Do we educate people to be physically active and nutritionally fit, thus preventing chronic<br />

disease? Or should we fund health programs that address early intervention? Perhaps<br />

working with nonprofits to provide a safety net for uninsured/underinsured individuals,<br />

or partnering with higher education and other entities to tackle the growing shortage of<br />

healthcare providers, especially in rural areas?<br />

According to a 2008 survey of DMF members, area nonprofits and health providers, along<br />

with regional data, it’s “yes, to all of the above”! To determine how we best improve health,<br />

DMF’s Strategic Planning Committee was tasked with weaving the above themes into our<br />

2009-2011 Strategic Plan. To do this, they identified two main areas of health intervention:<br />

Prevention and Access.<br />

Historically, we’ve worked with prevention programs such as Go Red North <strong>Dakota</strong> to<br />

educate women about preventing heart disease. Through Altru Health System’s Diabetes<br />

Prevention Program, we identified persons at risk of diabetes and enrolled them in a<br />

prevention program. Our Worksite Wellness pilot assessed the health risks of employees<br />

and implemented interventions to reduce risk. Access to healthcare has been addressed<br />

through the Prescription Assistance Program; Automated External Defibrillator, Dental<br />

Access, and Healthcare Workforce Shortage initiatives; and DMF’s Lend A Hand program.<br />

DMF’s Children’s Mental Health Initiative is working hard to improve early identification,<br />

intervention and access to mental health services for children ages 0-8.<br />

In 2009-2011, we’ll launch a new prevention program - the Cass Clay Healthy People Initiative,<br />

a community approach to encouraging active living and healthy eating. Our worksite wellness<br />

program will consider new partnerships to bring prevention and early intervention programs<br />

to more worksites across our region. Improved access to healthcare services will be achieved<br />

through implementation of effective healthcare workforce development strategies. Major<br />

conferences for professionals and the general public will deliver critical health education to<br />

our region.<br />

The future is exciting for <strong>Dakota</strong> Medical Foundation, as well as our nonprofit partners! A<br />

$2 million Bush Foundation grant to our strategic partner, Impact Foundation’s Institute<br />

for Innovation and Effectiveness, will continue to help build the capacity of nonprofits to<br />

more effectively improve health. Expanded Institute services include nonprofit board and<br />

executive leadership training, as well as other statewide capacity building education and<br />

training opportunities.<br />

How do we best ensure your good health? We will identify barriers to “good” health and<br />

work strategically with our partners to eliminate them. We will assess the capacity needs of<br />

health-related nonprofits and help address them. And we will listen to you – our members,<br />

our partners, and our communities. Together, we can achieve our ambitious goals!<br />

of cardiac arrest in order to improve mortality rates.<br />

- DMF has invested over $1 million*<br />

Children’s Mental Health<br />

CASS COUNTY, ND/CLAY COUNTY, MN<br />

2007-Present<br />

To facilitate the development of community resource tools,<br />

community education and collaborative efforts related to children’s<br />

mental health services.<br />

- DMF has invested $142,000*<br />

- Coordinator hired in 2007<br />

- A children’s mental health resource guide with information<br />

about services/providers in Clay County, MN and<br />

Cass County, ND, was developed; 500 guides were distributed<br />

to area schools, physician offices, hospitals and referral<br />

agencies<br />

- A website and newsletter provide additional resources<br />

- In May 2008, a Children’s Mental Health Fair and<br />

a series of three mental health trainings were held for parents,<br />

professionals and the general public<br />

- A marketing campaign utilizing the “Half A Child” logo was<br />

- A Leadership Team and Advisory Council meet on a quarterly<br />

CKF began as a four-year $1.3 million partnership between DMF<br />

and The Robert Wood Johnson Foundation. The goal of the initiative<br />

is to connect uninsured North <strong>Dakota</strong> children to existing free/<br />

low-cost health coverage programs.<br />

- DMF has invested $500,000<br />

- DMF is partnering with North <strong>Dakota</strong> Caring Foundation to<br />

