2010 Cost Report Training: Beginning, Intermediate and Advanced
For the first time in 15 years, CMS has significantly updated the Hospital Medicare cost report forms by deleting unused sections, adding new
data requirements, combining the Form 339 into the actual cost report, and reorganizing the forms to be more uniform for all hospitals. We will
try to ease your concerns about the new forms, since most of the changes involve the same data being reported in different locations, expose
some of the myths about the new forms, provide a crosswalk that will link the old forms to the new forms, and review information that should be
reclassified by hospitals from an optional category to a critically important category.
In order to gain a full understanding of the new 2010 cost report, our presentation will go over the following three categories
Beginning:
• Understanding cost reports basics – how does the cost report work
• The importance of accurate hospital source data - most critical part of any cost report filing
• Where to find departmental cost to charge ratios, per diem costs, RHC productivity calculations, and other useful information
• How to read a Medicare processed claims summary (PS&R) – solve the mystery
Intermediate:
• Detailed handout (crosswalk) identifying departmental line changes form old to new cost report forms
• List of new cost report departments
• Identifying the differences between the old and new forms
• New cost report myths and misunderstandings
Advanced:
• Reporting EHR cost and interim payments in order to arrive at a final settlement
• Completing the newly required charity care worksheet S-10
• Matching billed revenue codes with the correct departments on the cost report

Preserving Independence Through Hospital-Physician Alignment
In the wake of health care reform and the growing pressure on providers to be connected with health systems through mechanisms such
as health information exchange, electronic health records, PACS, and billing systems, there will also be increasing accountability for the
coordination and efficiency of care. This can present awkward and, at times, frustrating circumstances for those physicians wishing to
retain their independence while recognizing the need for an alignment strategy. Such circumstances can be exacerbated if their local
hospital is also pursuing an upstream affiliation with a regional health system. The decisions of how to align, with what organization, and
under what conditions not only have considerable impact on the clinical care model, but also can greatly influence the future hospitalphysician
relationship. In this session we will review the process by which an alignment strategy is built and alternative alignment models
and mechanisms that can be deployed.

Improving Operational Performance
Reform, economics, and competition are all demanding a different approach to care delivery–an integrated, patient-centered, lower-cost model
with improved clinical outcomes. How do we get there? This session will review the anticipated changes to care delivery models and the
implications on operations. We will discuss the importance of linking operational priorities with organization strategy, highlight potential areas
for operational improvement, and discuss the role of the various stakeholders in achieving the desired improvement. In addition, we will review
proven methodologies for driving clinical and financial improvements and discuss the role of performance management tools to sustain the
improvements.

We will present common findings that are uncovered when we perform documentation and billing reviews for CAHs. Often there are
misunderstandings regarding whether or not rules and regulations are applicable to CAHs (often they are!). We will present real examples,
discuss the relevant regulations, and identify where they can be found.

CAH Strategies and Opportunities
The economic climate is changing rapidly, and resources are becoming scarce. Every CAH has opportunities to improve cost-based
reimbursement. These improvements may be in the form of reporting more accurately, making elections, and identifying and implementing other
changes that are permitted within the Medicare regulations. Explore the most commonly overlooked opportunities and learn how to develop a
tool that will assist with the evaluation of changes.

Everything You Ever Wanted to Know About Rural Health Clinic Billing and Reimbursement
Both freestanding and provider-based RHCs are challenged to bill for their services correctly and complete Medicare cost reports annually.
We will review all aspects of RHC and non-RHC billing requirements, the proper method for reporting visits, FTEs, and other information on
the cost report in an effort to maximize RHC reimbursement. During this session, we will discuss strategies and options that are available to
avoid reimbursement pitfalls. In addition, we will discuss Medicare Advantage contracts and payment requirements. Medicaid managed-care
supplemental payments from the state will also be addressed from a federally mandated minimum standpoint. If CMS releases new RHC rules
or HRSA issues new HPSA rules in final or in draft, we will review those rules in detail. Currently, we do not know when such rules will be
issued, if ever.
