Do we educate people to be physically active and nutritionally fit, thus preventing chronic 

disease? Or should we fund health programs that address early intervention? Perhaps 

working with nonprofits to provide a safety net for uninsured/underinsured individuals, 

or partnering with higher education and other entities to tackle the growing shortage of 

healthcare providers, especially in rural areas?

According to a 2008 survey of DMF members, area nonprofits and health providers, along 

with regional data, it’s “yes, to all of the above”! To determine how we best improve health, 

DMF’s Strategic Planning Committee was tasked with weaving the above themes into our 

2009-2011 Strategic Plan. To do this, they identified two main areas of health intervention: 

Prevention and Access. 

Historically, we’ve worked with prevention programs such as Go Red North Dakota to 

educate women about preventing heart disease. Through Altru Health System’s Diabetes 

Prevention Program, we identified persons at risk of diabetes and enrolled them in a 

prevention program. Our Worksite Wellness pilot assessed the health risks of employees 

and implemented interventions to reduce risk. Access to healthcare has been addressed 

through the Prescription Assistance Program; Automated External Defibrillator, Dental 

Access, and Healthcare Workforce Shortage initiatives; and DMF’s Lend A Hand program. 

DMF’s Children’s Mental Health Initiative is working hard to improve early identification, 

intervention and access to mental health services for children ages 0-8. 

In 2009-2011, we’ll launch a new prevention program - the Cass Clay Healthy People Initiative, 

a community approach to encouraging active living and healthy eating. Our worksite wellness 

program will consider new partnerships to bring prevention and early intervention programs 

to more worksites across our region. Improved access to healthcare services will be achieved 

through implementation of effective healthcare workforce development strategies. Major 

conferences for professionals and the general public will deliver critical health education to 

our region. 

The future is exciting for Dakota Medical Foundation, as well as our nonprofit partners! A 

$2 million Bush Foundation grant to our strategic partner, Impact Foundation’s Institute 

for Innovation and Effectiveness, will continue to help build the capacity of nonprofits to 

more effectively improve health. Expanded Institute services include nonprofit board and 

executive leadership training, as well as other statewide capacity building education and 

training opportunities. 

How do we best ensure your good health? We will identify barriers to “good” health and 

work strategically with our partners to eliminate them. We will assess the capacity needs of 

health-related nonprofits and help address them. And we will listen to you – our members, 

our partners, and our communities. Together, we can achieve our ambitious goals!

Initiatives Update 

Automated External 

Defibrillators (AEDs) 

Regional 

2001-Present 

To provide immediate emergency medical treatment to victims 

of cardiac arrest in order to improve mortality rates. 

- DMF has invested over $1 million* 

- By September 2009, 482 AEDs will be placed in public 

locations, police squad cars, and rural first responder vehicles 

(see www.dakmed.org/grantmaking/initiative/aed.asp 

for a list of placement sites to-date) 

- Over 1,300 persons trained on equipment usage 

- 20 lives saved 

Children’s Mental Health 

CASS COUNTY, ND/CLAY COUNTY, MN 

2007-Present 

To facilitate the development of community resource tools, 

community education and collaborative efforts related to children’s 

mental health services. 

- DMF has invested $142,000* 

- Coordinator hired in 2007 

- A children’s mental health resource guide with information 

about services/providers in Clay County, MN and 

Cass County, ND, was developed; 500 guides were distributed 

to area schools, physician offices, hospitals and referral 

agencies 

- A website and newsletter provide additional resources 

(www.dakmed.org/childrenshealth) 

- In May 2008, a Children’s Mental Health Fair and 

a series of three mental health trainings were held for parents, 

professionals and the general public 

- A marketing campaign utilizing the “Half A Child” logo was 

developed with the message that caring for a child’s physical 

health is only addressing “half a child” 

- A Leadership Team and Advisory Council meet on a quarterly 

basis to guide the initiative 

- The 3 rd annual Children’s Mental Health Summit was 

held in September 2008 with over 50 mental health 

professionals attending to develop 2009 strategies 

- Identified strategies include piloting a hand-held assessment tool 

to identify and address children’s mental health issues and the 

potential addition of a children’s mental health consultant 

to serve as a central assessment and referral resource 

Covering Kids and Families (CKF) 

Statewide 

2003-present 

CKF began as a four-year $1.3 million partnership between DMF 

and The Robert Wood Johnson Foundation. The goal of the initiative 

is to connect uninsured North Dakota children to existing free/ 

low-cost health coverage programs. 