continue annual stakeholder meetings, outreach activities and<br />

marketing of the 1-877 KIDS NOW helpline resources<br />

including an annual back-to-school campaign, statewide<br />

television ads, radio ads, billboards and distribution of<br />

low-income coverage informational flyers to schools<br />

Provides ongoing support to volunteer dentist urgent care clinic in<br />

Moorhead, MN.<br />

- DMF has invested $655,000*<br />

- The Urgent Care Dental Clinic continues to provide critical<br />

dental care services to uninsured/low-income<br />

• 42 dentists and 8 dental specialists volunteer their time at<br />

the clinic and have performed 10,765 dental procedures on<br />

4,050 patients from 160 regional communities since 2002<br />

• Dental services provided total nearly $600,000<br />

• To meet the growing demand for services, a dental<br />

director/staff dentist will be hired in January 2009<br />

Diabetes Prevention<br />

Grand Forks, ND area<br />

2004-2008<br />

Altru Health System’s diabetes prevention pilot is designed to<br />

prevent the onset of diabetes in persons at-risk for developing the<br />

disease. DMF grant funding expired in 2008; Altru intends to continue<br />

the pilot and gather information that will be shared with DMF.<br />

Components of the pilot will be incorporated into DMF’s new Cass<br />

Clay Healthy People Initiative.<br />

- DMF has invested nearly $1 million in diabetes<br />

prevention/management grants since 1996 and $450,000 in<br />

the diabetes prevention initiative<br />

- Outcomes:<br />

• 2,500 people received diabetes prevention education<br />

• 41 community screenings held with 900 screened; 7%<br />

were positive for pre-diabetes<br />

• 165 persons with pre-diabetes enrolled in 12 diabetes<br />

prevention groups<br />

• Only six participants (4%) converted to diabetes vs. the<br />

national rate of 10%<br />

• Goal: 80% have blood pressure of 130/80 or less after<br />

five years<br />

Outcome: 65% have blood pressure under 130/90, with<br />

90% under 140/90<br />

• Goal: 50% achieve 5 lb. weight loss after two years<br />

Outcome: 59% have sustained 5 lb. weight loss after two<br />

years; 46% have sustained 7 lb. weight loss after two years<br />

Note: Significant weight loss was challenging for most<br />

participants<br />

• Blue Cross Blue Shield of ND is covering group visits and<br />

medical nutrition therapy for participants; they also<br />

funded a $106,000 18-month weight management pilot<br />

that includes reimbursement for exercise therapy for<br />

25 participants with body mass index over 35<br />

• The group medical visit concept was very successful,<br />

averaging 70% attendance, and has been duplicated in<br />

other Altru departments due to its cost-effectiveness<br />

• New program developed for children had<br />

disappointing participation results<br />

• Educational newsletter is distributed quarterly to<br />

3,000 individuals/agencies<br />

• Grand Forks Family Medicine residents assisted with<br />

group visits, thus educating them about the importance<br />

experiencing financial hardship due to a medical crisis.<br />

Lend A Hand makes a difference in people’s lives:<br />

• Kayla Dumas, who is making steady progress after a nearfatal<br />

automobile crash a year ago. In June, numerous<br />

volunteers, including students from Fargo North<br />

High School, joined 600 people at a benefit for Kayla.<br />

Everyone in the family was overwhelmed by the<br />

generosity and Lend A Hand’s support. Clay Whittlesey,<br />

volunteer, said, “The good will, mentally and physically,<br />

did so much for the family.”<br />

• Abby Schmidt, whose 1988 Marion, ND girls’ basketball<br />

teammates and other friends organized a benefit to help<br />

her financially survive breast cancer. Abby’s Fargo<br />

residency made her eligible for Lend A Hand matching<br />

funds, but the benefit was held in her hometown of<br />

Marion, ND. Having Lend A Hand’s guidance and<br />

motivation, plus online fundraising, made a huge<br />

difference, according to Abby and her friends.<br />

Go Red North <strong>Dakota</strong><br />

Women’s Heart Health Initiative<br />

2006-Present<br />

DMF granted $1.25 million over three years to the American<br />

Heart Association to implement a statewide women’s heart health<br />

initiative which educates women and healthcare professionals about<br />

women’s heart health risks to ultimately reduce the incidence of<br />

