<strong>Community</strong> <strong>Care</strong> Offers<br />

Information<br />

We answer questions about your health plan and your benefits. We<br />

answer your questions about your mental health or drug and alcohol<br />

treatment plans. Ask about self-help groups in your community.<br />

Referrals<br />

We refer you to health and social services not covered by your health<br />

plan. We can also refer you to self-help and support groups.<br />

Treatment<br />

We can help you find treatment for a mental health or drug and alcohol<br />

problem and help you make an appointment.<br />

Transportation<br />

We help you get to your mental health or drug and alcohol appointments<br />

and to our meetings.<br />

<strong>Health</strong> Coordination<br />

We help all of your health care providers work together to meet your<br />

needs.<br />

<strong>Care</strong> Management<br />

Your <strong>Care</strong> Managers are health professionals such as<br />

social workers, nurses or psychologists who work for<br />

<strong>Community</strong> <strong>Care</strong>. They will work with you and your<br />

provider to plan your treatment for behavioral health<br />

care services.<br />

Emergency Help<br />

<strong>Care</strong> Manager:<br />

A health care<br />

professional,<br />

such as a nurse,<br />

social worker, or<br />

psychologist,<br />

who works at<br />

<strong>Community</strong><br />

<strong>Care</strong>.<br />

Call your county’s Crisis <strong>Services</strong> Hotline 24 hours a day, 7 days a week<br />

if you have a behavioral health emergency. (See Important Contact<br />

Information.) Or call 911 for emergency help. You can call <strong>Community</strong><br />

<strong>Care</strong> or a <strong>Community</strong> <strong>Care</strong> provider in addition to calling your local crisis<br />

service.<br />
