Welder, Date of Accident - May 3,2012. Injured was assigned to patch
the back side of a rougher tub with Nordbak. There were some small
wear areas that needed patching. The injured had applied Nordbak to
three holes. The injured picked up the 25 lb. pail of Nordbak and
began to back up, he stepped into the tails launder area of the tub
(approximately ~20 in. drop sloped surface). This resulted in the
injured twisting his right knee when his leg dropped in the launder.
Upon further medical evaluation, injury occurred to cartilage/ligaments
in his knee and resulted in surgical repair in September 2012. Days

Furnace Attendant, Date of Accident - July 11, 2012. Injured was
pulling a wash down hose that was coiled up on the north end of the
dribble belts. He was pulling the hose down the walkway to hose out
the 1B feeder. As he was pulling the coiled hose out of the mud, the
hose became tangled. The employee then pulled harder to free it,
resulting in the left shoulder pain. Injured employee opted for surgery
in October 2012. Days Lost - 40