heart disease in women. Interventions include a statewide education<br />

campaign, health professional education, and targeted heart health<br />

interventions in Fargo, Jamestown, and Bismarck, ND.<br />

Outcomes to-date:<br />

- 22 Go Red community events held, with more than 4,000<br />

women participating; 21 of 22 communities offered free or<br />

low- cost heart health screenings in 2008 vs. 11 of 19 in 2007<br />

- Four tribal Go Red events held with cultural educational<br />

materials distributed<br />

- 10,000 North <strong>Dakota</strong> women joined the Go Red movement<br />

online at www.americanheart.org; nearly 2,000 completed the<br />

online heart check-up<br />

- A statewide marketing campaign delivered more than one<br />

million radio/television media impressions to increase<br />

awareness of Go Red and heart disease in women<br />

• 87% of women surveyed recognize heart disease as the<br />

leading cause of death for women<br />

• Awareness of the Go Red for Women Initiative has increased<br />

from 47% to 71%<br />

- Seven continuing medical education (CME) presentations<br />

made to 600 healthcare professionals; an additional 500<br />

healthcare professionals were educated at various conferences<br />

- The ND Student Nursing Association incorporated Heart<br />

program designed to create a model which will measurably improve<br />

the health of our state’s workforce. Impact Foundation partners with<br />

Altru Health System in Grand Forks, ND to administer<br />

the program.<br />

- DMF has invested nearly $200,000<br />

- Pilot sites include Grand Forks Public Schools,<br />

Grand Forks, ND and Jamestown College, West Country<br />

Products and Anne Carlsen Center, all in Jamestown, ND<br />

- Aggregate results include an average:<br />

• 3 lb. weight loss<br />

• 4.5 point improvement in systolic blood pressure rate<br />

• 1.66 point improvement in diastolic blood pressure rate<br />

• .63% decrease in body mass index (BMI)<br />

A Healthcare Home<br />

for the Underserved<br />

The mission of the Family HealthCare<br />

Center, Fargo, ND, is to provide quality familycentered<br />

healthcare that is accessible, culturally<br />

competent, and efficient.<br />

Family HealthCare Center (FHC)<br />

is a private, nonprofit healthcare provider.<br />

They are part of a national network of<br />

Federally Qualified Health Centers<br />

(FQHCs) – organizations devoted to<br />

providing healthcare services regardless of<br />

the patient’s ability to pay. FHC provides<br />

a variety of primary healthcare services<br />

including medical, dental, lab, x-ray and<br />

pharmacy. Medical services include family<br />

practice, gynecological, obstetrical, pediatric,<br />

and internal medicine (including geriatric<br />

medicine).<br />

FHC has served patients in our community<br />

for 15 years. They have developed the<br />

expertise and infrastructure necessary to<br />

care for underserved patients and provide<br />

a medical home for more than 11,000<br />

patients. The majority of their patients come<br />

from low-income families, struggling to<br />

access healthcare due to lack of coverage or<br />

inadequate health insurance.<br />

FHC’s services are based on documented<br />

community need. FHC has an active<br />

Patient Advisory Council that reports both<br />

to a Leadership Team and the Board of<br />

Directors. The Council bridges the gap<br />

between patients and providers and allows<br />

the voice of the customer to be heard.<br />

In 2007, FHC served over 5,200 uninsured<br />

patients. They also experienced a significant<br />

increase in the number of “underinsured”<br />

patients served. Underinsured patients have<br />

health insurance, but the policies have very<br />

high deductibles or excluded services that<br />

they cannot afford. Most uninsured patients<br />

are in working families, working more than<br />

one part-time job. Greater numbers of<br />

uninsured and underinsured are accessing<br />

healthcare at FHC. Approximately 17,000<br />

people in our community do not have health<br />

insurance or a healthcare home. To address<br />

their needs, FHC offers patients a sliding<br />

fee scale program based on household<br />

size and gross income, allowing patients to<br />

access affordable quality health and dental<br />

services.<br />

At FHC, patients have a healthcare home.<br />

They have access to personal medical and<br />

dental providers who provide continuous<br />

and comprehensive care and coordinate<br />

