Dentist Directory<br />

This directory is intended as a guide for selecting a <strong>Delta</strong> <strong>Dental</strong> Premier dentist. Selecting a<br />

dentist listed in the <strong>Delta</strong> <strong>Dental</strong> Premier directory will ensure that you receive your maximum<br />

benefits. The dentists listed in this directory are current as of the printing date shown on<br />

the bottom of each page of the directory. This means that any changes to the dentist network<br />

after this date will not be reflected in this directory.<br />

Role of Your Dentist<br />

Your dentist's job is to coordinate the dental services you receive, from preventive dental care<br />

such as exams and cleanings to dental treatments such as repairing a broken tooth, removing<br />

decay from a tooth, and/or restoring the tooth with a filling. An ongoing relationship with your<br />

dentist increases your chances of receiving the best care possible.<br />

Choosing a Dentist<br />

<strong>Delta</strong> <strong>Dental</strong> of <strong>Minnesota</strong> wants you to play an active role in decisions about your dental care .<br />

While you do not need to pre-select a dentist, selecting a dentist from the <strong>Delta</strong> <strong>Dental</strong> Premier<br />

directory will ensure that you receive the maximum benefits.<br />

In-Network: When you choose a dentist from this directory, your dental care is called<br />

“in-network” and you will receive important benefit advantages, including lower costs by receiving<br />

the maximum level of benefits available to you.<br />

Out-of-Network: If you receive dental care from a dentist not listed in this directory, your care is<br />

considered “out-of-network.” Benefit payment from an out-of-network dentist will be covered at a<br />

lower reimbursement and you will have greater out-of-pocket expenses. This is true even if the<br />

out-of-network dentist works in the same clinic or office as an in-network dentist. If dental<br />

services are received from an out-of-network dentist, reimbursement for services will be paid<br />

directly to the subscriber, and the subscriber would be responsible to pay the difference up to the<br />

submitted charge of the dentist.<br />

Remember to verify your dentist’s participation status at the time you make an appointment .<br />

Checking with your dentist or calling Customer Service to verify participation assures you that<br />

your dentist is still participating in the network. If your dentist's status has changed and he or she<br />

is no longer part of the network, you can continue seeing your dentist, however, this may result in<br />

a higher out-of-pocket cost for you. Or you may choose another <strong>Delta</strong> <strong>Dental</strong> Premier dentist<br />

and receive maximum benefits. Consult your Human Resources department or benefit booklet<br />

for more information regarding your specific plan design.
