Cancellations & Refunds
If written cancellation is received by November 1,
2012, a full refund minus a $100 processing fee.
Written cancellation requests received by Friday,
November 9, 2012 will be granted 50 % refund minus
the $100 processing fee. No refunds will be made
after Saturday, November 10, 2012. In the unlikely
event that the program is cancelled or rescheduled, a
full refund of the registration will be provided.

Texas Medical Association to provide continuing
medical education for physicians. The Medical City
Dallas Hospital designated this live activity for a
