4. Monthly Sales Volume Commitment
I understand and agree to choose a minimum monthly Sales Volume commitment of
340 RMB, and will be charged Preferred Customer Pricing.
4. Backup Order
Backup Order –Only for Preferred Customers.
When you’re away on vacation or business, don’t meet your minimum monthly Sales
Volume commitment, or just forget to place your order, and you don’t have a remaining
balance in your Preferred Customer Account or Credit Account, the Backup Order will
be sent automatically, keeping you in good standing to meet monthly Sales Volume ,to
receive your Preferred Customer 30-40% discount and other Preferred Customer
benefits. (If not fill in the products you selected, we will send you standard Backup
Order)

doesn’t reach specified amount, and you don’t have a remaining balance in your

6.Payment Authorization
For your convenience, you may use your individual debit card or credit card for payment.
Such bank account must be of the applicant himself/herself. This authorization is only to
allow Melaleuca to deduct money from the account for all orders including backup orders.

Customer Account.

7.Applicant’s guarantee and statement
I am of the legal age (18 years of age and above), I have carefully read and agree to
the Melaleuca Special Preferred Customer Agreement Terms and Conditions on the
Back of this Application Form. If the filling in this application form is not of integrity or
the attachment is not of completeness, this agreement will be regarded as invalid. If I
want to retreat or revise this agreement and the attachment, they are not to be
returned. This Melaleuca Special Preferred Customer Agreement is not valid unless
signed. If I don’t update personal data in time, Melaleuca doesn’t have any
responsibility about loss.
I understand and agree to provide hard copies of social card and bank account details
to Melaleuca when applying to be a Preferred Customer.
I guarantee that all the information I provided is true and effective, and the form is filled
in and signed in my free will. And I will be liable for any legal responsibility.
If there’re alterations of my individual information, I have a duty to inform Melaleuca as
soon as possible.
I may cancel this Agreement for any reason at any time by giving written notice to
Melaleuca bearing my original signature, printed name, address, and Social ID
Number. I understand that Written cancellations received by Melaleuca on or before
the 25th of the month will be effective the month received. Written cancellations
received by Melaleuca after the 25th of the month will be effective the following month.

Agreement
1.How to join “Melaleuca SPC”
The company welcomes anybody to join “Melaleuca SPC” and purchases our high quality product.
Only need to purchase the SPC Kit, which includes but not limited to: ‘Melaleuca Country (products
catalog)’, ‘SPC handbook’, 'Melaleuca SPC Card'& 'Discovery Melaleuca'(DVD), with total 20 RMB of
production cost of the card (SPC Kit hereto shall be deem to be construed as an integral part of this
contract, after customer take possession of the card, or purchase exceeds 30 days, we will not allow
customer to retract).
I have carefully read, totally accepted and signed thisAgreement.
Satisfy the following requirements, and it will be called Preferred Customer. Enjoy the Preferred Customer
benefits:
a)Keep minimum monthly Sales Volume commitment is more than 340 RMB;
b)One-month backup order(340 RMB);and become Melaleuca Preferred Customer.
Satisfy the following requirements, and it will be called Special Preferred Customer. Enjoy the Special
Preferred Customer benefits:
a)Keep minimum monthly Sales Volume commitment is more than 340 RMB;
b)Provide individual valid Credit Card and authorize Melaleuca to deduct monthly payment from this
account; or Two-month backup order(340 RMB);
Note: Melaleuca only accept Couple jointly application, and issue a same one “Melaleuca SPC card”. The
first applier is valid.
7.Error Correction Measure
If Melaleuca SPC violates the obligation of this agreement, other Melaleuca policy or any other illegal
act, fraud or deceit, Melaleuca will take following one or multiple measures:
1.Verbal warning;
2.Written warning and proclaim;
3.Pause purchase right;
4.Freeze SPC rights in whole or in part;
5.Cancel membership;
6.Other measures prescribed in other Melaleuca policy and regulation;

3.Backup Order Agreement
I agree and totally accept the item of “Backup Order Agreement” in this Agreement.
If any product in the Backup Order (include Pack-Products selected by oneself) becomes unavailable is
out of stock, I autherize Melaleuca to substitute another product or products of equal or greater value to
ensure that I meet my minimum monthly Sales Volume commitment. If there is any price differential for
substituted product(s), I authorize Melaleuca to deduct the above differential.
if I need to modify or cancel my Backup Order, I shall fill in the “Modify/Cancel Backup Order Notice” by
myself, and submit it to Melaleuca Service Center or mail to Melaleuca. I must send all data to Melaleuca
by/on 25th of every month, and it will become effective in this month. After/on 26th, it will be become
effective at next month.
4.Select Pack/Backup Order Substitutions
In the event that any product, or if the Product Points for any products I have selected are reduced, I
authorize Melaleuca. I further authorize Melaleuca to change my account for any price difference for the
substituted product(s). If I am not satisfied with the substitute product(s), I may deal with it according to
「satisfaction guaranteed」.
8.Term
The term of this agreement is without day and will remain in effect until membership termination;
This Agreement will suspend automatically for SPC wouldn’t maintain monthly sales volume commitment;
If I want to terminate the membership, I shall submit “Melaleuca SPC Termination Agreement” to
Melaleuca personally
4.Payment Authorization
I authorize Melaleuca to withdraw payment from the specified account directly for my backup orders, or
the orders I place directly.
I guarantee the specified bank account is legally applied and use, not existing any criminal offenses,
which includes but not limited to counterfeit, remark, malicious overdraft, money laundering. Otherwise, I
will be liable for all legal responsibility, and it’s not concerned in Melaleuca;
I will be liable any adverse consequence caused by the wrong or fate bank account information I fill in,
and it’s not concerned in Melaleuca;
Melaleuca has the right to cancel the order and refuse delivering for my bank account without remaining
account or invalid cause Melaleuca withdraw the payment on time or in full;
I shall not apply the bank to refuse payment. If it occurs wrong deduction, I shall inform Melaleuca within
30 days from the date on which I find the wrong.
9. Other
Any dispute arising from, it shall be settled through friendly consultation between the two Parties.
Otherwise any Party may choose to resolve the disputes by submitting such dispute to Shanghai
Arbitration Committee.
This Agreement shall come into force upon the affixation of the company chop and the signature of the
authorized representative of each party. This Agreement is written in two originals, one for each party.
5. Change/Return Product
For the detail of Melaleuca change/return product and 100% satisfaction guarantee, please see the
Appendix “SPC Kit”.
6. SPC Obligation
I will not display or resell Melaleuca product(s) through internet or any other way;
I will not use trademark, shop name, domain name system and other intellectual property in any way.
I will not discuss Melaleuca or release any Melaleuca related advertisement through the media;
I will not do anything harmful to other customer recommended by the Direct Sales of Melaleuca;
When Melaleuca Member introduces Melaleuca product(s) to others, it shall not contain any exaggerated
or false advertising;
Melaleuca Member could apply for Melaleuca Direct Seller, and recommend final customer. However, the
maximum monthly expenditure of every final customer shall not exceed RMB 2500.
For my personal data I presented, Melaleuca shall take the confidential responsibility. Melaleuca Member
also agrees that I will take the confidential responsibility for the commercial secrets acknowledged during
enjoying member rights.
