Objectives
The Health Systems 20/20 Project is a Leader with Associates award that helps USAID-supported
countries solve problems in health governance, finance, operations, and capacity building. By
working on these dimensions of strengthening health systems, HS20/20 enables people in developing
countries to gain access to and use priority population, health, and nutrition (PHN) services.

The delivery of all health services, including priority PHN services, depends on the underlying
health system. To combat malaria, TB, HIV, and maternal and child health problems, the health
system needs adequate and appropriately allocated financing, inclusive decision making and
accountability, and financial and human resource management systems that deliver inputs where
and when needed. HS20/20 takes a holistic approach to the health system by addressing its
weaknesses comprehensively in the areas of
financing, governance, and operations, while
building sustainable capacity for addressing system
issues across these areas.
• Finance – HS20/20 partners with national
actors to improve mobilization and allocation
of financing, reduce financial barriers to the
use of priority services, especially by the
disadvantaged, and introduce incentives for
efficiency and quality.
• Governance – HS20/20 works with citizens and
oversight entities inside and outside of
government to enhance their voice and
accountability, while simultaneously promoting
institutional arrangements that enhance the
likelihood of improved governance.
• Operations – HS20/20 provides expertise in
process management, tools for financial and
human resources management, and training to address weaknesses in planning,
implementing, and organizational processes in a sustainable manner.
• Capacity Building – HS20/20 helps key stakeholders effectively contribute to systems
strengthening efforts through development of supportive policies and institutional
arrangements, strengthening mechanisms to engage stakeholders, and developing the capacity
of institutions and individuals.

In late 2006, HS20/20 launched global leadership activities in all of the above topic areas and
began implementing field support projects in Côte d’Ivoire, Ghana, Kenya, Rwanda, and Yemen.
HS20/20 continues to provide global leadership on National Health Accounts and sub-accounts
on reproductive health, child health, and HIV/AIDS. In HIV/AIDS, HS20/20 is working to
address human resource issues in African countries and to implement ART Information Systems
(ARTIS) to capture patient information for national monitoring and evaluation. In Yemen,
HS20/20 is helping to strengthen health system stewardship, increase use of health information
systems to improve planning, strengthen the role of midwives to improve reproductive health
services, and building capacity to improve routine immunizations. With REDSO funds, HS20/20
is planning a regional conference in Africa to share what is known about performance-based
financing and identify next steps for design, implementation, and evaluation.