services for patients across the entire<br />

healthcare system. They strive to improve<br />

patient health outcomes and manage chronic<br />

conditions, thereby reducing unnecessary<br />

hospitalizations and emergency room visits.<br />

FHC’s programs and services have a<br />

direct impact on the well-being of our<br />

Serving the underserved is often<br />

complicated and requires adequate resources.<br />

DMF and FHC have had a close relationship<br />

since the Foundation’s inception, with a<br />

joint mission to improve access to healthcare<br />

services for all. DMF has provided over $1.3<br />

million to FHC since 1996, supporting their<br />

operations and growth and giving FHC the<br />

opportunity to serve more patients with<br />

better services.<br />

Elks Camp Grassick…<br />

more than just a summer camp<br />

Nestled on the sunny south shore<br />

of Lake Isabel in south central North <strong>Dakota</strong>,<br />

five miles south of Dawson, is a long-time<br />

institution of love and learning that has<br />

touched the hearts and lives of thousands of<br />

individuals and families.<br />

Camp Grassick was started in 1928 as a<br />

ND Anti-Tuberculosis outdoor living program<br />

for undernourished children. It was<br />

named for Dr. James Grassick, a pioneer<br />

physician in North <strong>Dakota</strong>. In 1947, the ND<br />

Elks Association purchased Camp Grassick<br />

and worked with the ND Easter Seal Society<br />

to provide a summer camp program for children<br />

with speech/language problems or physical<br />

disabilities. In the early 1970s, the Elks<br />

assumed responsibility for the program and<br />

have operated the camp ever since.<br />

A staff of qualified, caring and concerned<br />

therapists, counselors, instructors and<br />

support staff are hired each year to work with<br />

children and adults from across the state of<br />

North <strong>Dakota</strong> and bordering cities. In 2008,<br />

campers from 34 of 53 North <strong>Dakota</strong> counties<br />

participated. A nominal camp fee is charged,<br />

but no one is ever turned away. Campers<br />

enjoy the usual camp experiences such as<br />

singing around a campfire, pontoon rides,<br />

swimming and craft activities, but they also<br />

participate in programming aimed to build<br />

their speech, language and independent living<br />

skills while enhancing their self-esteem.<br />

Dan Mimnaugh, the camp’s enthusiastic<br />

director, has dedicated his life to the camp.<br />

Each year, Dan shares wonderful stories about<br />

the camp’s impact – like the mother who<br />

wrote: “My daughter always feels like an outcast<br />

at school but the camp makes her feel like<br />

she belongs,” and another mom, whose two<br />

sons have attended camp for the past three<br />

years, writes, “Thank you for the most positive<br />

experience my children ever have!”<br />

Camp Grassick serves various age groups<br />

and populations through six different sessions<br />

including: 1) a three-week therapy camp<br />

addressing speech, language, occupational<br />

therapy and reading skills, 2) a two-week<br />

camp focusing on peer interaction and social<br />

skills, 3) a one-week camp for Jamestown’s<br />

Anne Carlsen Center residents, 4) a transitional<br />

independent living skills camp for ages<br />

17-21, 5) a one-week adult camp for physically<br />

or mentally-challenged persons and 6) a<br />

ND Association of the Blind summer camp.<br />

Elks Camp Grassick served 107 students and<br />

79 adults during the six 2008 sessions.<br />

Camp Grassick is funded by Elks membership<br />

contributions, camper registration fees,<br />

charitable donations and grants. In 1980, over<br />

33,000 ND Elks supported the camp. By the<br />

late 1990s, membership in the Elks dropped<br />

dramatically, raising concern for the future of<br />

Elks Camp Grassick. To address this decline<br />

in membership and related support, a diverse<br />

mix of funding sources was sought. A grant<br />

was submitted to <strong>Dakota</strong> Medical Foundation<br />

and, recognizing the value of Camp Grassick’s<br />

mission, DMF provided a three-year $330,000<br />

grant to fund camp operations while the Elks<br />

worked to build a $3 million endowment to<br />

sustain camp operations.<br />

DMF also worked with the Elks to develop<br />

and implement a strategic plan and provided<br />

fundraising training through Impact<br />

