Mitacs Liability Waiver Form

in relation to my participation in the Program, whether due to the negligence of any of Mitacs
Inc., the University, or any Company, or any director, officer, employee, agent, contractor or
student of any of them (collectively, the “Releasees”) or otherwise.
This agreement will remain in effect despite the conclusion or termination of my participation in the
Program for any reason whatsoever.
This letter is for the benefit of each of Mitacs Inc., the University any Company, and the fact that any of
them has not signed this letter will not limit their rights to rely upon it.
My participation in the Program is contingent upon my agreement to the above provisions, and I
acknowledge that if I do not agree to these provisions, then my selection as a participant in the program
will be immediately revoked.
Further, I agree that this letter will be governed by and interpreted under the laws of the province in
which the University is located and the laws of Canada applicable therein.
ACKNOWLEDGEMENT/AGREEMENT
I acknowledge and agree to all of the terms and conditions set out in the above letter from MITACS.
