YOU MUST APPLY FOR THIS VISA, IN PERSON,<br />

Between MAY 20 and JUNE 14, 2013<br />

Use this <strong>checklist</strong> if:<br />

Your UNIVERSITY (as shown on your Student ID) or PERMANENT RESIDENCE (as shown on<br />

your Drivers License) is located <strong>in</strong> AZ, CO, UT, Southern CA counties (Los Angeles, San Diego,<br />

Imperial, Inyo, Kern, Orange, Riverside, San Bernad<strong>in</strong>o, San Luis Obispo, Santa Barbara,<br />

Ventura).<br />

You MUST submit your Student Visa application and documents listed <strong>in</strong> the <strong>checklist</strong> below, IN PERSON<br />

to the Los Angeles Consulate between May 20 and June 14, 2013, as it may take four to six weeks for the<br />

<strong>consulate</strong> to process your <strong>visa</strong>.<br />

<strong>of</strong> appo<strong>in</strong>tment you are schedul<strong>in</strong>g. Appo<strong>in</strong>tments fill up quickly, so please make your appo<strong>in</strong>tment<br />

Important Notes:<br />

• If you are a Full Year <strong>student</strong>, or plan on stay<strong>in</strong>g for another semester (180+ days) please see<br />

additional <strong>in</strong>structions (11-12) at the end <strong>of</strong> the <strong>checklist</strong>.<br />

• If you plan on travel<strong>in</strong>g outside the USA over the summer, please contact Lisa Anthony<br />

(lanthony@syr.edu) immediately for additional <strong>in</strong>structions.<br />

Submit the follow<strong>in</strong>g documents from the <strong>checklist</strong> below:<br />

1. Orig<strong>in</strong>al Passport AND Photocopy <strong>of</strong> Passport ID page: valid until at least June 2014 (fall<br />

<strong>student</strong>s) and December 2014 (yearlong <strong>student</strong>s). Passports must have at least one blank page to<br />

affix the <strong>visa</strong>. Passports must be signed!<br />

In addition to a national passport, Non-US citizens must present evidence <strong>of</strong> Immigration Status <strong>in</strong><br />

the US via the I-20 /IAP-66 or Alien Registration Card <strong>in</strong> and a permit to return to the United<br />

States <strong>of</strong> America <strong>in</strong> the form <strong>of</strong>: a resident card, or a <strong>student</strong>/work <strong>visa</strong> that is stamped on the<br />

passport and is valid for a period <strong>of</strong> three months beyond the expected length <strong>of</strong> stay <strong>in</strong> the<br />

Schengen area, or the period <strong>of</strong> validity <strong>of</strong> the <strong>visa</strong>, whichever is longer. (N/A TO US CITIZEN).<br />

*Photocopies required as well.<br />

*Before submitt<strong>in</strong>g your passport, make an extra copy for your own records! If your passport<br />

expires before June 2014 (fall <strong>student</strong>s) and December 2014 (yearlong <strong>student</strong>s), you will need<br />

to renew it us<strong>in</strong>g an expedited service. Details <strong>of</strong> this can be found here:<br />

2. I.D. card and copy <strong>of</strong> either <strong>of</strong> the follow<strong>in</strong>g: US Driver’s license, US State ID card,<br />

Voter’s Registration Card, or current Student ID.


3. Two National Application forms (orig<strong>in</strong>al + copy): Application Forms must be completed <strong>in</strong><br />

black or blue <strong>in</strong>k, and <strong>in</strong> pr<strong>in</strong>t (not cursive). Before be<strong>in</strong>g copied, the application form must<br />

be signed at the bottom <strong>of</strong> page 4 (please f<strong>in</strong>d application form <strong>in</strong>structions below).<br />

4. 2 color passport sized photos: Passport photos can be taken at any K<strong>in</strong>ko’s, Walgreens,<br />

CVS, etc. Photos must be RECENT with white background and pr<strong>of</strong>essionally taken.<br />

Photos must be GLUED to each application form <strong>in</strong> the top right corner. DO NOT<br />

STAPLE PHOTOS TO APPLICATION FORMS!<br />

5. Letter <strong>of</strong> acceptance as a full time <strong>student</strong> from Spa<strong>in</strong>'s University/School or US program: You<br />

MUST contact Lisa Anthony for these letters, at least two weeks prior to your <strong>visa</strong><br />

appo<strong>in</strong>tment, so they may be sent to you.<br />

6. Roundtrip airl<strong>in</strong>e reservation or it<strong>in</strong>erary: USA – Spa<strong>in</strong> – USA.<br />

