About Your Physician and Healthcare Provider Directory<br />

This <strong>directory</strong> contains the names, addresses, and telephone numbers <strong>of</strong> contracted doctors,<br />
hospitals, and <strong>healthcare</strong> <strong>providers</strong> (including but not limited to durable medical equipment, home<br />
health care, skilled nursing facilities, radiology centers, hearing and speech therapy <strong>providers</strong>, and<br />
physical/occupational therapy, vision and dental <strong>providers</strong>) that are contracted to provide<br />
services to Today’s Options PFFS enrollees.<br />
What Is a Service Area<br />

The counties and/or the specific county zip code areas served by a <strong>Medicare</strong> Advantage plan are<br />