- DMF has invested $500,000 

- DMF is partnering with North Dakota Caring Foundation to 

continue annual stakeholder meetings, outreach activities and 

marketing of the 1-877 KIDS NOW helpline resources 

including an annual back-to-school campaign, statewide 

television ads, radio ads, billboards and distribution of 

low-income coverage informational flyers to schools 

Dental Access 

REGIONAL 

2001-Present 

Provides ongoing support to volunteer dentist urgent care clinic in 

Moorhead, MN. 


- The Urgent Care Dental Clinic continues to provide critical 

dental care services to uninsured/low-income 

individuals 

• 42 dentists and 8 dental specialists volunteer their time at 

the clinic and have performed 10,765 dental procedures on 

4,050 patients from 160 regional communities since 2002 

• Dental services provided total nearly $600,000 

• To meet the growing demand for services, a dental 

director/staff dentist will be hired in January 2009

Diabetes Prevention 

Grand Forks, ND area 

2004-2008 

Altru Health System’s diabetes prevention pilot is designed to 

prevent the onset of diabetes in persons at-risk for developing the 

disease. DMF grant funding expired in 2008; Altru intends to continue 

the pilot and gather information that will be shared with DMF. 

Components of the pilot will be incorporated into DMF’s new Cass 

Clay Healthy People Initiative. 

- DMF has invested nearly $1 million in diabetes 

prevention/management grants since 1996 and $450,000 in 

the diabetes prevention initiative 

- Outcomes: 

• 2,500 people received diabetes prevention education 

• 41 community screenings held with 900 screened; 7% 

were positive for pre-diabetes 

• 165 persons with pre-diabetes enrolled in 12 diabetes 

prevention groups 

• Only six participants (4%) converted to diabetes vs. the 

national rate of 10% 

• Goal: 80% have blood pressure of 130/80 or less after 

five years 

Outcome: 65% have blood pressure under 130/90, with 

90% under 140/90 

• Goal: 50% achieve 5 lb. weight loss after two years 

Outcome: 59% have sustained 5 lb. weight loss after two 

years; 46% have sustained 7 lb. weight loss after two years 

Note: Significant weight loss was challenging for most 

participants 

• Blue Cross Blue Shield of ND is covering group visits and 

medical nutrition therapy for participants; they also 

funded a $106,000 18-month weight management pilot 

that includes reimbursement for exercise therapy for 

25 participants with body mass index over 35 

• The group medical visit concept was very successful, 

averaging 70% attendance, and has been duplicated in 

other Altru departments due to its cost-effectiveness 

• New program developed for children had 

disappointing participation results 

• Educational newsletter is distributed quarterly to 

3,000 individuals/agencies 

• Grand Forks Family Medicine residents assisted with 

group visits, thus educating them about the importance 

of identifying and addressing pre-diabetes in their patients 

• UND College of Nursing students developed a diabetes 

prevention program for UND Native American students 

and four tribal sites 

DMF Lend A Hand 

Cass County, ND/Clay County, MN 

2007-Present 

The DMF Lend A Hand program supports and improves fundraising 

efforts to help residents of Cass County, ND and Clay County, MN 

experiencing financial hardship due to a medical crisis. 


- Five local churches participate in a pilot program 

- Resources include matching funds, online donation 

capabilities and a fundraising toolkit with planning and 

marketing templates 

- In 2008, more than $300,000 was raised through volunteer

Lend A Hand makes a difference in people’s lives: 

• Kayla Dumas, who is making steady progress after a nearfatal 

automobile crash a year ago. In June, numerous 

volunteers, including students from Fargo North 

High School, joined 600 people at a benefit for Kayla. 