7. Pro<strong>of</strong> <strong>of</strong> f<strong>in</strong>ancial means dur<strong>in</strong>g your stay: <strong>SU</strong> <strong>Abroad</strong> will provide the follow<strong>in</strong>g:<br />

• Letter from the University or School <strong>in</strong> Spa<strong>in</strong> or <strong>in</strong> the USA assum<strong>in</strong>g full f<strong>in</strong>ancial<br />

responsibility dur<strong>in</strong>g your stay.<br />

8. Pro<strong>of</strong> <strong>of</strong> Health Insurance: Provide Orig<strong>in</strong>al Letter guarantee<strong>in</strong>g full International Health<br />

<strong>in</strong>surance at a m<strong>in</strong>imum <strong>of</strong> €30,000 Euros, <strong>in</strong>clud<strong>in</strong>g the necessary Repatriation Coverage<br />

for the total period <strong>of</strong> stay <strong>in</strong> Spa<strong>in</strong>. Please contact Lisa Anthony at least two weeks prior<br />

to your <strong>visa</strong> appo<strong>in</strong>tment at lanthony@syr.edu, if you need <strong>SU</strong> <strong>Abroad</strong> to provide you<br />

with this letter.<br />

9. US Postal Service Money Order payable to the “Consulate General <strong>of</strong> Spa<strong>in</strong>” for $160.00<br />

(US citizens), $125.96 (Canadian Citizens) or $78.60 (most non-US citizens). For nationals<br />

<strong>of</strong> Russia, Ukra<strong>in</strong>e, Macedonia, (ARYM), Montenegro, Serbia, Albania and Moldavia the<br />

fee is $45.85. Do not submit cash, personal checks, or bank checks because the Consulate<br />

will not accept them. These can be purchased at any US Post Office.<br />

10. Pre-paid “US Express Mail” Envelope from the US Postal Service*, Self-Addressed with $19.95 <strong>in</strong><br />

stamps. Once the <strong>visa</strong> is affixed to your passport (<strong>visa</strong> process<strong>in</strong>g time takes about 6 weeks), you<br />

can either go pick it up <strong>in</strong> person or choose this option, where the Spanish Consulate will mail your<br />

passport back to you us<strong>in</strong>g this envelope. In the addressee section <strong>of</strong> your mail<strong>in</strong>g label, write the<br />

address where you expect to be over the summer. In the “Sender” portion <strong>of</strong> the label, please write:<br />

Full Year Students (180+ days) ONLY:<br />

11. Medical Certificate (orig<strong>in</strong>al and photocopy): Doctor’s recent statement (no older than 3<br />

months) <strong>in</strong> doctor’s or medical center letterhead, stat<strong>in</strong>g: “This medical certificate attests<br />

that Mr./ Mrs. ……………………… does not suffer from any illness that would pose a threat<br />

to public health accord<strong>in</strong>g to the International Health Regulations <strong>of</strong> 2005.” The medical


certificate must be signed by a M.D. It must also bear the <strong>of</strong>ficial stamp <strong>of</strong> the adm<strong>in</strong>ister<strong>in</strong>g<br />

center.<br />

*The orig<strong>in</strong>al must be translated <strong>in</strong>to Spanish and submitted along with a photocopy, contact<br />

a legal translator.<br />

12. Certification <strong>of</strong> Absence <strong>of</strong> Police Records (orig<strong>in</strong>al and photocopy): FBI or State<br />

Police Crim<strong>in</strong>al Background Check legalized with the “Apostille <strong>of</strong> the Hague<br />

Convention” from the U.S. A police record must be obta<strong>in</strong>ed (crim<strong>in</strong>al background check)<br />

from all non EU countries where you have lived for the past five years for more than 6<br />

months, legalized by the Spanish Consulate or Embassy <strong>in</strong> the respective countries. If the<br />

Certificate is issued outside the USA it must be duly legalized by the correspond<strong>in</strong>g<br />

Consulate <strong>of</strong> Spa<strong>in</strong>. LOCAL POLICE BACKGROUND CHECK WILL NOT BE<br />

ACCEPTED.<br />

*The orig<strong>in</strong>al must be translated <strong>in</strong>to Spanish and submitted along with a photocopy,<br />

contact a legal translator.<br />

*FBI background checks can take 8-12 weeks to get back from the FBI.<br />