Foundation’s Institute. Since 1999, DMF has<br />

provided over $1.3 million in grants to Camp<br />

Grassick, including several match grants to<br />

boost the Elks’ fundraising efforts. Camp<br />

Grassick’s leadership has identified their<br />

challenges and “stepped up to the plate”.<br />

As a result, the camp’s future looks much<br />

brighter. DMF’s support is ensuring that<br />

Camp Grassick will touch hearts and change<br />

lives for years to come.<br />

North <strong>Dakota</strong> Caring Foundation<br />

– Caring for Children<br />

The mission of Caring for Children is to<br />

identify and enroll eligible children, linking<br />

them to participating healthcare providers<br />

who will give them the primary and preventive<br />

health and dental care they need, so that<br />

all children will have a healthy start and<br />

healthy future.<br />

Thousands of North <strong>Dakota</strong><br />

children do not receive the primary and<br />

preventive health and dental care they need.<br />

They are from low-income working families<br />

who cannot afford health insurance. These<br />

children fall through the cracks in terms of<br />

receiving basic health and dental services.<br />

Blue Cross Blue Shield of North <strong>Dakota</strong><br />

(BCBSND) founded the North <strong>Dakota</strong> Caring<br />

Foundation in 1989. The Foundation’s main<br />

focus has been the Caring for Children program,<br />

which provides low-income children<br />

with free medical, dental and mental health<br />

care coverage.<br />

An estimated 11,000 North <strong>Dakota</strong> children<br />

do not receive basic healthcare. Primary<br />

and preventive health and dental care is<br />

something all children need and deserve.<br />

Since 1989, Caring for Children has<br />

connected 7,000 North <strong>Dakota</strong> children to<br />

health coverage. They address the need for a<br />

medical and dental home and provide routine<br />

medical care for children whose families do<br />

not qualify for government-funded programs<br />

such as Medicaid or the State Children’s<br />

Health Insurance Program (SCHIP), but<br />

cannot afford private insurance. Without<br />

Caring for Children, these kids would<br />

go without health coverage, resulting in an<br />

absence of preventive healthcare and an<br />

increased risk for serious health problems.<br />

Annually, Caring for Children provides<br />

coverage to about 1,100 North <strong>Dakota</strong> children.<br />

In 2008, 5,351 different health services<br />

were provided to children, totaling $476,833.<br />

This amount included 291 hospital and<br />

1,391 dental services.<br />

Lisa Faul, program director, shares the<br />

story of a single mom whose 4 year-old<br />

daughter enrolled in Caring for Children’s<br />

health coverage program:<br />

In her own words:<br />

“I was a single mother of a wonderful 4 yearold,<br />

Madilyn. I was attending graduate school<br />

and was employed part-time as a graduate<br />

assistant. There were no benefits available,<br />

and although I was on other assistance (child<br />

care and food stamps), I still wasn’t eligible<br />

for subsidized health care for my daughter.<br />

I was less than $100 over the eligibility limit.<br />

The social service agency referred me to the<br />

Caring for Children program for those who<br />

“fall between the cracks.” I was definitely one<br />

of these people!<br />

My primary concern for my daughter at<br />

the time was her severe tooth decay. She was<br />

literally spitting out chunks of her teeth and the<br />

surgery to help her was nearly half of my yearly<br />

income. Before we were covered by Caring for<br />

Children, I was told I would need to pay all of<br />

this out of pocket because, at the time, I did not<br />

have adequate insurance. Once I was enrolled<br />

in Caring for Children, I was blessed with the<br />

ability to get the surgery she needed and have<br />

been ever grateful. In addition to the surgery,<br />

I was also able to provide my daughter with<br />

the preventive care she deserved and felt like a<br />

better mother because of it.<br />

After getting through this difficult time in<br />

my life, I have since completed graduate school<br />

and have been blessed to find a great job with<br />

wonderful benefits. In addition, I felt the need<br />

to give back to the same program that helped<br />

me through this challenging time in my life.<br />

