GRAND OPENING CONTEST RULES<br />
SAM ASH MUSIC 34th STREET GRAND OPENING PRIZES. To introduce you <strong>to</strong> our magnificent new 34th Street location in the heart<br />
of Manhattan, we are giving away a prize every ten minutes at the s<strong>to</strong>re during the Grand Opening on March 7, 2013.<br />
No purchase or payment needed <strong>to</strong> enter or win. A purchase will not increase your odds of winning.<br />
YOU MUST BE PRESENT AT SAM ASH MUSIC, 333 West 34th Street, NY, NY AT the time of the prize drawing <strong>to</strong> win that prize.<br />
One prize per cus<strong>to</strong>mer. One entry per cus<strong>to</strong>mer. CUSTOMER WILL BE DISQUALIFIED IF THEY SUBMIT MORE THAN ONE ENTRY.<br />
HOW TO ENTER:<br />
• Fill out an entry form at our Grand Opening on March 7, 2013 at the <strong>Sam</strong> <strong>Ash</strong> <strong>Music</strong> S<strong>to</strong>re at 333 West 34th Street, New York, NY,<br />
and deposit it in our drawing box.<br />
• Entries must be submitted in person during our Grand Opening on March 7, 2013, <strong>to</strong> be included in the contest. You must be 18<br />
or older <strong>to</strong> enter. If under 18, a parent or guardian must be present. Only one entry will be permitted per person. New drawings<br />
will be held throughout the day.<br />
• On March 7, 2013, we will select 60 winners from all contest entries. The winners will be selected at random from all entries by a<br />
judge (the “Judge”) selected by the <strong>Sam</strong> <strong>Ash</strong> companies from its staff members. The Judge’s decision on all matters will be final<br />
and non appealable.<br />
• We may post winners on SAMASHMUSIC.COM. The winners give us the right <strong>to</strong> use his or her name and likeness in our advertising<br />
in any media.<br />
• The winner must be present <strong>to</strong> win. If the contestant is not present when a winning entry is drawn, the entry will be disqualified<br />
and another entry selected. The winner will be required <strong>to</strong> provide a fully completed and signed contest winner affidavit in order<br />
<strong>to</strong> claim the prize.<br />
• The <strong>Sam</strong> <strong>Ash</strong> companies (“<strong>Sam</strong> <strong>Ash</strong>”), and the companies supplying the prizes, will be given e-mail address and other information<br />
about contest entrants. Contest entrants may be contacted with promotional e-mail and mail by Sponsor, <strong>Sam</strong> <strong>Ash</strong>, and the<br />
suppliers of the contest prize.<br />
• We reserve the right <strong>to</strong> reject any entry for violation of contest rules and <strong>to</strong> modify the contest if required by law all as determined<br />
by the Judge.<br />
• If the winner does not provide the signed and completed contest winner affidavit, the winner will be disqualified and the winner<br />
will be the next entry selected by the Judge at random.<br />
• <strong>Sam</strong> <strong>Ash</strong> is not responsible for misdirected or illegible entries all of which will be rejected at the discretion of the Judge.<br />
Game is open <strong>to</strong> legal residents of the United States over the age of 18, except for the employees and immediate family (spouses and<br />
parents, siblings, children and each of their spouses) of Sponsor, <strong>Sam</strong> <strong>Ash</strong>, the prize supplier, their affiliates, subsidiaries, advertising<br />
agencies and any other company involved in the design, production, execution or distribution of the sweepstakes drawings. All<br />
sweepstakes entries become the property of the Sponsor and <strong>Sam</strong> <strong>Ash</strong> and will not be returned.<br />
ODDS OF WINNING: Odds of winning depend on the number of eligible entries received. 45,000 entries are being issued and only<br />
60 prizes will be awarded.<br />
IF YOU WIN: The verified prize winners will be required <strong>to</strong> sign an Affidavit of Eligibility and Publicity/Liability Release before collecting<br />
a prize. Winners release the Sponsor, <strong>Sam</strong> <strong>Ash</strong>, the prize suppliers, and each of their affiliates, officers, agents, and employees<br />
from any responsibility or liability in connection with any loss, accident, or death incurred in connection with the use of or the installation<br />
of the prizes won in the giveaway. Each winner hereby consents <strong>to</strong> the use of his or her name and/or likeness by the <strong>Sam</strong> <strong>Ash</strong><br />
companies, and the prize suppliers for advertising purposes without additional compensation unless prohibited by law. Failure <strong>to</strong><br />
comply may result in disqualification and the selection of an alternate. Any tax liabilities are the responsibility of the winner only and<br />
the winner will be required <strong>to</strong> provide his or her US social security number for tax purposes. <strong>Prize</strong> components are not exchangeable,<br />
transferable, or redeemable for cash. <strong>Sam</strong> <strong>Ash</strong> does not make any warranty (including fitness for particular purpose) or guarantee<br />
with regard <strong>to</strong> any prize. If any of the stated prizes is not available, <strong>Sam</strong> <strong>Ash</strong> may substitute an item of comparable value.<br />
