Symposium on <strong>Advances</strong> <strong>in</strong> <strong>Pharmacy</strong><br />

<strong>Practice</strong>, a cont<strong>in</strong>u<strong>in</strong>g pharmacy<br />

educational program, developed for pharmacists to be kept<br />

up-to-date on pharmacy practice. This annual symposium<br />

provides <strong>the</strong> most current cont<strong>in</strong>u<strong>in</strong>g education on newly<br />

approved drugs presented by <strong>the</strong> foremost authorities <strong>in</strong><br />

<strong>the</strong> field. Daniel A. Hussar, PhD, will present New Drugs<br />

<strong>of</strong> 2012 and Steve Sheaffer, PharmD, FASHP will present<br />

Medication Safety.<br />

The <strong>in</strong>troduction <strong>of</strong> new drugs <strong>in</strong>to <strong>the</strong> marketplace is<br />

one <strong>of</strong> <strong>the</strong> most important topics for pharmacists. For<br />

• Identify <strong>the</strong> most important adverse effects and<br />

precautions <strong>of</strong> <strong>the</strong> new drugs.<br />

• Compare <strong>the</strong> new drugs to <strong>the</strong> older <strong>the</strong>rapeutic agents<br />

to which <strong>the</strong>y are most similar <strong>in</strong> activity.<br />

• Def<strong>in</strong>e and differentiate terms related to medication safety,<br />

medication errors and safe medication practices.<br />

• Describe <strong>the</strong> <strong>in</strong>tent <strong>of</strong> REMS (Risk Evaluation and Mitigation<br />

Strategies) and <strong>the</strong>ir implications to pharmacy practice.<br />

• Apply <strong>the</strong> follow<strong>in</strong>g medication pr<strong>in</strong>ciples to community<br />

and hospital pharmacy practice: high alert medications, look<br />

alike-sound alike medications, error-prone abbreviations,<br />

root cause analysis (RCA), <strong>in</strong>dependent verification.<br />

• Identify opportunities to utilize medication reconciliation<br />

process dur<strong>in</strong>g transitions <strong>of</strong> care between <strong>the</strong> community<br />

and acute care sett<strong>in</strong>gs<br />

• Debate strategies for and benefits <strong>of</strong> document<strong>in</strong>g,<br />

benchmark<strong>in</strong>g, review<strong>in</strong>g and report<strong>in</strong>g medication events.<br />

• Describe <strong>in</strong>itiatives by private and government payers to<br />

l<strong>in</strong>k quality with improved patient outcomes.<br />