Everyone in the family was overwhelmed by the 

generosity and Lend A Hand’s support. Clay Whittlesey, 

volunteer, said, “The good will, mentally and physically, 

did so much for the family.” 

• Abby Schmidt, whose 1988 Marion, ND girls’ basketball 

teammates and other friends organized a benefit to help 

her financially survive breast cancer. Abby’s Fargo 

residency made her eligible for Lend A Hand matching 

funds, but the benefit was held in her hometown of 

Marion, ND. Having Lend A Hand’s guidance and 

motivation, plus online fundraising, made a huge 

difference, according to Abby and her friends.

Healthcare Workforce Shortage

DMF has invested more than $1.5 million; strategies include 

supporting the Grand Forks Family Medicine Residency program, 

increasing Bachelor of Science in Nursing (BSN) education slots, 

access to distance education, simulation equipment placement, 

certified nursing assistant (CNA) training and health career 

scholarship programs. 

- $375,000 grant to UND College of Nursing, Grand Forks, ND 

($125,000-DMF, $250,000-Robert Wood Johnson Foundation 

grant) to address nurse recruitment/retention, nurse 

faculty workforce, distance education and recruitment of 

disadvantaged students 

- $520,500 invested to expand BSN nursing slots by a total of 66 

slots per year at Tri-College University, Fargo, ND and

Go Red North Dakota 

Women’s Heart Health Initiative 

Statewide 

2006-Present 

DMF granted $1.25 million over three years to the American 

Heart Association to implement a statewide women’s heart health 

initiative which educates women and healthcare professionals about 

women’s heart health risks to ultimately reduce the incidence of 

heart disease in women. Interventions include a statewide education 

campaign, health professional education, and targeted heart health 

interventions in Fargo, Jamestown, and Bismarck, ND. 

Outcomes to-date: 

- 22 Go Red community events held, with more than 4,000 

women participating; 21 of 22 communities offered free or 

low- cost heart health screenings in 2008 vs. 11 of 19 in 2007 

- Four tribal Go Red events held with cultural educational 

materials distributed 

- 10,000 North Dakota women joined the Go Red movement 

online at www.americanheart.org; nearly 2,000 completed the 

online heart check-up 

- A statewide marketing campaign delivered more than one 

million radio/television media impressions to increase 

awareness of Go Red and heart disease in women 

• 87% of women surveyed recognize heart disease as the 

leading cause of death for women 

• Awareness of the Go Red for Women Initiative has increased 

from 47% to 71% 

- Seven continuing medical education (CME) presentations 

made to 600 healthcare professionals; an additional 500 

healthcare professionals were educated at various conferences 

- The ND Student Nursing Association incorporated Heart 

Disease Prevention in Women Guidelines into North Dakota’s 

nursing education curriculum 

- Intervention/Control Group Study: 15 trained wellness 

coaches lead 19 small groups: 380 women in intervention 

group and 296 in control group from eleven participating 

worksites 

• Physical activity continues to be much better in the 

intervention group 

• Body mass index (BMI) and body weight remained much 

lower in the intervention group 

• HDL “good” cholesterol increased in the intervention 

group; decreased in control group 

• Retention rates: 50% for intervention group; 42% for

- Number of Medicare recipients served has decreased from 

80% in 2005 to 40% in 2008

Worksite Wellness Pilot

DMF has contracted with Impact Foundation to manage a pilot 

program designed to create a model which will measurably improve 

the health of our state’s workforce. Impact Foundation partners with 

Altru Health System in Grand Forks, ND to administer 

the program. 

- DMF has invested nearly $200,000 

- Pilot sites include Grand Forks Public Schools, 

Grand Forks, ND and Jamestown College, West Country 

Products and Anne Carlsen Center, all in Jamestown, ND 

- Aggregate results include an average: 

• 3 lb. weight loss 

• 4.5 point improvement in systolic blood pressure rate 

• 1.66 point improvement in diastolic blood pressure rate 

• .63% decrease in body mass index (BMI) 

• Improvements in nutrition, fitness and stress 

management noted

A Healthcare Home 

for the Underserved 

The mission of the Family HealthCare 

Center, Fargo, ND, is to provide quality familycentered 

healthcare that is accessible, culturally 

competent, and efficient. 

