br />
The <strong>in</strong><strong>for</strong>mation we obta<strong>in</strong> will be <strong>an</strong>alyzed statistically, <strong>an</strong>d general f<strong>in</strong>d<strong>in</strong>gs will likely be published <strong>in</strong><br />
scientific journals, but <strong>in</strong>dividual particip<strong>an</strong>ts' identities will not be known.<br />
Withdrawal: <strong>Participation</strong> is voluntary. We will <strong>an</strong>swer <strong>an</strong>y questions you may have about the study.<br />
You are free to withdraw your consent <strong>an</strong>d discont<strong>in</strong>ue participation at <strong>an</strong>y time. If you decide to<br />
withdraw from this study, you will still receive <strong>an</strong>y credit <strong>for</strong> participat<strong>in</strong>g that was promised you.<br />

prejudice, <strong>an</strong>d recognize that my activities <strong>an</strong>d data generated by my participation<br />
will rema<strong>in</strong> strictly confidential. I also underst<strong>an</strong>d that at the conclusion of the study I<br />
c<strong>an</strong> choose to destroy <strong>an</strong>y records of my participation, <strong>an</strong>d that if I desire I c<strong>an</strong><br />
request a copy of the f<strong>in</strong>al report describ<strong>in</strong>g the research's conclusions.<br />
