Complementary options<br />
Accuracy in the diagnosis of aneuploidy using PGS-FISH<br />
varies between IVF clinics. Increasing the accuracy rate of<br />
tests may <strong>help</strong> to improve IVF success rates. One strategy is<br />
to combine PGS-FISH w<strong>it</strong>h a number of complementary<br />
options that are inexpensive, quick, reliable and perhaps,<br />
noninvasive. F<strong>or</strong> example, the selection of gametes <strong>or</strong><br />
embryos based on m<strong>or</strong>phology and developmental characteristics<br />
is a simple, fast and relatively inexpensive in<strong>it</strong>ial<br />
step—an ideal complementary option to PGS-FISH. In this<br />
regard, the location of the PGS lab<strong>or</strong>at<strong>or</strong>y may be imp<strong>or</strong>tant<br />
fact<strong>or</strong>. Is the diagnosis perf<strong>or</strong>med “in house” <strong>or</strong> are the<br />
fixed cells sent to an outside PGS lab<strong>or</strong>at<strong>or</strong>y? Although<br />
sending fixed materials to a reference PGS center is usually<br />
m<strong>or</strong>e cost effective, “in house” PGS-FISH <strong>does</strong> provide<br />
some unique benef<strong>it</strong>s, including immediate embryo rebiopsy<br />
(if needed, due to a lack of a nucleus from the cell<br />
that first was removed). It may also perm<strong>it</strong> direct communication/consultation<br />
between the embryologist and PGS staff<br />
on individual embryo qual<strong>it</strong>y and other parameters that are<br />
imp<strong>or</strong>tant to know when FISH analyses are perf<strong>or</strong>med.<br />
Sperm parameters A low sperm qual<strong>it</strong>y has been c<strong>or</strong>related<br />
w<strong>it</strong>h a higher incidence of embryo aneuploidy<br />
[133]. Here, we discuss some specific sperm parameters<br />
that are simple and inexpensive to assess, and when<br />
combined w<strong>it</strong>h PGS-FISH technique, will improve the<br />
success of fertil<strong>it</strong>y.<br />
