Back Office<br />
In January 20<strong>10</strong>, the Alzheimer Societies in Peterborough and Kawartha Lakes voluntarily integrated their<br />
organizations to create a single entity. Direct service delivery was enhanced as the agency created a single<br />
administrative structure and board governance.<br />
Chronic Disease Prevention and Management<br />
<strong>Health</strong> service providers in Peterborough initiated a voluntary integration to streamline the management of<br />
vascular conditions. The Comprehensive Vascular Disease Prevention and Management Initiative<br />
involves Peterborough Regional <strong>Health</strong> Centre, the Peterborough Networked Family <strong>Health</strong> Teams, Primary<br />
<strong>Health</strong> Care Services of Peterborough, Peterborough Regional Nephrology Associates, Vascular <strong>Health</strong><br />
Network, Astra Zeneca Canada and the Heart and Stroke Foundation of Ontario and is designed to develop<br />
and track comprehensive care pathways for the identification and management of vascular conditions.<br />
Telestroke<br />
The <strong>Central</strong> <strong>East</strong> LHIN was one of four LHINs that partnered with the Cardiac Care Network and local<br />
hospitals to enhance the Provincial Telestroke Program. This facilitated integration now provides timely<br />
access for the Peterborough Regional <strong>Health</strong> Centre and Lakeridge <strong>Health</strong> Oshawa (both regional stroke<br />
centres in the <strong>Central</strong> <strong>East</strong> LHIN) with continued access to the Provincial Telestroke Network.