Family HealthCare Center (FHC) 

is a private, nonprofit healthcare provider. 

They are part of a national network of 

Federally Qualified Health Centers 

(FQHCs) – organizations devoted to 

providing healthcare services regardless of 

the patient’s ability to pay. FHC provides 

a variety of primary healthcare services 

including medical, dental, lab, x-ray and 

pharmacy. Medical services include family 

practice, gynecological, obstetrical, pediatric, 

and internal medicine (including geriatric 

medicine). 

FHC has served patients in our community 

for 15 years. They have developed the 

expertise and infrastructure necessary to 

care for underserved patients and provide 

a medical home for more than 11,000 

patients. The majority of their patients come 

from low-income families, struggling to 

access healthcare due to lack of coverage or 

inadequate health insurance. 

FHC’s services are based on documented 

community need. FHC has an active 

Patient Advisory Council that reports both 

to a Leadership Team and the Board of 

Directors. The Council bridges the gap 

between patients and providers and allows 

the voice of the customer to be heard. 

In 2007, FHC served over 5,200 uninsured 

patients. They also experienced a significant 

increase in the number of “underinsured” 

patients served. Underinsured patients have 

health insurance, but the policies have very 

high deductibles or excluded services that 

they cannot afford. Most uninsured patients 

are in working families, working more than 

one part-time job. Greater numbers of 

uninsured and underinsured are accessing 

healthcare at FHC. Approximately 17,000 

people in our community do not have health 

insurance or a healthcare home. To address 

their needs, FHC offers patients a sliding 

fee scale program based on household 

size and gross income, allowing patients to 

access affordable quality health and dental 

services. 

At FHC, patients have a healthcare home. 

They have access to personal medical and 

dental providers who provide continuous 

and comprehensive care and coordinate 

services for patients across the entire 

healthcare system. They strive to improve 

patient health outcomes and manage chronic 

conditions, thereby reducing unnecessary 

hospitalizations and emergency room visits. 

FHC’s programs and services have a 

direct impact on the well-being of our 

community – they play a key role in keeping 

our community healthy by maintaining a 

healthy workforce, activating the economic 

engine, promoting wellness, helping patients 

self-manage their conditions, and providing 

cost-efficient healthcare services. 

Serving the underserved is often 

complicated and requires adequate resources. 

DMF and FHC have had a close relationship 

since the Foundation’s inception, with a 

joint mission to improve access to healthcare 

services for all. DMF has provided over $1.3 

million to FHC since 1996, supporting their 

operations and growth and giving FHC the 

opportunity to serve more patients with 

better services. 

Patricia Patron, FHC’s executive director, 

shares: “FHC works collaboratively with 

other healthcare providers and community 

partners in an effort to serve vulnerable 

populations in our community. With 

mounting decreases in federal and state 

funding, DMF’s support has been critical to 

ensuring access to comprehensive healthcare 

services for our patients. We are thankful 

for the support DMF has offered FHC 

and proud of the role DMF plays in our 

community.”

plays in our community in providing a 

medical home to those in need!

individuals and families. 

Camp Grassick was started in 1928 as a 

ND Anti-Tuberculosis outdoor living program 

for undernourished children. It was 

named for Dr. James Grassick, a pioneer 

physician in North Dakota. In 1947, the ND 

Elks Association purchased Camp Grassick 

and worked with the ND Easter Seal Society 

to provide a summer camp program for children 

with speech/language problems or physical 

disabilities. In the early 1970s, the Elks 

assumed responsibility for the program and 

have operated the camp ever since. 

A staff of qualified, caring and concerned 

therapists, counselors, instructors and 

support staff are hired each year to work with 

children and adults from across the state of 

North Dakota and bordering cities. In 2008, 

campers from 34 of 53 North Dakota counties 

participated. A nominal camp fee is charged, 

but no one is ever turned away. Campers 

enjoy the usual camp experiences such as 

singing around a campfire, pontoon rides, 

swimming and craft activities, but they also 

participate in programming aimed to build 

their speech, language and independent living 

skills while enhancing their self-esteem. 

