STUDENT VISA CHECKLIST
CONSULATE OF SPAIN IN LOS ANGELES
YOU MUST APPLY FOR THIS VISA, IN PERSON,
Between MAY 20 and JUNE 14, 2013
Use this checklist if:
Your UNIVERSITY (as shown on your Student ID) or PERMANENT RESIDENCE (as shown on
your Drivers License) is located in AZ, CO, UT, Southern CA counties (Los Angeles, San Diego,
Imperial, Inyo, Kern, Orange, Riverside, San Bernadino, San Luis Obispo, Santa Barbara,
Ventura).
You MUST submit your Student Visa application and documents listed in the checklist below, IN PERSON
to the Los Angeles Consulate between May 20 and June 14, 2013, as it may take four to six weeks for the
consulate to process your visa.

You must have an appointment to submit your student visa application. To make an appointment, please

of appointment you are scheduling. Appointments fill up quickly, so please make your appointment
EARLY!
Important Notes:
• If you are a Full Year student, or plan on staying for another semester (180+ days) please see
additional instructions (11-12) at the end of the checklist.
• If you plan on traveling outside the USA over the summer, please contact Lisa Anthony
(lanthony@syr.edu) immediately for additional instructions.
Submit the following documents from the checklist below:
1. Original Passport AND Photocopy of Passport ID page: valid until at least June 2014 (fall
students) and December 2014 (yearlong students). Passports must have at least one blank page to
affix the visa. Passports must be signed!
In addition to a national passport, Non-US citizens must present evidence of Immigration Status in
the US via the I-20 /IAP-66 or Alien Registration Card in and a permit to return to the United
States of America in the form of: a resident card, or a student/work visa that is stamped on the
passport and is valid for a period of three months beyond the expected length of stay in the
Schengen area, or the period of validity of the visa, whichever is longer. (N/A TO US CITIZEN).
*Photocopies required as well.
*Before submitting your passport, make an extra copy for your own records! If your passport
expires before June 2014 (fall students) and December 2014 (yearlong students), you will need
to renew it using an expedited service. Details of this can be found here:

2. I.D. card and copy of either of the following: US Driver’s license, US State ID card,
Voter’s Registration Card, or current Student ID.

3. Two National Application forms (original + copy): Application Forms must be completed in
black or blue ink, and in print (not cursive). Before being copied, the application form must
be signed at the bottom of page 4 (please find application form instructions below).
4. 2 color passport sized photos: Passport photos can be taken at any Kinko’s, Walgreens,
CVS, etc. Photos must be RECENT with white background and professionally taken.
Photos must be GLUED to each application form in the top right corner. DO NOT
STAPLE PHOTOS TO APPLICATION FORMS!
5. Letter of acceptance as a full time student from Spain's University/School or US program: You
MUST contact Lisa Anthony for these letters, at least two weeks prior to your visa
appointment, so they may be sent to you.
6. Roundtrip airline reservation or itinerary: USA – Spain – USA.
7. Proof of financial means during your stay: SU Abroad will provide the following:
• Letter from the University or School in Spain or in the USA assuming full financial
responsibility during your stay.
8. Proof of Health Insurance: Provide Original Letter guaranteeing full International Health
insurance at a minimum of €30,000 Euros, including the necessary Repatriation Coverage
for the total period of stay in Spain. Please contact Lisa Anthony at least two weeks prior
to your visa appointment at lanthony@syr.edu, if you need SU Abroad to provide you
with this letter.
9. US Postal Service Money Order payable to the “Consulate General of Spain” for $160.00
(US citizens), $125.96 (Canadian Citizens) or $78.60 (most non-US citizens). For nationals
of Russia, Ukraine, Macedonia, (ARYM), Montenegro, Serbia, Albania and Moldavia the
fee is $45.85. Do not submit cash, personal checks, or bank checks because the Consulate
will not accept them. These can be purchased at any US Post Office.
10. Pre-paid “US Express Mail” Envelope from the US Postal Service*, Self-Addressed with $19.95 in
stamps. Once the visa is affixed to your passport (visa processing time takes about 6 weeks), you
can either go pick it up in person or choose this option, where the Spanish Consulate will mail your
passport back to you using this envelope. In the addressee section of your mailing label, write the
address where you expect to be over the summer. In the “Sender” portion of the label, please write:

*Please, do not send any other type of envelope (e.g. US Priority, DHL, UPS, FedEx, etc.) as the
Spanish Consulate will not accept it.
*Envelopes without a tracking number will not be accepted

Full Year Students (180+ days) ONLY:
11. Medical Certificate (original and photocopy): Doctor’s recent statement (no older than 3
months) in doctor’s or medical center letterhead, stating: “This medical certificate attests
that Mr./ Mrs. ……………………… does not suffer from any illness that would pose a threat
to public health according to the International Health Regulations of 2005.” The medical

certificate must be signed by a M.D. It must also bear the official stamp of the administering
center.
*The original must be translated into Spanish and submitted along with a photocopy, contact
a legal translator.
12. Certification of Absence of Police Records (original and photocopy): FBI or State
Police Criminal Background Check legalized with the “Apostille of the Hague
Convention” from the U.S. A police record must be obtained (criminal background check)
from all non EU countries where you have lived for the past five years for more than 6
months, legalized by the Spanish Consulate or Embassy in the respective countries. If the
Certificate is issued outside the USA it must be duly legalized by the corresponding
Consulate of Spain. LOCAL POLICE BACKGROUND CHECK WILL NOT BE
ACCEPTED.
*The original must be translated into Spanish and submitted along with a photocopy,
contact a legal translator.
*FBI background checks can take 8-12 weeks to get back from the FBI.

but your permanent address is in another country, OR if you are a non-US citizen and your
