4. How quickly were you contacted by the county after a referral was made?<br />

5. Tell me about your first meetings/contacts with Birth to 3.<br />

SERVICE COORDINATION<br />

6. Describe to me the role you felt the service coordinator has/had with your family.<br />

What were his/her responsibilities with your family?<br />

7. How often do you talk with your service coordinator?<br />

8. How would you rate, from 1-10, the services provided by your service coordinator?<br />

9. Tell me about how the Birth to 3 Program coordinates what they do with other<br />

services/programs you participate in.


EVALUATION AND IFSP<br />

10. Did the Birth to 3 Program explain who was to be involved in your child’s<br />