Dan Mimnaugh, the camp’s enthusiastic 

director, has dedicated his life to the camp. 

Each year, Dan shares wonderful stories about 

the camp’s impact – like the mother who 

wrote: “My daughter always feels like an outcast 

at school but the camp makes her feel like 

she belongs,” and another mom, whose two 

sons have attended camp for the past three 

years, writes, “Thank you for the most positive 

experience my children ever have!” 

Camp Grassick serves various age groups 

and populations through six different sessions 

including: 1) a three-week therapy camp 

addressing speech, language, occupational 

therapy and reading skills, 2) a two-week 

camp focusing on peer interaction and social 

skills, 3) a one-week camp for Jamestown’s 

Anne Carlsen Center residents, 4) a transitional 

independent living skills camp for ages 

17-21, 5) a one-week adult camp for physically 

or mentally-challenged persons and 6) a 

ND Association of the Blind summer camp. 

Elks Camp Grassick served 107 students and 

79 adults during the six 2008 sessions. 

Camp Grassick is funded by Elks membership 

contributions, camper registration fees, 

charitable donations and grants. In 1980, over 

33,000 ND Elks supported the camp. By the 

late 1990s, membership in the Elks dropped 

dramatically, raising concern for the future of 

Elks Camp Grassick. To address this decline 

in membership and related support, a diverse 

mix of funding sources was sought. A grant 

was submitted to Dakota Medical Foundation 

and, recognizing the value of Camp Grassick’s 

mission, DMF provided a three-year $330,000 

grant to fund camp operations while the Elks 

worked to build a $3 million endowment to 

sustain camp operations. 

DMF also worked with the Elks to develop 

and implement a strategic plan and provided 

fundraising training through Impact 

Foundation’s Institute. Since 1999, DMF has 

provided over $1.3 million in grants to Camp 

Grassick, including several match grants to 

boost the Elks’ fundraising efforts. Camp 

Grassick’s leadership has identified their 

challenges and “stepped up to the plate”. 

As a result, the camp’s future looks much 

brighter. DMF’s support is ensuring that 

Camp Grassick will touch hearts and change 

lives for years to come.

North Dakota Caring Foundation 

– Caring for Children 

The mission of Caring for Children is to 

identify and enroll eligible children, linking 

them to participating healthcare providers 

who will give them the primary and preventive 

health and dental care they need, so that 

all children will have a healthy start and 

healthy future. 

Thousands of North Dakota 

children do not receive the primary and 

preventive health and dental care they need. 

They are from low-income working families 

who cannot afford health insurance. These 

children fall through the cracks in terms of 

receiving basic health and dental services. 

Blue Cross Blue Shield of North Dakota 

(BCBSND) founded the North Dakota Caring 

Foundation in 1989. The Foundation’s main 

focus has been the Caring for Children program, 

which provides low-income children 

with free medical, dental and mental health 

care coverage. 

An estimated 11,000 North Dakota children 

do not receive basic healthcare. Primary 

and preventive health and dental care is 

something all children need and deserve. 

Since 1989, Caring for Children has 

connected 7,000 North Dakota children to 

health coverage. They address the need for a 

medical and dental home and provide routine 

medical care for children whose families do 

not qualify for government-funded programs 

such as Medicaid or the State Children’s 

Health Insurance Program (SCHIP), but 

cannot afford private insurance. Without 

Caring for Children, these kids would 

go without health coverage, resulting in an 

absence of preventive healthcare and an 

increased risk for serious health problems. 

Annually, Caring for Children provides 

coverage to about 1,100 North Dakota children. 

In 2008, 5,351 different health services 

were provided to children, totaling $476,833. 

This amount included 291 hospital and 

1,391 dental services. 

Lisa Faul, program director, shares the 

story of a single mom whose 4 year-old 

daughter enrolled in Caring for Children’s 

health coverage program: 

In her own words: 

“I was a single mother of a wonderful 4 yearold, 

Madilyn. I was attending graduate school 

and was employed part-time as a graduate 

assistant. There were no benefits available, 

and although I was on other assistance (child 

care and food stamps), I still wasn’t eligible 

for subsidized health care for my daughter. 