I have since given monthly to this program<br />

and am ever grateful to the wonderful, caring<br />

people who helped me through this.”<br />

The cost to provide a child with one year<br />

of health coverage is $369.90 or $30.80 per<br />

month. Caring for Children operates solely<br />

on charitable donations and is supported by<br />

individuals, businesses, community organizations<br />

and church groups. Caring for<br />

Children partners with seven of the nine<br />

North <strong>Dakota</strong> United Way agencies. A partnership<br />

with Vision Service Plan’s Sight for<br />

Students makes it possible for enrolled children<br />

to receive vision services and eyeglasses<br />

at no cost once per year. They also work with<br />

the ND Department of Human Services to<br />

find and enroll children in programs with<br />

one simplified application process. This has<br />

greatly increased the number of applications<br />

received. BCBSND, in cooperation<br />

with Dental Service Corporation of North<br />

<strong>Dakota</strong>, contributes all administrative costs.<br />

As a result, 100% of funds are used to<br />

provide benefits to children.<br />

Since 2002, <strong>Dakota</strong> Medical Foundation<br />

has been a major sponsor of Caring for<br />

Children, providing nearly $1.3 million in<br />

support. Caring for Children’s mission to<br />

provide direct access to primary healthcare<br />

for children will continue to play a key role<br />

in DMF’s strategy to improve childrens’<br />

access to healthcare.<br />

Over the 24-hour period, 1,452 donations<br />

were made totaling over $325,000! DMF<br />

provided up to $5,000 in matching funds<br />

to each participating nonprofit and incentive<br />

grant funds to the top three fundraisers.<br />

DMF’s match funding and incentive grant<br />

funds provided an additional $165,091 to<br />

the Giving Hearts Day nonprofits. (Due to<br />

Donor Advised Funds enable the donor<br />

to establish a fund or account at the<br />

Foundation and recommend grants<br />

from their fund to qualified charitable<br />

organizations. The recommendations are<br />

non-binding; however, the Foundation<br />

gives each recommendation full<br />

consideration, provided that the suggested<br />

recipient is eligible to receive such a grant.<br />

The Foundation oversees administration<br />

of the fund, allowing donors to focus on<br />

designing charitable strategies that will<br />

produce the greatest impact. A Donor<br />

Advised Fund can be established with a<br />

tax-deductible, irrevocable contribution<br />

of $10,000 or more. At this time, it is<br />

the Foundation’s practice not to charge<br />

expenses to administer these types of funds<br />

other than the direct costs of external<br />

in memory or in honor of an individual<br />

to commemorate that person’s life,<br />

achievements, or dedication to a particular<br />

cause. The Foundation generally works with<br />

institutes of higher education to administer<br />

scholarship grant funds. The donor<br />

may select institutions to administer the<br />

scholarship grant funds as well as develop<br />

criteria for the scholarship award. DMF<br />

The Foundation’s cash and<br />

investment portfolio decreased its market<br />

value during the last twelve months to<br />

$92,005,540 from $118,950,631 including<br />

expenditures for fiscal year ending<br />

September 30, 2008 totaling $6,019,636.<br />

The Foundation’s investment strategy<br />

resulted in a (17.9%) return for the<br />

year, down from the prior year’s 10.7%.<br />

On recommendations by our financial<br />

advisors, the Foundation established a<br />

board-approved target asset allocation in<br />

2007 that reflects a long-term focus and<br />

diversification commensurate with other<br />

similar foundations and public institutional<br />

investors. The target allocation strategy of<br />

55% equities, 35% fixed income, and 10%<br />

alternative investments should allow the<br />

portfolio to grow over the long-term, taking<br />

advantage of future market conditions to<br />

improve the portfolio returns at acceptable<br />

risk levels. The Foundation continued with<br />

a manager-of-managers approach to the<br />

management of its investment portfolio<br />

during fiscal year 2008, as recommended<br />

by our financial advisors. This strategy<br />

provides the DMF portfolio with a broad<br />

diversification of multiple investment<br />

managers, styles, and securities.<br />