I was less than $100 over the eligibility limit. 

The social service agency referred me to the 

Caring for Children program for those who 

“fall between the cracks.” I was definitely one 

of these people! 

My primary concern for my daughter at 

the time was her severe tooth decay. She was 

literally spitting out chunks of her teeth and the 

surgery to help her was nearly half of my yearly 

income. Before we were covered by Caring for 

Children, I was told I would need to pay all of 

this out of pocket because, at the time, I did not 

have adequate insurance. Once I was enrolled 

in Caring for Children, I was blessed with the 

ability to get the surgery she needed and have 

been ever grateful. In addition to the surgery, 

I was also able to provide my daughter with 

the preventive care she deserved and felt like a 

better mother because of it. 

After getting through this difficult time in 

my life, I have since completed graduate school 

and have been blessed to find a great job with 

wonderful benefits. In addition, I felt the need 

to give back to the same program that helped 

me through this challenging time in my life. 

I have since given monthly to this program 

and am ever grateful to the wonderful, caring 

people who helped me through this.” 

The cost to provide a child with one year 

of health coverage is $369.90 or $30.80 per 

month. Caring for Children operates solely 

on charitable donations and is supported by 

individuals, businesses, community organizations 

and church groups. Caring for 

Children partners with seven of the nine 

North Dakota United Way agencies. A partnership 

with Vision Service Plan’s Sight for 

Students makes it possible for enrolled children 

to receive vision services and eyeglasses 

at no cost once per year. They also work with 

the ND Department of Human Services to 

find and enroll children in programs with 

one simplified application process. This has 

greatly increased the number of applications 

received. BCBSND, in cooperation 

with Dental Service Corporation of North 

Dakota, contributes all administrative costs. 

As a result, 100% of funds are used to 

provide benefits to children. 

Since 2002, Dakota Medical Foundation 

has been a major sponsor of Caring for 

Children, providing nearly $1.3 million in 

support. Caring for Children’s mission to 

provide direct access to primary healthcare 

for children will continue to play a key role 

in DMF’s strategy to improve childrens’ 

access to healthcare.

Donor Advised Funds enable the donor 

to establish a fund or account at the 

Foundation and recommend grants

non-binding; however, the Foundation

DMF SCHOLARSHIP FUND 

Scholarship Funds are created by 

individuals, families, schools, and other 

organizations to provide financial 

assistance to qualified students in pursuit 

of higher education in health-related 

fields. These funds are often established 

in memory or in honor of an individual 

to commemorate that person’s life, 

achievements, or dedication to a particular 

cause. The Foundation generally works with 

institutes of higher education to administer 

scholarship grant funds. The donor 

may select institutions to administer the 

scholarship grant funds as well as develop 

criteria for the scholarship award. DMF 

does not charge a fee to manage these funds. 


Scholarship Fund 

The Mack V. Traynor, M.D. Scholarship 

Fund was established by Dr. Traynor’s 

children to commemorate his lifelong 

commitment to delivering high quality 

healthcare services to his patients. 

The fund provides scholarship grants to one

The Foundation’s cash and 

investment portfolio decreased its market 

value during the last twelve months to 

$92,005,540 from $118,950,631 including 

expenditures for fiscal year ending 

September 30, 2008 totaling $6,019,636. 

The Foundation’s investment strategy 

resulted in a (17.9%) return for the 

year, down from the prior year’s 10.7%. 

On recommendations by our financial 

advisors, the Foundation established a 

board-approved target asset allocation in 

2007 that reflects a long-term focus and 

diversification commensurate with other 

similar foundations and public institutional 

investors. The target allocation strategy of 

55% equities, 35% fixed income, and 10% 

alternative investments should allow the 

portfolio to grow over the long-term, taking 

advantage of future market conditions to 

improve the portfolio returns at acceptable 

risk levels. The Foundation continued with 

a manager-of-managers approach to the 

management of its investment portfolio 

during fiscal year 2008, as recommended 

by our financial advisors. This strategy 

provides the DMF portfolio with a broad 

diversification of multiple investment 

managers, styles, and securities.
